Abstract

발치후에 나타나는 치조골의 흡수에 의하여 의치의 유지력 및 지지력의 결여등이 야기되며 심한 경우에는 안모의 변형도 초래할 수 있다. 따라서 치조골의 흡수를 방지하거나 줄일 수 있는 방법에 관한 연구가 활발하게 진행되고 있으며, 이러한 연구의 일환으로서 치근을 인위적으로 치조골내에 잔존시킴으로써 치조골의 흡수를 막을수 있는가의 가능성에 대한 연구도 진행되도 있다.
이에 착안하여 본 실험에서는 근관충전한 치근, 근관확대만 한 치근, 생활치수 상태의 치근을 인위적으로 치조골 하방 2㎜에 잔존시켜 주위조직의 반응을 비교 연구하여 다음과 같은 결론을 얻었다.
1. 생활치수 상태로 치조골내에 잔존시킨 치근은 2주후부터, 근관충전을 한 후 잔존시킨 치근과 근관확대만 시행한 후 잔존시킨 치근은 4주후부터 결체조직 및 상피조직에 의하여 피개되었다.
2. 주위 치조골로부터의 신생 치조골의 형성은 생활치수 상태로 잔존시킨 치근에서는 4주후부터 관찰할 수 있었으나,근관충전후 잔존시킨 치근과 근관확대만 시행한 수 잔존시킨 치근에서는 8주 후에도 관찰할 수 없었다.
3. 근관충전후 잔존시킨 치근과 인접한 연조직에서는 유리된 근관충전용 호제에 의한 염증이 8주후에서도 관찰되었다.
4. 치주인대조직의 절단 치근면을 통한 연결은 생활치수 상태로 잔존시킨 치근에서는 2주후부터,근관확대만 시행한 후 잔존시킨 치근 및 근관충전후 잔존시킨 치근에서는 제4주후부터 관찰할 수 있었다.
이상의 사항들을 종합하여 볼 때, 일반적으로 생활치수 상태로 잔존시킨 치근군에서 치수 및 절단치근 상부의 연조직 치유와 골조직 형성이 빠른 것으로 관찰되었다.
[영문]
This study was performed to evaluate the tissue responses to submerged root with different conditions.
In this study 4 adult mogrel dogs were used. Canal-filled roots, canal-enlarged root, and vital-pulp roots were intentionally submerged at the level of 2mm below the alverolar bone. The experimental animals were sacrificed at 1,2,4, and 8 weeks after the submersion and the mandibular specimens were fixed, decalcified, and embedded according to the routine methods. Specimens were sectioned serially at 6㎲ thickness and stained with hematoxylin-eosin solution.
The following results were obtained:
1. The submerged vital-pulp roots were covered by epitheliae and connective tissue after 2 weeks, the submerged canal-filled roots and canal-enlarged roots were covered by those tissues after 4 weeks.
2. New bone formation from the adjacent alveolar bone was observed over the submerged vital-pulp roots after 4 weeks and not observed over the canal-filled and canal-enlarged roots until 8 weeks.
3. Mild inflammatory responses were observed in the connective tissue on the submerged canal-filled roots until 8 weeks.
4. The connection of the periodontal ligament over the surface of the submerged vital-pulp roots were observed after 2 weeks, and also after 4 weeks over the submerged canalfilled and canal-enlarged roots.
In summary, it was observed that the healing of the pulpal, overlying connective tissues, and new bone formation were more rapid in the group of submerged vital-pulp roots than in those of the submerged canal-filled and canal-enlarged roots.
