의료근접도 및 용이성이 의료이용에 미치는 영향 : 연세건강공제회원의 외래의료이용을 중심으로

의료이용을 결정하는 중요한 의료인에 대한 근접도 또는 용이성은 이미 많은 연구들에 의하여 규명된 바 있다.

본 연구는 연세대학교에 근무하는 교직원과 그 가족 등을 대상으로 구성된 연세건강공제회의 경험을 통하여 공제회원들 중 의료이용의 근접도가 높은 집단과 그렇지 않은 집단의 의료이용 양상의 차이를 규명하기 위하여 시행되었다.

본 연구에서 이용한 근접도는 현재 지정의료시설까지의 거리에 의한 교통편이도, 둘째 회원직종의 의료직-비의료직의 여부, 세째 의료직 중에서도 처방권이 있거나 환자진료에 직접참여하는지의 여부로 본 직종편이도 등 3가지이었다. 의료이용 중 비교적 근접도에 예민하게 반응하리라고 생각되는 외래의료이용을 조사했으며 조사대상은 전체연구대상 4,352명 중 체계표본추출된 713명이었다.

연구결과는 다음과 같다.

(1) 연세건강공제회의 연간 1인당 외래 방문율은 1.66 회이었다.

(2) 교통편이도가 높은 군은 낮은 군보다 연간 1인당 외래방문율이 0.51회 높다.

(3) 의료직-비의료직별 외래이용율은 두 군 사이에 큰 차이가 없었다.

(4) 직종편이도별 외래이용율은 직종편이도 높은 군이 직종편이도 낮은 군에 비해 연간 1인당 0.29회 높았으며 외래방문질병당 외래방문에 있어서 외래급여율이 낮아진 1976년 2학기와 당초의 외래급여율을 적용한 동년 1학기의 차이는 직종편이도 높은군에서는 0.43회 감소하였고 직종편이도 낮은 군은 0.32회 증가하였다.

Accessibility to medical facilities and personnels has been known as one of important determinants of medical care utilization. This study attempted to identify the effects of medical accessibility in terms of geographical distance and occupational opportunity to the medical utilizations.

Two-years-experiences of Yonsei University Health Insurance Coopertives were used as the sources of data. Out patient utilization patte군 of 713 members sampled from 4,352 members of Health Insurance Cooperatives were analyzed in order to identify

the effects of medical accessibilities.

The results were as below;

1. Average clinic visit rate of Yonsei Health Insurance is 1.66 per person per year.

2. The utilization rates of geographically more accessible group were 33% higher than that of less accessible group.

3. No marked difference in clinic visit rate were observed between medical and non-medical personnel and their family members.

4. Clinic visit rates among occupationally accessible group were slightly higher than those of less accessible. The utilization rate was more sensitively changed by the insurance policy changes in occupationally accessible group.
