일차적 고환기능저하증에 대한 치료로서 이용되는 합성 테스토스테론 주입은 특히 사춘

기에 장기간 주입후 초래될 수 있는 체액내 질소정체 및 골밀도 변화등 여러가지 합병증

으로 인하여 최근에는 이에 대한 보완이나 또는 새로운 치료방법의 개발이 요구되고 있다

. 따라서 본 연구에서는 안전하고 새로운 생물학적 안드로젠 대체요법으로서 생체내 Leyd

ig세포이식 가능성을 알아보고자 실험적으로 사람의 사춘기에 해당하는 생후 6주의 흰쥐

고환에서 분리한 일정수의 Leydig세포를 자가이식후 이식세포의 생존에 따른 시기별 테스

토스테론 분비능의 변화와 조직형태학적 변이과정을 관찰함으로써 다음과 같은 결과를 얻

었다.

1. 이식부위별로는 신피막하나 복강내 이식이 피하 또는 음낭내 이식에 비해 상대적으

로 높은 테스토스테론치를 보였고, 이식세포수는 테스토스테론 분비와 의의있게 비례하는

상관성을 보였다. 그러나 이식 3개월 후에는 기능적 저하를 보였다.

2. 합성 교원질에 흡수후 복강내 이식시에는 동일수의 Leydig세포를 복강내에 직접 이

식한 경우에 비해 기간별로 전반적인 테스토스테론 분비능의 향상을 보였으며, 특히 이식

3개월 후 측정된 테스토스테론치는 Leydig세포만 주입한 경우에 비해 약 2.7배 높았다.

3. (125)**I -hCG의 활성도는 정상대조군에 비해 이식후 각각 1, 2 및 3개월에 평균 37

.7%, 38.3% 그리고 21.0%로 모두 감소하였으며, 시기별 (125)**I -hCG 결합정도와 혈청

테스토스테론치는 비례하는 성향을 보였다.

4. 이식 4주후에는 부분적인 신생 모세혈관이 나타나 응집생착된 정상 형태의 Leydig세

포군을 보였다. 3개월 이후에는 세포군내에 부분적으로 미세구조적인 괴사와 퇴화 소견을

보였고 일부 세포는 핵의 농축현상과 지질색소침착의 증가를 보였으며, 골지체 및 미토

콘드리아의 수적인 감소와 함께 크기도 전반적으로 작았다.

이상에서 Leydig세포는 체액이나 혈액의 공급이 원활할 경우 이식후 4주내에 생착과 함

께 기능적 회복을 보였고, 또한 이식 세포수를 늘리거나 교원질에 흡수 이식하는 방법을

이용함으로써 단기간내에 테스토스테론 분비량을 체내에 필요한 최소한의 수준으로 올릴

수 있다고 생각되므로 특히 인체에서 사춘기부터 정상 성인 형태를 완성시까지 유용할 것

으로 기대된다. 그러나 이식 3개월 이후에는 부분적인 퇴화 및 기능 저하를 나타내고 있

어서 이에 대한 규명과 아울러 이러한 퇴행현상을 지연 또는 방지하기 위한 보다 많은 연

구가 필요하리라 생각된다.

It is ordinarilly accepted teat the synthetic testosterone administration is the

choice of treatment for the primary testicular dysfunction. But recently not a few

medical scientists reported that the long-standing testosterone replacement is not

without side effects. For that reason, we tried to investigate a possibility of in

vivo transplantation of Leydig cells as a new biologic androgen substitution

therapy. The Leydig cells procured from 6 week-old male Sprague-Dawely rat were

autotransplanted and the level of testosterone secretion and histostructural

changes were observed.

The results obtained are summarized as follows:

1. For the selection of transplantation sites, compared to subcutaneous or

scrotal counterparts, renal subcapsular and intraperitoneal transplant showed

higher levels of testosterone and the number of transplanted cells was correlated

with the level of measured testosterone.

2. Further more, if the Leydig cells were transplanted intraperitoneally after

the uptake on synthetic collagen, testosterone levels were higher than the ones

simply transplanted, resulting in 2.7 times higher at 3 months.

3. The activity of (125)**I -hCG decreased 20 to 47% at each month after

transplantation compared to the normal levels, but no statistical significance was

noted among different periods.

4. Histologic examination revealed neovascularized capillaries and well

demarcated uptake of sheetlike group of eosinophilic Leydig cells were observed at

4 weeks. But the evidence of destructive changes such as a focal inflammation with

central dystropic ossification was noted after 3 month. On electron microscopy, the

marked indentation of nucleus and presence of lipochrome pigment were seen and the

number and size of smooth endoplasmic reticulum and mitochondria were reduced.

In conclusion, testosterone output could be increased to the physiologic range by

increasing the number of transplant cells or collagen uptake utilizing, etc.

However, the decrease of testosterone production after 3 months is believed to be a

result of focal inflammation and degeneration which in turn casued the decrease in

number of secreting Leydig cells and reduced activity of hCG receptors. Further

effort is necessary on delaying or preventing the structural and functional

decrement of Leydig cells.
