서울시내 일부 고등학생 시력의 시계열별 변화와 관련요인

고등학교 학생들의 과거 10년간 시력상태 및 시력저하 관련요인을 분석하기 위하여 서울 시내 남 녀 공학 고등학교 1개교 3학년 학생의 건강기록부 자료와 구조화된 설문지를 이용하였다. 최종 연구대상자는 314명(남:150명, 여: 164명)으로 연구 결과는 다음과 같다.

첫째, 고등학교 3학년의 과거 10년간의 평균시력은 남녀 모두 학년의 증가에 따라 저하되는 경향을 보였다. 남자는 중학교 3학년에서, 여자는 중학교 3학년과 고등학교 2학년에서 가장 낮은 시력상태를 보였으며 전 학년에서 여자가 남자보다 평균적으로 더 낮았다. 평균시력이 1.0 미만으로 저하되기 시작한 시기는 여자가 국민학교 3학년, 남자가 중학교 2학년으로 여자의 시력이 남자보다 일찍부터 저하되었다. 시력저하자의 발생은 남자의 경우 학년의 증가에 따라 증가 경향을 보였으며, 여자의 경우 뚜렷한 경향을 볼 수 없었다.

둘째, 고등학교 3학년에서 시력저하자는 남자가 54.0%, 여자가 74.0%로 여자의 비율이 유의하게 많았다(p<0.05).

셋째, 남자의 경우 출생순위, 부모의 교육수준, 학교성적 등은 어떤 뚜렷한 경향을 볼 수 없었으며 통계학적으로 유의한 차이가 없었다. 여자의 경우 출생순위, 부모의 교육수준별로는 뚜렷한 경향을 볼 수 없었고, 중학교와 고등학교 성적에서 학교성적이 좋을수록 시력저하자가 많았으나, 통계학적으로 유의한 차이가 없었다.

넷째, 고등학교 3학년에 있어 시력저하군과 상시력군의 가족 시력 특성별로는 어머니만 안경을 착용한 군에서 시력저하자가 가장 많았다. 형제가 둘이상일때 시력저하군에서 안경을 착용한 형제가 더 많았으며 형제가 하나일 때는 착용하지 않은 형제가 더 많았으나 모두 통계학적으로 유의하지 않았다.

다섯째, 고등학교 3학년에 있어 시력저하군과 정상시력군의 악기 학습년수별로는 시력저하군이 학습년수가 더 길었으나, 통계학적으로 유의한 차이가 없었다.

여섯째, 악기 학습군과 비학습군의 평균시력의 변화는 남녀 모두 학년의 증가에 따라 저하되는 경향을 보였으며, 학습군이 비학습군보다 평균적으로 시력이 낮았다. 학습군의 경우 시력이 가장 낮은 시기는 중학교 3학년, 비학습군은 고등학교 2학년이었다. 평균시

력이 1.0 미만으로 저하되기 시작한 시기는 남녀 모두 학습군에서 일찍 나타났다.

일곱재, 악기 학습군의 악기 학습 년수별 고등학교 3학년에서의 평균시력은 학습 년수가 길수록 낮았으며, 통계학적으로 유의한 차이가 있었다. 악기 학습군의 악기 학습 수준별 고등학교 3학년에서의 평균시력은 학습 수준 중 집단에서 가장 낮았으나, 통계학적으로 유의하지 않았다.

이상과 같이 조사대상 학생들의 시력은 학년의 증가에 따라 점차 저하경향을 보였으며, 성별에 따라 여자가 남자보다 일찍부터 시력이 저하되기 시작하며 전 학년에서의 시력상태도 평균적으로 여자가 남자보다 낮았다. 악기학습군이 비학습군보다 시력이 일찍부터 저하되기 시작하며, 시력상태도 평균적으로 학습군이 더 낮았다.





[영문]

The purpose of this study was to observe the ten-year changes of visual acuity and to determine the factors of reducing visual acuity-especially leaning musical instrument-. For the study, data from ten-years health record of one high school in Seoul that checked by school yearly and self-written questionnaire was analysed.

The subject of study were 314(150: male, 164: femille).

The results of study were summarized as follows.

1. The average visual acuity of female was more reduced than male by aging during past ten years. The onset of low average visual acuity that defined less than 1.0 in this study was third grade of elemantary school in female and second grade of middle school in male. In male, the rate of low visual acuity increased by aging but in female the rate fluctuated.

2. The propotion of low visual acuity at third grade of high school was 54.0% in male, 74.0% in female.

3. The performance of middle and high school was associated with low visual acuity but not statistically significant in female, other general factors of visual acuity were statistically insignificant.

4. In family history of visual acuity, low visual acuity were associated with putting on mother's and brother's glasses but not statistically significant.

5. The group of low visual acuity at third grade of high school was longer than group of normal visual acuity but not statistically significant.

6. The average visual acuity of group learning musical instrument was lower than group not learning. The onset of low average visual acuity of group learning musical instrument was earlier than group not leaning.

7. By the period of learning musical instrument, rate of low visual acuity were increased and statistically significant, but by the level of operating instrument was not statistically significant.

To sum up tee results, visual acuity was gradually reduced by aging and the onset of low visual acuity of female was earlier than male. At all age the average of female's visual acuity was lower than male's. The onset of low visual acuity of group loaning musical instrument was earlier than group not learning musical instrument and the average visual acuity of group learning musical instrument was lower than group not learning musical instrument.
