나종나 환자에 있어서 Thiacetazone-Isoniazid 복합체의 치료효과

선명회 특수피부진료소 및 나병연구소에서 규칙적으로 치료받은 환자중 diaminodiphenylsulfone(DDS)과 rifampicin의 장기치료도 불구하고, 임상적 또는 세균학적 치료효과의 호전이 없는 나종나 환자들을 선택하여 thiacetazone-isoniazid 복합체로 치료한 결과 아

래와 같은 결론을 얻었다.

1) 나종나 환자중 DDS와 rifampicin 치료로 임상 및 세균학적으로 치료효과를 관찰할 수 없었던 11예와 DDS 및 rifampicin으로 치료하여 균음전화는 되었으나 계속 신경통을 호소하는 환자 1예를 선택하여 thiacetazone-isoniazid 복합체를 투여하였던 바 11예에서 임상증상과 세균지수가 호전됨을 관찰하였고 만성 신경통을 호소하는 1예에서는 신경통이 소실됨을 관찰하였다.

2) 임상적 세균학적 치료효과를 관찰할 수 있었던 11예 가운데 8예에서는 2년 이내에 모두 세균학적 균음전을 가져왔으며 3예에서는 thiacetazone-isoniazid 복합체로 치료전 세균지수 +1∼+3에서 2년∼2년3개월 치료후 세균지수가 +0.2∼+1로 감소하였다.

이상의 성적으로 보아 thiacetazone-isoniazid의 복합요법은 DDS 또는 rifampicin의 장기치료에도 치료효과를 볼 수 없는 내성환자의 치료에 탁월한 효과가 있을 것으로 본다.

[영문]

Thiacetazone-isoniazid complex was used to treat Lepromatous type of leprosy patients who showed poor response by DDS and rifampicin.

1) Among those 12 0patients, we observed good clinical and bacteriological improvement in 11 patients, clinical improvement in 1 patient.

2) Among 11 patients who showed clinical and bacteriological improvement, 8 patients are negatively converted in bacteriological index within 2 years, but 3 patients are converted to +0.2-+1 from +1-+3 of bacteriological index after 2-2 1/4 yeats treatment.

3) We observed control of chronic neuralgia in 1 patient who showed negative bacteriological index.

These results show that thiacetazone-isoniazid compex-might be used as secondary drug to treat leprosy patients who respond poorly with DDS and rifampicin. It might be used as drug to control chronic neuralgia after the bacteiological index is negatively converted.
