제왕절개술시 Ketamine 마취의 임상적 연구

dc.description.abstract[한글] Phencylidine 유도체에 속하는 Ketamine은 1965년 Domino 등에 의해 강한 진통효과가 있다고 보고된 이래 여러 분야의 수술을 위한 마취에 광범위하게 사용되어 왔다. 산과마취에 있어서 두가지 점 즉 마취유도시 일시적인 혈압상승작용과 회복시 정신흥분 현상을 제외한다면 다음과 같은 이유로 적절하게 쓸 수 있다고 하였다. 즉 ① 신생아를 억압시키는 일이 적고, ② 마취유도시 후두반사와 기도가 유지되며 ③ 안전범위가 넓고 호흡이나 심장에 대한 억압작용이 적으며 ④ 자궁수축이 강하여 출혈이 적고 ⑤ 수술 후 오심 구토가 적으며 회복이 빠르다.(Galloon, 1971; Galloon 및 Dick, 1971; Spoerel, 1971). Corssen(1974) 과 McDonald등 (1972)도 제왕절개술에 Ketamine을 사용하고 산모와 태아에게 좋은 점이 있음을 보고하였다. 이러한 이점에 착안하여 저자는 제왕절개술에 대한 마취중 thiopental로 마취유도한 25예와 ketamine으로 마취한 27예를 대상으로 하였으며 후자는 다시 그 마취방법에 따라 즉 ① ketamine 단독마취시 ② ketamine 과 succinylcholine으로 마취유도 및 기관내삽관시 그리고 ③ ketamine 과 pancuronium으로 유도 및 삽관한 3군으로 분류하여 마취전, 중 및 후의 산모 및 태아의 상태를 비교관찰하여 다음과 같은 결론을 얻었다. 1) Ketamine은 강력하고도 빠른 진통효과를 나타냈으며 응급 및 선택수술에 모두 사용할 수 있었다. 2) 마취방법에 있어서는 thiopental 또는 ketamine 단독마취보다 아산화질소(N^^2 O)와 근육이완제를 함께 사용하는 것이 좋으며 succinylcholine을 반복정주하는 것보다 pancuronium을 1회 정주하는 것이 좋았다. 3) 마취유도시 ketamine은 2mg/kg 이하의 정주가 추천할 만하며 그보다 용량이 많을 때는 태아를 억제시키는 경향이 있었다. 4) Ketamine은 산모의 혈압을 상승시키는 효과가 있었으며 ketamine-pancuronium 마취때는 수축기 및 특히 확장기혈압을 현저히 증가시킴을 관찰하였다. 5) Ketamine은 산모의 맥박수를 증가시켰으며 ketamine-pancuronium 방법에서는 타군에 비해 마취유도시 28.5%의 의의있는 증가를 보았다. 6) Ketamine 과량으로 인한 1예를 제외하고는 태아억압의 예는 없었으며 I.D.I.가 길어질 때 Apgar score가 나빠지는 것을 관찰하였다. 7) Ketamine 마취시 태아만출후 이완성자궁출혈의 예는 없었다. 8) 수술 및 마취시간이 가장 짧게 걸린 군은 hiopental 군 이었으며 회복이 가장 빨랐던 군은 ketamine-pancuronium-ether 마취방법이었다. 이상의 성적을 종합하여 볼 때 ketamine 근육이완제 N^^2 O 기관내 마취법은 고혈압을 동반한 임신중독증이나 정신과적 문제를 가진 산모나 태아 절박상태를 제외한다면 제왕절개술시 산모와 태아에 대한 안전하고도 만족스러운 유용한 마취방법의 하나라고 생각한다. [영문] The pharmacological actions of ketamine in human volunteers were reported by Domino et al. in 1965, and use in 130 patients by Corssen and Domino (1966). Since then, its use in a wide variety of surgical procedures has been reported throughout the world. Several authors (Galloon, 1977; Galloon and Dick, 1971; Spoerel, 1971) reported that ketamine gas several advantages over conventional anesthetics. The advantages of using ketamine anesthesia are : preservation of pharyngeal reflex and airway maintenance during induction of anesthesia, stimulation, fast recovery, little nausea and vomiting after anesthesia, little depression of the fetus and good uterine contraction with minimal bleeding. On the other hand, ketamine has also disadvantages : elevation of arterial pressure and pulse rate temporarily during induction of anesthesia, poor muscle relaxation and post-operative psychotic reactions are not uncommonly found. The author tried to find out the easibility of ketamine anesthesia for Cesarean section over the conventional method of thiopental-muscle relaxant-N^^2 O with IPPV technique. Materials and Methods 52 Korean parturients were selected for Cesarean section including emergency and elective operation for this study. Thioental group. 25 cases were induced for anesthesia with about 3.5㎎/㎏ of thiopental and intubated with the help of 1㎎/㎏ of succinylcholine. After delivering the baby, anesthesia was maintained with N^^2 O - O^^2-ether throughout the procedures. Ketamine A group. 9 cases, just before skin incision, were injected intravenously with ketamine 1.9㎎/㎏ slowly for over one minute with or without N^^2 O:O^^2 (2:1 L/min) through a mask. After delivering the fetus, a supplement of ketamine and diazepam 10㎎ Ⅳ was given intermittently. Ketamine B group. Anesthesia was induced by 1.9㎎/㎏ ketamine and 1㎎/㎏ of succinylcholine with endotracheal intubation. After delivery, N^^2O with O^^2 and additional ketamine were given to 9 patients. Ketamine C group. Anesthesia was performed with 1.7㎎/㎏ of ketamine, 0.08㎎/㎏ of pancuronium, N^^2 0, with endotracheal intubation for 9 patients, ether supplement was given following delivery. Conclusion With these mentioned methods of anesthesia, the author formed several conclusions about ketamine anesthesia in Cesarean section. 1. Ketamine can be used as the main anesthetic or for induction in elective and emergency Cesarean section because of its rapid onset and intense analgestic effect. 2. As in the method of anesthesia, it is useful to combine N^^2 O-O^^2 mixture and muscle relaxants such as succinylcholine or pancuronium. The technique is more suitable for maintenance of anesthesia because of the poor muscle relaxation of ketamine alone. 3. For induction of anesthesia, under 2㎎/㎏ of ketamine is advisable. Exceeding this dose, the infant respiration is more likely to be depressed because of hypertonicity of the skeletal musculature. 4. Ketamine has a maternal cardiovascular stimulation effect particularly diastolic blood pressure and pulse rate in the Ketamine-pancuronium-N^^2 O-intubation group. 5. Less bleeding was found during and after the delivery, recovery due to an increased uterine contraction from ketamine. 6. Disadvantage of ketamine included a prolonged maternal recovery period, and newborn respiratory depression and these seemed to be dose related. From th above, ketamine anesthesia appears to be another safe and satisfactory method of anesthesia for Cesarean section, provided that toxemia of pregnancy patients with hypertension and patients who have had psychotic problems previously are avoided.-
