난시 교정에 있어 렌즈의 가감이 시력에 미치는 영향

dc.description.abstract[한글] 난시는 시력장애 뿐만 아니라 안정피로를 이르키는 중요한 원인의 하나다. 난시의 정도가 경할 때는 이를 시광학적으로 완전히 교정하는 것은 별 문제가 없으나 정도가 심한 경우에는 어린아이에서는 이를 전부 교정하여 줄 때 잘 적응할 수 있으나 어른에서는 적응하지 못하는 경우가 있다. 또한 양안의 난시도의 차이가 심할때는 렌즈의 광학적 중심부 이외의 주변부를 통하여 주시하면 인위적으로 사위가 발생하며 양안의 난시의 도와 축이 같은 때라도 원주렌즈의 경선에 따라 상의 확대에 차이가 있어 망막에 맺히는 상이 의곡된다. 이와같은 경우에는 적응이 가능한 연소자를 제외하고는 처음부터 난시를 완전히 교정하여 주는 것보다는 원주렌즈의 도를 감소시켜 덜 교정된 상태에서 적응시킨 후 점차 원주렌즈의 도를 증가시켜주는 것이 바람직하다. Copeland (1928)는 원주렌즈의 도를 감소시키는 방법으로 Sturm의 원추체 중에서 최소착란원을 망막위에 위치하게 하는 spherical equivalent법을 고안하였으며 Prangen (1941)과 Eggers (1946)는 치소착란원의 위치에서는 상은 명확하지는 않으나 의곡이 없으므로 spherical equivalent법을 사용하여 원주렌즈의 도를 감소시키면 비교적 좋은 교정시력을 얻을 수 있으며 적응도 쉽다고 하였다. 그러나 Sallman (1930)은 일반적으로 시시표의 문자나 모양은 수직선이 명확히 보이면 비교적 쉽게 판독할 수 있는 경우가 많기 때문에 직난시에서는 spherical equivalent법을 사용하여 원주렌즈의 도를 감소시켜 최소착란원을 망막위에 위치하게 하는 것보다 구면렌즈의 도는 변화시키지 않고 원주렌즈의 도만 감소시켜 후초선을 망막위에 위치하게 하는 것이 오히려 더 좋은 교정시력을 얻을 수도 있다고 하였다. 저자는 1981년 6월부터 10월까지 연세대학교 의과대학 부속세브란스병원을 방문한 환자중 난시성굴절이상이 있는 각종의 굴절이상환자로 굴절이상 이외에는 시력장애를 일으킬만한 안질환이나 눈의 변화가 없으며 교정시력이 20/20이 나오는 42명의 54안을 대상으로하여 spherical equivalent법 을 사용하여 원주렌즈의 도를 감소시키는 방법과 구면렌즈의 도는 변화시키지 않고 원주렌즈의 도만 감소시키는 방법을 사용하여 난시의 종류와 감소시킨 정도에 따라서 얻은 교정시력을 조사하여 다음과 같은 결과를 얻었다. 1. 직난시에서 spherical equivalent법을 사용하여 원주렌즈의 도를 감소시킨 경우 평균교정시력은 0.50 D 감소시켰을 때 0.877, 1.00 D 감소시켰을 때 0.716, 1.50 D 감소시켰을 때 0. 593, 2.00 D 감소시켰을 때 0.551로 저하되었다. 2. 도난시에서 spherical equivalent법을 사용하여 원주렌즈의 도를 감소시켰을 경우 평균교정시력은 0.50 D 감소시켰을 때 0.838, 1.00 D 감소시켰을 때 0.683, 1.50 D 감소시켰을 때 0.584, 2.00 D 감소시켰을 때 0.343으로 저하되었다. 3. 직난시에서 구면렌즈의 도는 변화시키지 알고 원주렌즈의 도만 감소시키는 방법으로 원주렌즈의 도를 감소시켰을 경우 평균교정시력은 0.50 D 감소시켰을 때 0.815, 1.00 D 감소시켰을 때 0.608, 1.50 D 감소시켰을 때 0.629, 2.00 감소시켰을 때 0.433으로 저하되었다. 4. 도난시에서 구면렌즈의 도는 변화시키지 않고 원주렌즈의 도만 감소시키는 방법으로 원주렌즈의 도를 감소시켰을 경우 평균교정시력은 0.50 D 감소시켰을 때 0.766, 1.00 D 감소시켰을 때 0.479, 1.50 D 감소시켰을 때 0.403, 2.00 D 감소시켰을 때 0.370으로 저하되었다. 5. 원주렌즈의 도를 감소시키는 방법에 따른 교정시력을 비교하여 보면 직난시에서 1.50 D 감소시켰을 때와 도난시에서 2.00 D 감소시켰을 때를 제외하고 spherical equivalent법을 사용하여 원주렌즈의 도를 감소시키는 방법이 구면렌즈의 도는 변화시키지 않고 원주렌즈의 도만 감소시키는 방법보다 교정시력이 더 좋았으며 그 차이는 통계학적 유의성이 있었다. 6. 직난시와 도난시 사이의 원주렌즈의 도를 감소시키는 방법에 따른 교정시력의 차이를 비교하여 보면 구면렌즈의 도는 변화시키지 않고 원주렌즈의 도만 감소시키는 방법에서 도난시가 직난시보다 시력저하가 많이 나타났으며 spherical equivalent법을 사용하여 감소시키는 방법에서는 직난시가 도난시보다 교정시력이 더 좋았으나 통계학적 유의성은 없었다. [영문] Astigmatism is one of the most important cause that is concerned with asthenopia and visual disturbance. Children usually accept the full correction of high astigmatic errors because they may be able to tolerate. The correction of larger astigmatic errors, however, necessarily produce appreciable difficulties for adult. Any asymmetry between the correction of the two eyes leads to