농촌지역의 안전분만에 대한 요인

농촌지역의 의료인에 대한 안전분만율을 조사하고 분만 개조자로서 비의료인을 선택하는데 영향을 주는 요인을 밝히기 위해 강화보건원에 등록된 3개면의 임산부중 1981년 1월1일부터 1984년 12월31일까지 정상분만된 569예를 대상으로 분석하여 다음과 같은 결과를 얻었다.

1. 의사, 조산원, 간호원등 의료인에 의한 분만개조율은 51.5%였다.

2. 요인별 비의료인에 의한 분만개조 비교위험도를 본 결과:연령이 30세 이상의 산모에서 24세 이하의 산모에서보다 1.467배 높았고, 국민학교 교육수준 이하의 산모에서 중학교 교육수준 이상의 산모에서보다 1.407배 높았으며, 초임부에서보다 임신회수 2회인 산

모에서 1.670, 3회의 산모에서 1.681, 4회의 산모에서 2.281로 더 높았다. 경산부에서 초산부보다 1.614배 높았고, 인공유산 경험이 있는 부인에서 인공유산 경험이 없는 부인에서보다 1.426배 높았으며, 출산간격이 24개월 이하인 산모에서 25개월 이상인 산모에서보다 1.297배 더 높았고, 의료보험 실시 이후보다 의료보험 실시 이전에 1.295배 높았으며 이들은 각각 95% 신뢰구간에서 통계적으로 유의했다.

The purposes of this study were to know the rates and factors related with the medically attended deliveries in a rural community. The data used in this study was obtained from the pregnancy registration cards of 569 women who had had normal spontaneous delivery, in three towns of Gang Wha County during the period of January 1, 1981 through December 31, 1984.

The results were as follows:

1. Fifty-two percent(52%) of deliveries were attended by medical personnel.

2. The proportions of deliveries not medically attended estimated by relateve risk were:

1) 1.467 times higher among the women aged 30 or older than the women at aged 24 or younger,

2) 1.407 times higher among the women with less than 6 year education than the women with 6 year education or higher,

3) 1.670 times higher among the women with one previous pregnancy, 1.681 times higher with two previous pregnancies, 20281 times higher with four or more pregnancies than primigravida,

4) 1.641 times higher with four or more pregnancies than primigravida,

5) 1.426 times gigher among women with previous experience of induced abortion than women without previous experience of induced abortion,

6) 1.297 times higher among women with 25 months birth interval or longer than women with 24 months interval or shorter, and

7) 1.295 times higher before the application of medical insurance than after the application.
