한국인 성인 심장의 좌관상동맥 근위부에 대한 형태학적 연구

관상동맥은 형태학적 양상이 매우 다양한 것으로 알려져 왔다. 좌관상동맥은 전실간지와 회선지로 나뉘며 거의 전체 좌심실과 좌심방 또한 대부분의 심실중격에 분포하는 중요한 동맥이다. 임상적으로 흉부외과 영역의 수술 시 좌관상동맥의 분지 양상 및 분지부까지의 길이 동의 형태학적 특성은 중요한 것으로 알려져 왔다.

좌관상동맥 근위부의 형태학적 특성에 관해서는 이미 오래전부터 여러 보고가 있어 왔으나 국내에는 거의 없는 실정이다. 또한 최근 국내에서도 관상동맥조영술 및 심장수술이 보편화되고 있다. 이에 저자는 한국인 성인 사체 심장 50예에서 좌관상 동맥 근위부의 형태학적 특성을 규명해 보고자 본 연구를 시도하여 다음과 같은 결론을 얻었다.

1. 좌관상동맥 근위부의 길이는 5-16 mm (평균 9.7 mm)이었다.

2. 기시부의 외경은 3.2-7.7 mm (평균 5.3mm)이었다.

3. 좌관상동맥의 분지 양상은 이분지형이 58%, 삼분지형이 38%, 사분지형이 4%이었다.

4. 전실간지의 외경은 2.708.1 mm (평균 4.5 mm)이었다. 회선지의 외경은 2.1-6.2 mm(평균 3.9 mm)이었다.

It is well known that the morphologic feature of coronary artery is variant as figer print. Left coronary artery is divided into the anterior interventricular artery and circumflex artery and supplies almost left atrium. left ventricle and interventricular septum. Clinically, the morphologic features of number of branches and the length of the left main coronaty artery anre important.

In Korea, there are few reports about morphlogic study of the coronary artery, and the coronary angiogram and open heart sugery is common. Now the author studied the morphology of the prximal part of the left coronary artery in Korean 50 Adult cadavers and obtained the following results :

1. The length of the left main coronary artery is from 5 mm to 16 mm(mean 9.7 mm)

2. The diameter of the proximal part of the left comary artery is from 3.2 mm to 7.7 mm (mean 5.3 mm)

3. The features of the branching are bifurcation (58%) trifurcation (38%) and quadrofurcation (4%).

4. The diameter of the anterior interventricular artery is from 2.7 mm to 8.1 mm (mean 4.5 mm) and circumflex artery, from 2.1 to 6.2 mm (mean 3.9 mm)
