전두개와 경막에 분포하는 동맥에 관한 형태학적 연구

Abstract

해부학 교과서에 의하면 전두개와에는 안동맥의 가지인 전 및 후사골 동맥의 경막지, 내경동맥 해면정맥동부에서의 미세 가지, 그리고 중경막동맥의 전지의 일부 등이 분포하는 것으로 기술되어 있다. 중두개와와 후두개와에 분포하는 동맥은 비교적 크고 이에 관한 보고도 다수 있으나 전두개와의 동맥 분포에 관해서는 거의 보고된 것이 없는 실정이다. 이에 저자는 전두개와의 혈관에 관한 형태학적 연구가 큰 의의가 있으리라 사료되어 본 연구를 시도하였다. 한국인 사체 34례에서 전두개와의 경막 및 대뇌겸에 분포하는 경막지를 관찰한 바 다음과 같은 결과를 얻었다

1. 전사골동맥의 경막지는 34례 중 20례(58.8%)에서 관찰되었으며, 1례에서는 후사골동맥의 경막지가 분포하는 것이 관찰되었다.

2. 후사골동맥의 경막지는 34례 중 14례(41.2%)에서 관찰되었다. 3. 전두개와의 내측 3/1은 전 및 후사골 동맥의 경막지가 분포하였으며, 두 동맥은 3례(8.8%)에서 함께 관찰되었다.

4. 전겸동맥은 3례(8.8%)에서 관찰되었다.

5. 중경막동맥의 전지는 전 예(100%)에서 관찰되었으며, 전두개와의 외측 1/3에 분포하였다. 동맥의 분포 양상은 혈관의 주행에 의해 네가지 형으로 분류하였는데, 제Ⅰ형은 프테리온 상방을 지나 분포하는 형으로 11례(32.4%)에서 관찰되었고, 프테리온을 지나 분포

하는 제Ⅱ형은 4례(11.8%)에서, 접형골소익을 가로 질러 분포하는 제Ⅲ형은 16례(47.1%)에서, 제Ⅰ형과 제Ⅲ형의 혼합형인 제Ⅳ형은 3례(8.8%)에서 각각 관찰되었다.

The author have studied 34 sides of calvaria about the anatomical structure of the passage of meningeal arteries in the anterior cranial meninges, the origin, number and territory of anterior and prosterior ethmoidal artery, the relation between anterior and posterior ethmoida artery, the origin, number and distance of anterior falx artery, distance from anterior falx artery to inner table of frontal bone, the frontal branch of middle meningeal artery which supplies the lateral part of meninges in anterior cranial fossa.

The results were analyzed and evaluated as following;

1. Meningeal branch of anterior ethmoidal artery was presented in 20 cases (58.8%).

2. Meningeal branch of posterior ethmoidal artery was presented in 14 cases (41.2%).

3. Anterior and posterior ethmoidal artery supply the area of medial third of meninges in anterior cranial fossa.

4. Anterior falx artery presented in 3 cases (8.7%). Origin of all cases were revealed as anterior ethmoidal artery.

5. Frontal branch of middle meningeal. artery in anterior cranial. fossa was presented in all cases. This branch supplies the lateral part of meninges in anterior cranial fossa.

The author could divide the group of frontal branch of middle meningeal artery presented in meninges of anterior cranial fossa into following 4 types according to the sitd of which presented in.

type Ⅰ : 11 cases (32.3%) type Ⅱ : 4 cases (11.7%)

type Ⅲ : 16 cases (47.2%) type Ⅳ : 3 cases (8.8%)
