Moraxella (Branhamella) catarrhalis는 상기도의 상재균으로 알려져 왔으나 소아의 급성 상악 부비동염과 급성 중이염 및 면역 기능이 저하된 환자에게는 중독한 폐렴을 일으킬 수 있음이 근년에 보고되었다. B-lactamase 생성균주의 출현과 급속한 증가로 인하여

임상 검체에서 이 세균의 분리와 신속한 감별이 필요하게 되었으나 이에 관한 연구가 적다.

이에 이 연구에서는 선택 배지 사용에 따른 M. (B.) catarrhalis의 검출율에 대한 영향과 Griffonia simplicifolia의 lectin에 의한 응집 시험과 B-lactamase 시험을 이 세균의 신속한 선별에 이용할 수 있는지를 검토하고자 하였다. 선택 배지 사용에 따른 M. (B.) catarrhalis의 분리율을 검토하기 위해서는 1992년 4월부터 1993년 2월 사이에 연세의료원 임상병리과에 세균 배양이 의뢰 된 입원 및 외래환자의 인두 및 객담 검체를 대상으로 하였으며, Vaneechoutte 배지 및 혈액 한천에 접종하여 전자는 공기 환경의, 후자는 5% CO2 환경의 35℃ 배양기에 24시간 배양하였다. M. (B.) catarrhalis의 동정은 형태와 그람염색성 및 생화학적 성상에 의하였다. G. simplicifolia의 lectin에 의한 응집반응은 여러 효소 등으로 전 처리 한 것과 전 처리하지 않은 세균을 여러 농도의 lectin 및 완충액의 ph 등 여러 시험 조건으로 슬라이드에서 시험하였다.

호흡기 검체에서 M. (B.) catarrhalis 분리율은 선택 배지를 사용할 때는 1.5%이었으나, 혈액 한천을 사용할 때는 0.4%이었고, 연령, 입원 및 외래에 따른 분리율은 비슷하였으나 겨울철이 여름철에 비해 약간 높았다. 선택 배지에서 다른 세균이 중등도 이상 증식된 경우는 4예(7.7%)에 불과하였으며, 이중 비병원성 Neisseria는 3예(5.8%)에 불과하였다. G. simplicifolia의 lectin과 M. (B.) catarrhalis의 응집반응은 약할 뿐 아니라 균부유액이 자가 응집을 일으켰으며, 시험 조건을 달리하여도 반응이 개선되지 않았다.

B-lactamase 양성 율이 M. (B.) catarrhalis는 95.8%이나 N. sicca는 3.3%이었다.

이 연구의 결과로 M. (B.) catarrhalis를 호흡기 검체에서 분리하기 위해서는 선택 배지 사용이 더 정확한 결과를 얻을 수 있으며, 선택 배지를 사용할 때는 M. (B.) catarrhalis의 동정이 용이하며, lectin 응집 시험은 선별 시험으로 사용할 수 없으나, B-lactamase 시험은 보조적인 감별 시험으로 유용하다는 결론을 얻었다.

[영문]

Moraxella (Branhamella) catarrhalis, which had been considered as nonpathogenic normal flora of upper respiratory tract, was reported to cause acute mastioiditis and otitis media of children and serious pulmonary infection in compromised adults. Although the isolation and rapid identification of the organism from clinical madeira became necessary, there are few such studies at the moment.

The purpose of the study was to determine the effect of use of selective medium on thedetection rate of the organism d\and the usefulness of the tests of slide agglutination with Gruffibua simplicifolia lectin and B-lactamase on the rapid presumptive identification of M. (B.) catarrhalis.

To determine the isolation rates of M. (B.) catarrhalis, specimens of throat and sputum taken from in-and outpatients during the period of April 1992 to February 1993 were inoculated onto Vaneechoutte ajar and blood afar and

incubated at 35℃ for 24 hours in air and 5% CO2, respectively. M. (B.) catarrhalis was identified by gram-stained cell morphology and biochemical characteristics. G. simplicifolia lectin agglutination was tested on a slide.

Various concentrations of the cell which were either retreated or untreated with various enzymes or with heat were mixed with various concentration of lectin at various test condition including different ph of buffer solution.

B-lactamase was tested by rubbing colonies on the Cefinase disk.

The detection rate of M. (B.) catarrhalis from respiratory specimens were 1.5% with Vaneechoutte medium, while it was 0.4% with blood agar.

The rates from inpatients and outpatients and from different age groups were similar, but was slightly higher in the winter months than summer months.

Only 4 (7.7%) of specimens showed moderate or heavy growth of normal flora on the selective medium. G. simplicifolia lectin and M. (B.) catarrhalis agglutination was weak even at various conditions and various pretreatment did not prevent

autoagglutination. B-lactamase positive rates were 95.8% for M. (B.) catarrhalis and 3.3% for Neisseria sicca.

In conclusion, selective media is very useful for the dection of M. (B.) catarrhalis from respiratory specimens, and use of the media renders identification of the organism easier. For the presumptive screening of the organism, lectin agglutination is not useful, but B-lactamase test can be used.
