측두하악장애 환자와 정상인의 교근 휴지기에 관한 연구

교근의 휴지기가 측두하악장애 환자의 객관성있는 진단 방법의 하나로서 갖는 중요성을 파악하기 위하여, 연세대학교 치과대학에 재학중인 학생과, 부속병원에 내원한 환자중, 측두하악장애의 대표적 증상인 관절부 동통, 관절잡음, 악골운동의 제한을 보이는 36명을 실험군으로, 연세대학교에 재학중인 학생과 치과대학 부속 병원에 재직중인 수련의 중에서, 측두하악장애가 없는 정상인 33명을 대조군으로 하여, 근전도 상에서 교근의 휴지기를 측정하고 비교한 결과 다음과 같은 결론을 얻었다.

1. 실험군에서 휴지기는 평균 36.97±9.23msec 이었고, 대조군에서는 평균 25.62±5.24mser이었다.

2. 실험군과 대조군 모두에서 휴지기는 성별에 따른 유의차가 없었다.

3.실험군에서 대조군에 비해 휴지기가 연장되어 나타났다(p<0.01)

이상의 결론으로, 측두하악장애 환자의 진단 및 치료의 평가시에 교근의 휴지기가 유용한 자료의 하나가 될 것으로 사료된다.

This study was conducted for the assessment of the usefulness of masseteric silent period on electromyogram as a diagnostic method for temporomandibular disorder of students and dentists in Dental College o( Yonsei University and patients, 36 experimentalsubiects with symptoms such as clicking sound, pain in the temporomandibular joint area and limitation of jaw movement, and 33 control subjects without such symptoms were selected for this study. on each subject electromyographic masseter muscle silent period fellowed by mention tap was recorded with surface electrodes and analysed with computerized system.

The following results were obtained:

1. The mean silent period was 36.97±9.23 msec in experimental group, and 25.62±5.24 msec in control group respectively.

2. There were no statistically significant differences in silent period between male and female in either experimental and control group.

3. Silent periods in experimental group were more prolonged than those of control group (P(7.01).

Taken all together, electromyographic masseter muscle silent period may be useful for diagnosis and evaluation of temporomandibular disorder.
