면역성 혈소판감소증은 혈소판에 대한 동종 또는 자가항체에 의해 혈소판이 파괴되는 질환으로서, 동종항체는 흔히 임신 또는 수혈에 의해 야기되며 자가항체는 원인을 알 수 없는 특발성의 경우와, 자가면역질환, 약물 또는 다른 질환과 관련된다. 동종항체를 유도하는 혈소판 동종항원은 혈소판 특이창원의 종류에 따라 개인별 대립인자로서 존재하며 혈소판 표면의 서로 다른 당단백에 위치함으로써 하나의 혈소판 특이항원계(human platelet specific antigen: HPA Ⅰ-Ⅴ)를 구성한다.

혈소판 특이항원은 기본적으로 항원 단백의 구조상 아미노산의 차이에 기인하며 이는 항원성이나 항체 결합에 영향을 미칠 수 있는 것으로 보고된 바 있어 개인별 특이항원의 대립인자, 즉 HPA 다형성에 따라 혈소판항체와 관련된 면역반응의 차이가 있을 것으로 추정된다.

한편 자가항체를 유도하는 자가항원 역시 혈소판 당단백에 존재하나 그 구체적인 당단백의 종류 및 항원결정기(epitope)와 이에 따른 면역반응 또는 조절에 관한 연구는 아직 미미하다. 특히 혈소판 특이항원과의 상호 연관성에 관한 연구는 거의 없다. 혈소판 특이

항원과의 연관성을 보기 위하여는 특이 항혈청을 사용한 혈청학적 표현형 규명을 통한 방법이 있겠으나 최근에는 분자생물학적인 방법에 의한 유전자형 분석방법이 새로이 개발되고 있다.

이에 본 연구에서는 혈소판 특이항원 유전자 다형성과 혈소판감소증의 발생 빈도, 혈소판항체검사, 혈소판 수와의 상호 연관성을 관찰하기 위하여 정상대조군 160명 및 면역성 혈소판감소증 26명을 대상으로 HPA Ⅰ-Ⅴ에 대한 유전자형을 중합효소 연쇄반응 및 제한효소 절편다형성 분석법과 대림인자 특이 유전자 증폭법으로 조사하여 다음과 같은 결과를 얻었다.

1. HPA Ⅰ-Ⅴ 중 HPA Ⅰ 및 HPA Ⅳ는 대조군 및 환자군 모두에서 단일 유전자형(a+b-)으로서 다형성을 관찰할 수 없었다.

2. HPA Ⅱ의 경우, 대조군의 유전자형 빈도는 a+b-87.4%, a+b+12.0%, a-b+0.6%이었으며, 환자군의 유전자형 빈도는 a+b-93.0%, a+b+7.0%, a-b+0.0%로서 두 군간에 통계적으로 유의한 차이가 없었다.

3. HPA Ⅲ의 경우, 대조군의 유전자형 빈도는 a+b-17.6%, a+b+68.6%, a-b+13.8%이었으며, 환자군의 유전자형 빈도는 a+b-17.0%, a+b+75.0%, a-b+8.0%로서 두 군간에 통계적으로 유의한 차이가 없었다.

4. HPA Ⅴ의 경우, 대조군의 유전자형 빈도는 a+b-98.1%, a+b+1.9%, a-b+0.0%이었으며, 환자군의 유전자형 빈도는 a+b-87.5%, a+b+12.5%, a-b+0.0%로서 두 군간에 통계적으로 유의한 차이가 있었다.

5. HPA(Ⅱ, Ⅲ, Ⅴ) 유전자형에 따라 환자군의 입원기간 최저 혈소판 수와 혈소판항체 검사결과는 통계적으로 유의한 차이가 없었다.

6. HPA Ⅲ 유전자형에 따라 환자군의 혈소판감소의 중(경)등도는 통계적으로 유의한 차이가 있었다.

이상의 결과를 종합하면, 서구인에 비해 한국인에서의 HPA 유전자 다형성은 비교적 적으며 유전자형의 빈도에 있어서도 서로 차이가 있음을 알 수 있었다. 또한 정상대조군과 환자군 사이에 HPA Ⅴ의 유전자 다형성 차이가 관찰됨으로써 면역성 혈소판감소증의 발생

기전과 HPA Ⅴ 유전자형 사이에 연관성이 있음을 알 수 있었다. 이는 HPA Ⅴ가 활성화된 T 림프구의 VLA(very late antigen) 2와 구조가 동일하므로 혈소판 당단백 수용체와 세포매개 면역작용 또는 interleukin과의 상호 연관성을 간접적으로 시사하는 것으로 사료

된다.

