한국성인 여성에서 SN-MP각의 차이에 수반되어 나타나는 두부 및 안면부 골격의 변화에 관한 연구

한국성인 여성에 있어서 SN-MP각의 변화에 따른 두부 및 안면부 골격에서 각, 길이 및 비율의 변화를 측정하기 위해서, 만17세부터 21세까지의 성인여성 73명을 대상으로 두부방사선 규격사진을 채득하고 분석하여 아래와 같은 결론을 얻었다.

1. 정상교합을 지닌자에서도 SN-MP 및 OP-MP각은 현저히 다른 양상을 나타내었고 High group에서 Low group으로 전환됨에 따라, SN-Mp각, OP-MP각은 현저히 감소되었다.

2. SN-MP각이 감소함에 따라 SNA, SNB각은 증가하였다.

3. SN-MP각이 증가할수록 ANB각은 컸다.

4. SN-MP각이 클수록 facial convexity도 크게 나타났다.

5. SN-MP각이 감소함에 따라, ant, facial Height도 감소하며, upper facial Height 보다는, Lower facial Height가 Ant, facial Height에 끼치는 영향이 컸다.

6. SN-MP각이 감소함에 반하여 Ramus Height와 후방안면고경(post, Facial Height)은 증가했다.

7. SN-MP각이 감소함에 따라, anterior facial Height 대 후방안면고경의 백분율은 증가했고, 이는 SN-MP각이 감소와 더불어 counter clockwise growth pattern을 가짐을 알 수 있었다.

8. 상하악 제1대구치의 고경은 SN-MP각과 무관했으며, 일반적으로 하악제1대구치 고경이 상악제1대구치 고경보다 컸다.

9. SN-MP각과 다른 38개의 변수들 간의 상관계수는 posterior facial Height 대 Anterior facial Height의 백분율이 -0.92로써 가장 높은 상관관계를 나타내었고, Saddle Angle, Articular Angle, Gonial Angle의 합이 0.84로써 비교적 높은 상관관계를 나타내었다.

10. T-test 결과 1%의 신뢰도에서 유의성을 나타낸 변수는 High Group대 Average Group에서, Gonial Angle을 위시하여 11개 변수이며, Low Group대 Average Group에서 14개 변수이고 High Group대 Low Group에서 22개 변수이었다. 5%의 신뢰도에서 유의성을 나타낸

변수는 High Group 대 Average Group에서 8개 변수이었고 Low Group대 Average Group에서 4개 변수이었고, High Group대 Low Group에서 1개 변수였다.

11. 정상교합을 지닌 자에서도 SN-MP각에 의해 분류된 3Group에서, 각, 길이, 비율의 측정 결과, 서로 다른 양상을 나타내었으므로 이러한 다양성에 부합된 진단, 치료계획 및 치료가 이루어져야만, 각 개인의 이상교합을 찾아 줄 수 있음을 본 연구 결과 재확인했다.

In order to know the variation of the associated craniofacial skeletal angle and linear distance according to the change of SN-Mn plane angle, this roentgenocephalometric study was undertaken in Korean female adults ages from 17 to 21 years by cephalometric analysis and electronic computer.

The following results were obtained.

1) Those who have normal occlusion had the marked difference in the angle of SN-MP and OP-MP.

2) The angles of SNA and SNB increased as the angle of SN-MP decreased.

3) AnB difference angle increased as SN_MP angle increased.

4) Facial convexity increased as SN-MP angle increased.

5) The anterior facial height increased appreciably when the SN-MP angle increased, most of increases at anterior facial height were induced by lower facial height.

6) Ramus height and posterior facial height increased as going from high SN-MP angle to low SN-MP angle.

7) The percentage of posterior facial height to anterior facial height was increased when the angle of SN-MP decreased, and growth pattern of cranio-facial complex tended towards counter-clockwise growth pattern as decreasing SN-MP angle.

8) The height of upper and lower first molars was not related to the SN-MP angle, generally mandibular first molar height was higher than maxilla.

9) Correlation coefficients of others 38 parameters with SN-MP angle were calculated. The correlation coefficient of percentages of posterior facial height to anterior facial height was -0.92 and ranked first in weighting for predicting the SN-MP angle.

10) It was resulted on each subgroups by T-test that the numbers of parameters that were significant at 1% level of confidence were 11 parameters on High Group; Average Group, 14 parameters on Low Group; Average Group and 22 parameters on High Group; Low Group, those of significant at 5% level of confidence were 8 parameters on High Group; Average Group, 4 parameters on Low Group; Average Group, and only one parameters on High Group; Low group.

11) Because persons with normal occlusion had a great variation in the size of skeletal angles, dental angles and linear distance ratio, it was proved that it was necessary to establish the individual diagnosis, treatment planning and treatment corresponding the individual variation for the better treatment.
