흰쥐에서 척수신경 뒤뿌리 절단 후 뒤뿔의 substance P 함유신경섬유와 calcitonin gene-rlated peptide 함유신경섬유에 대한 면역조직화학적 연구

척수신경절과 척수 뒤뿔에 분포하며 통각 전달에 관여하는 신경펩타이드로 알려진 substance P와 calcitonin gene-related Peptide(CGRP) 함유 신경종말의 차이를 규명하기 위하여 성숙한 Sprague-Dawley계 흰쥐를 이용하여 제 1 요수에서 제 1 미수까지 양쪽 척수 뒤뿌리를 절단 한 실험군과 아무런 전 처치를 하지않은 정상 대조군, 뒤 뿌리 절제술을 제외한 같은 수술적 방법을 사용한 sham operation군 간에 수술 1주∼10일 후에 제 6 요수부터 제 1 천수의 뒤뿔에 남아 있는 substance P와 CGRP의 면역반응도의 차이를 컴퓨터

를 이용한 영상분석기로 분석하여 비교하였으며, 전자현미경적 관찰을 통해 다음과 같은 결과를 얻었다.

1. 제 1 요수에서 제 1 미수까지의 양쪽 척수 뒤뿌리를 절제한 실험군에서 제 6 요수, 제 1 천수, 제 2 천수의 뒤뿔 모두에서 거의 CGRP 면역 양성 반응이 나타나지 않았다.

2. 제 1 요수에서 제 1 미수가지의 양쪽 척수 뒤뿌리를 절제한 경우, substance P 면역반응 양성 면적이 정상대조군에 비해 평균 59.5% 에서 68.2% 수준으로 저하되었고, 회색도는 정상대조군의 69%에서 81.6% 수준으로 저하되었다.

3. 전자현미경관찰에서 대조군의 CGRP 면역반응 양성 구조는 축삭과 축삭종말에서만 관찰되었으나 substance P 면역반응 양성 구조는 축삭과 축삭종말 외에 가지돌기에서도 관찰되어 척수에 내재하는 신경원에도 substance P가 함유되어 있다는 것이 확인되었다.

4. 실험군의 전자현미경 관찰에서 CGRP 면역반응 양성구조는 거의 찾아볼 수 없었으며, 실험군의 모든 분절에서 substance P 양성구조는 보다 드물게 관찰되기는 하였으나 축삭과 축삭종말, 가지돌기 모두에서 관찰되었다.

척수 뒤뿔에서 CGRP는 축삭에서만 관찰할 수 있는 반면 substance P는 축삭과 가지돌기에서 모두 관찰할 수 있었다. 일차구심성 신경 섬유의 가지돌기가 척수 뒤뿔에는 없는 점을 고려할 때 CGRP는 일차구심성섬유에만 존재하는 신경펩타이드이며 substance P는 일차

구심성섬유와 척수고유신경원에 모두 존재한다.

Among the peptide neurotransmitters and neuromodulators which are considered to play key roles in the central and peripheral nervous system, substance P has the longest history and probably is the best characterized as far as distribution, biological properties and physiological roles are concerned. Substance P is

considered to play key roles in the transmission of nociceptive impulses from the peripheral nerves to the spinal cord via primary afferent neurons. Because its distribution in the superficial layers of the spinal dorsal horn as unmyelinated or

lightly myelinated fibers, it selectively excites those cells that respond to peripheral noxious stimulation. Its depletion by neurotoxic capsaicin causes selective degeneration of small diameter sensory fibers as well as increases in the nociceptive threshold.

Calcitonin gene-related peptide(CGRP) has the same distribution pattern to those of substance P in the spinal dorsal horn and coexists in perikarya of the spinal dorsal root ganglia. Several studies have concluded that there is a synergistic interaction with substance P although the nature of interaction is still in

dispute.

It has been proven by light microscope that the origin of CGRP in the spinal dorsal horn is entirely from the Primary afferent dorsal root gan91ion cell.

Subsance P is considered to exist in both popriospinal and primary afferent fibers.

In the present study, the origin of CGRP and substance P immunoreactivity in the dorsal horn was investigated by electron microscope after dorsal rhizotomy to eliminate the primary afferent input. Fusthermore the area and gray level of the

CGRP and substance P immunoreactivity of the dorsal rhizotomized dorsal horn was determined by area densitometry on the image analyzer. The results are summerized as follows.

1. The area of the CGRP-immunolabelled dorsal horn disappeared entirely after bilateral dcrsal rhizotomy.

2. The area of the substance P-immunolabelled area decreased after dorsal rhizotorny. The average levels of L6-S2 segments were 59.5% to 68.2% of the control level. The gray level of substance P immunolabelled area also decreased. Average levels of L6-S2 segments were 59.5% to68.2% of the control level.

3. The CGRP immunolabelled profiles were found in axons and axon terminals while substance.

4. The CGRP immunolabelled profiles observed in electron microscopy disappeared almost entirely after multiple bilateral dorsal rhizotomy while substance P-immunolabelled profiles were still found in dendrites, axons, and axon terminals.

In this study the hyopothesis that the CGRP immunoreactivity in the dorsal horn of the rat spinal cord is a primary afferent fibers were confirmed in the electron microscopic level. Also part of substance P immnoreactivity in the dorsal horn after multiple bilateral dorsal rhizotomystill remained.