an artificial heterophoria on looking any direction other than through the optical centers of the two lenses. This considerable upset of binocular vision is further aggravated even in symmetrical correction by inclination and distortion in the retinal image. Such disturbances may be eliminated by under-correction of the astigmatic errors. The usual method of under-correction is that the spherical equivalent of the refractive correction must be kept constant when reducing cylinder power, a concept based on the Sturm's conoid and the supposition that the visual acuity is best when the "circle of least confusion" falls on the retina. Although this is probably true in many cases, a patient with "with the rule" astigmatism may obtain best visual acuity with vertical focal line of Sturm's conoid falling on the retina simply because more of our customary test letters are recognizable when the vertical strokes are clear. This study is to observe the discrepancy between the visual acuity obtained by the spherical equivalent of refractive correction and the visual acuity obtained by reducing cyclinder power without changing sphere power. The results were as follow: 1. In "with the rule" astigmatism, each corrected visual acuity obtained by spherical equivalent of refractive correction was 0.877, 0.716, 0.593 and 0.551 respectively by reducing cylinder power of 0.50D, 1.OOD, 1.50D and 2.000. 2. In "against the rule" astigmatism, each corrected visual acuity obtained by spherical equivalent of refractive correction was 0.838, 0.683, 0.584 and 0.343 respectively by reducing cylinder power of 0.50D, 1.00D, 1.50D and 2.00D. 3. In "with the rule" astigmatism, each corrected visual acuity obtained by reducing cylinder power without changing sphere power was 0.815, 0.608, 0.629 and 0.433 respectively by reducing cylinder power of 0.50D, 1.00D, 1.50D and 2.00D. 4. In "against the rule" astigmatism, each corrected visual acuity obtained by reducing cylinder power without changing sphere power was 0.766, 0.479, 0.403 and 0.370 respectively by reducing cylinder power of 0.50D, 1.00D, 1.50D and 2.00D. 5. Corrected visual acuity obtained by the spherical equivalent of refractive correction was significantly better than that only by reducing cylinder power. 6. In the spherical equivalent of refractive correction, there was no significant difference between the corrected visual acuity of "with the rule" astigmatism and that of "against the rule" astigmatism. But when cylinder power is reduced without changing sphere power, corrected visual acuity of "against the rule" astigmatism was more significantly decreased than that of "with the rule" astigmatism.-
