중증 뇌질환 환자의 내경정맥혈중 산소양에 관한 연구

dc.description.abstract[한글] 뇌사(cerebral death)판정 방법의 하나로 내경동맥혈과 내경정맥혈간의 산소함유량(Vol.%)의 차이를 측정하였다. 각종 중증 뇌질환 환자 19명중 총 20회에 걸쳐 측정한 결과 그 평균치는 신뢰도 95%에서 0.733∼1.699 Vol.%로 산출되었다. 즉 중증뇌질환 환자에서는 내경동맥혈과 내경정맥혈간의 산소함유량의 차이가 적었으며, 또한 그 차이가 1.7 Vol.% 이하인 경우에는 뇌사(cerebral death )라고 간주 할 수 있었다. 이상의 실험 성적을 문헌 고찰과 함께 그 가능한 기전들에 대해 검토하여 보았다. [영문] Hyperoxia of internal jugular venous blood seems to be thought as possible one of the laboratory criteria in brain death, easily taken at bedside. Recently many authors have been augumented that the difference of oxygen content between the internal carotid arterial and internal jugular venous blood is a little in the comatose patients, especially more in brain death. We have studied the difference of oxygen content between internal carotid arterial and internal jugular venous blood in 20 cases of 10 patients of various servere ontracranial lesions. The mean value and standard error of it was 1.216±0.231 Vol.% and at 95 confidence its area was 0.733 to 1.699 Vol.%. Therefore the author speculate that the level lower than 1.7 Vol.% of arterio-jugular venous oxygen content difference is brain death. Possible mechanism of high oxygen content in the jugular venous blood in the state of brain death are discussed.-
