성견 치주질환 발치와에 즉시임프란트 매식술시 Titanium Plasma Sprayed 임프란트와 Hydroxyapatite Coated 임프란트간의 계면조직에 관한 주사전자현미경적 연구

dc.description.abstract[한글] 치아상실 후 발생하는 저작계의 경조직 및 연조직 변형에 있어 이를 해소코자 임프란트가 치과영역에 도입된 이후로 골유착을 이용한 다양한 형태의 임프란트들이 개발되어져 왔다. 본 연구의 목적은 성견의 치주질환 이환발치와에 Titanium plasma sprayed(TPS)임프란트와 Hydroxyapatite(HA) coated 임프란트를 즉시 매식하여 두 가지 매식체간에서 골과 계면조직의 결합정도를 규명하기 위함이다. 이를 위해 성견의 소구치에 외과적 치조골 결손부를 형성한 후 교정용 탄성사를 결찰하여 치주염을 유발시진 후 12주 경과 후 각 치아를 발치하고 TPS 임프란트를 즉시 매식한 경우를 대조군으로 HA coated 임프란트를 즉시 매식한 경우를 실험군으로 하여 매식 12주 후 치유결과를 주사전자현미경으로 비교 관찰하여 다음과 같은 결론을 얻었다. 1. 치주질환 이환 발치와에서 대조군, 실험군 공히 골유착이 관찰되었다. 2. 전체 임프란트 계면에서 실험군이 대조군에 비해 보다 더 많은 골유착이 관찰되었다. 3. 임프란트 근단부 hole부위에서는 대조군, 실험군 공히 다른 계면부위에서 보다 더 많은 결합조직에 의한 부착이 관찰되었고, 임프란트와 결합된 결합조직은 불규칙하게 부착됨이 관찰되었다. 4. 실험군의 경우 HA와 골과의 결합면은 대조군의 TPS와 골과의 결합면에 비해 긴밀하게 결합되었으나, 임프란트 본체부와 HA coating은 뚜렷한 경계를 나타내는 빈 공간이 관찰되었다. 5. HA coaling은 약 50μ의 일정한 두께로 관찰되었다. Prof. Jung Kiu Chai, D.D.S., M.S.D., Ph.D) Dental implant was introduced into dentistry fur the treatment of hard and soft tissue deformation due to tooth lose. Osseointegrated implants were developed in various types. The purpose of this study is to evaluate the nature of the interface between bone and two types of implants(TPS implant, HA coated implant). In this study, TPS implants and HA coated implants were installated on periodontally involved extraction sockets of adult dogs. Experimental periodontitis was induced on premolars of adult dogs by the ligation of orthodontic threads after surgical creating periodontal defect. Thereafter following the extraction of each tooth, TPS implants and HA coated implant? were inserted into the extraction socket immediately. Control group was TPS implants, and experimental group was HA coated implants. Thereafter dogs were sacrificed at 12 weeks and the specimens were prepared for scanning electron microscopy. The conclusion of this study were as follows. 1. In both, control and experimental group, osseointegration was observed. 2. Experimental group demonstrated significantly more direct contact compared to control group in all the implant surfaces. 3. In implant apical hole area, the control and experimental group had areas of connective tissue interrupting the bone to implant contact. The connective tissue in fibrous integration area was dominated by collagen bundle running in different direction. 4. In the experimental group, bone to HA interface showed an intimate contact. The HA coating was detached from the titanium surface and some fracture lines were present inside the thickness of the HA coating. 5. HA coating was stable (50μ thickness).
