일부 농촌지역 노인들의 안녕상태에 대한 조사 연구 : 전라북도 옥구군 지역을 중심으로

본 연구는 일부 농촌지역 노인들의 사회적, 심리적, 신체적, 안녕상태를 조사한 기술 조사연구이다. 안녕상태의 유지가 모든 연령층에서 증요한 목표지만 평균수명의 연장과 함께 특히, 농촌지역의 급속한 노령화 현상이 사회적지원망의 미비로 소외되기 쉽고, 경제성이 낮은 농촌노인 인구에서는 절실히 필요하다고 보아 그들의 안녕 상태를 조사함으로써 농촌지역 노인의 사회적 안녕과 심리적 안녕, 신체적 안녕 상태를 파악하며, 그들의특성에 따른 신체적 안녕 상태의 차이를 분석하고자 한다.

연구 대상은 전북 옥구군 관내 보건 진료소 관할 범위 내에 거주하는 만 60세 이상의 노인 230명을 대상으로, 방문 면접을 통한 설문지 조사를 하여 분석 가능한 총 212부가 자료 분석에 사용되었으며, 자료 수집 기간은 1994년 4월 21일부터 4월 30일 까지 총 10일간 실시되었고, 보든 자료는 SAS로 전산통계처리하여 다음과 같은 결론을 얻었다.

1. 사회적 안녕의 범주에서 사회적 원조를 조사한 결과 혼자 사는 노인이 21.2%었으며,집안에서 중요한 일을 상의하는 정도는 80.2%였고, 따로 사는 자녀와 만나는 횟수로 월 2-3회 이상이 69.8%를 차지했고 가까운 자녀집에 갈 때 걸리는 시간으로는 대상 노인의 59.0%가 1시간 이내로 응답하여 자녀들로부터 지지를 받고 있음을 보여 주었다. 경제적 상태가 있어서 , 43.9%가 일을 해서 용돈을 마련하는 높은 경제 활동력을 나타냈으나 자녀에게 의지하는 경우도 37.3%를 차지하여 농촌 노인들의 경제적 상태가 좋지 않음을 나타냈으며, 환경적 상태에 있어서는, 문화시설이나 난방 상태, 부엌 구조에 있어서는 편리한 점이 많으나, 화장실 구조에 있어서는 재래식이 70.8%로 개선되어야 할 문제이다.

2. 심리적 안녕 에서는 1주일에 4-5회 이웃과 이야기하는 대상이 81.6%를 차지하며, 77.5%가 친구가 있다고 하였다. 그러나 외로움을 느끼는 경우가 83.9%로 대상 노인들의 외로움에 대한 심각성을 보여 준다.

3. 신체적 안녕에 있어서 4점 척도로 본 신체적 상태에서는 총 평균 점수가 3.33이었는데, 그 중 팔.다리.허리의 아픈 정도가 가장 좋지 않은 점수인 2.22으로 낮았고, 가래 객담 배출정도, 두통정도, 현기증 정도는 평균 3.0이하를 나타낸 반면, 뜨겁고 찬 것 구별 정도, 청력상태, 배변.배뇨상태, 혈압상태, 허리굽은 정도는 평균 3.50점 이상의 점수를 나타냈다. 노인의 할 수 있는 일상활동의 조사 결과 버스나 자동차 타기, 육교나 계단 오르내리기, 집안에서 힘든 일하기, 혼자 목욕하기, 손톱또는 발톱깍기에서는 총 평균3.20점이었고, 영양 상태에 관련된 사항들인 체중유지 상태, 식욕상태, 소화상태, 치아상태에 있어서는 총 평균 2.98점을 보였다.

4. 대상 노인의 특성에 따른 신체적 안녕 상태를 분석한 결과 유의한 차이를 보인 항목으로는 성별 (t=2.9625, p<0.01), 교육 정도(t=-5.8189, p<0.01), 과거 병력(t=3.6518, p<0.01), 정기적인 병원 검진 여부(t=-2.9233, p<0.01), 정기적인운동 여부(t=-3.7079, p<0.01)이었으며, 결혼 여부(t=2.4082, p<0.05)와 과거병력 (f=4.27, p<0.05)이었다.

이상의 결과에서 연구대상 노인들의 신체적인 안녕에서는 예방적인 측면의 간호 중재가 심리적 안녕과 사회적 안녕을 충족시킬 수 있는 사회적 지지가 필요한 것을 알 수 있었으며, 연구 결과의 일반화를 위하여 공범위한 노인을 대상으로 안녕상태 연구가 필요함을 제언한다.

This research investigated the physical, social, and mental hygiene wellbeing conditions for the elderly in a particular district of rural, Korea. I investigation was done because a study of the maintenance of well-being in the elderly was seriously needed. The elderly are also an important target age group too. The researcher used a questionnaire and interview 230 people were under the jurisdiction of the OK-KU health center, in North Cholla province.

Of the 230 interviewees, data was inadequate for 18 so that data only 212 subjects was analyzed. The data collect was done over 10 days from the April 21, to April 31, 1994. The data was analyzed using software from SAS.

The results are as follows:

1. The percentage of the elderly who lived alone with social

security was 21.2%, who were consulted in, important family matters at home was 80.2% and who meet with children who live separately 2 or 3 times in a month, 69.8%, The results showed that 59.0% of them lived within 1 hour distance from their children, that is they lived closely and with good social security. Economically, 43.9% of them earned their pocket money by working and they were very positive in economic activities. But 37.3% of them were dependent of their children, and the rural elderly were not in good economic condition. In living

conditions, they lived in a very convenient situation with convenient heating and kitchen facilities, but 70.8% of their toilets are of the conventional type, and need to be improved. 2. With regard to conditions of psychological well-being, 81.6% of them talked with neighbor 4 or 5 times a week , 77.5% of them had friends, but 84.9% of them felt lonely which was considered to be a very serious, situation.

3. With regard to conditions of physical wellbeing on the basis of a four measured points Likert scale, the subjects felt they were generally reported being healthy with a score of 3.33 points for the body, but their arms, legs and waists were not good usually as a result of pain.

Those wh felt generally good scored 3.20 points in daily life, but those who did not feel good scored 2.98 points in nutrition

4. With regard to conditions of physical well-being according to specific characteristics of the elderly, there was little difference in sex (t=2.9625, p<0.01), education(t=5.8189, p<0.01), experience sickness (t=3.6518, p<0.01), medical regular examination(t=2.9233, p<0.01) and regular excise(t=3.7079, p<0.01), married(t=2.4082, p<0.05), or for different occupations(F=4.27, p<0.05).