Alloimmune-mediated platelet destruction by antibodies to platelet-specific antigens may be seen in at least three well-known clinical entities. These entities include neonatal alloimmune thrombocytopenia, posttransfusion purpura, and refractoriness to platelet transfusions. These conditions require detailed serologic analysis for immunophenotyping as well as the identification of antibodies directed at human platelet specific antigens(HPA).

According to the rapidly increasing demand of platelet transfusion in clinical medicine, knowledge of platelet immunology has rapidly expanded by the introduction of new molecular biological technology. Especially the ability to generate large amounts of cDNA from platelet-specific mRNA sequences should make possible direct molecular characterization of normal platelet proteins.

All platelet-specific alloantigens as yet investigated are due to single DNA base changes leading to single amino acid substitution at the protein level and all HPA systems to date have been found to be localized on platelet membrane glycoproteins.

HPA Ⅰ and Ⅳ are localized on GP Ⅲa and HPA Ⅲ on the heavy chain of GP Ⅱb. HPA Ⅱ is located on GP Ⅰb and GP Ⅰa carries the HPA Ⅴ antigenic epitopes.

Genotyping of five distinct diallelic human platelet alloantigen(HPA) systems, HPA Ⅰ(Pl**A , Zw), HPA Ⅱ(Ko, Sib), HPA Ⅲ(Bak, Lek), HPA Ⅳ(Pen, Yuk) and HPA Ⅴ(Br, Zav, Hc) _was performed to identify whether HPA genotype relates to the development of immune thrombocytopenia and whether HPA genotype contributes to the difference in platelet counts or platelet antibody tests in immune thrombocytopenic patients

To determine the genotype and gene frequencies for the HPA Ⅰ-Ⅴ, DNA from 160 healthy controls and 26 patients of immune thrombocytopenia were isolated from peripheral blood mononuclear leukocytes and specific sequences were amplified using polymerase chain reaction(PCR).

DNA-based RFLP analysis using PCR product for HPA Ⅰ, Ⅱ, Ⅴ and allele-specific PCR for HPA Ⅲ, Ⅳ were done. The obtained genotype frequencies of normal Korean population were 100.0%, 0.0 %, 0.0% for HPA Ⅰ a+b-, a+b+, a-b+ and 87.4%, 12.0%, 0.6% for HPA Ⅱ a+b-, a+b+, a-b+ and 17.6%, 68.6%, 13.8% for HPA Ⅲ a+b-, a+b+, a-b+ and 100.0%, 0.0%, 0.0% for HPA Ⅳ and 98.1%, 1.9%, 0.0% for HPA Ⅴ a+b-, a+b+, a-b+, respectively. And the gene frequencies were 1.000, 0.000 for HPA Ⅰ a, b and 0.934, 0.066 for HPA Ⅱ a, b and 0.520, 0.480 for HPA Ⅲ a, b and 1.000, 0.000 for HPA Ⅳ a, b and 0.990, 0.010 for HPA Ⅴ a, b respectively.

The genotype frequencies of immune thrombocytopenic patients were also done and compared with those of normal population, showing statistically significant difference in HPA Ⅴ genotype polymorphism(P=0.03). HPA Ⅴ residing on the Gp Ⅰa/Ⅱa complex is identical with the 2 heterodimer complex of VLA-2 molecules on

very late activated T lymphocytes. And the expression of VLA-2 can be up regulated on T lymphocyte in response to mitogen or antigen stimulation. The same molecular structures of HPA Ⅴ on activated T lymphocytes provide evidence for the concept of

cell mediated immunity or cytokine control of platelet receptor in immune thrombocytopenia - new and its importance has yet to be clarified in platelet immunology.

There were no statistically significant difference between genotype polymorphism of HPA Ⅰ-Ⅴ and the lowest platelet counts found during admission period of immune thrombocytopenic patients. But the same analysis concerning the severity of thrombocytopenia with the grouping of patient to mild and severe cases showed, interestingly, statistically significant difference in HPA Ⅲ system(p=0.01). In HPA Ⅲ system, amino acid dimorphisms control both the formation as well as the expression of the alloantigenic determinants as other HPA systems but the

post-translational modifications (ie, glycosylation) of the polypeptide chain may play a secondary role in the formation of the antibody-binding site on the target glycoprotein.

In conclusion, this study demonstrated the genetic association of HPA Ⅴ -in immune thrombocytopenia and suggests that the concurrent activation of cell-mediated toxicity to platelets.
