임상에서 감염성 질환의 치료제로 사용되고 있는 aminoglycoside 계통의 항생제 중의 하나인 kanamycin은 신장 및 청각 계통에 대한 독성 이외에 순환기계에도 장애를 초래함이 보고되었다. 즉 kanamycin의 투여 농도에 비례하여 심근의 수축력이 저하되며, 분리한
심근막에서의 Ca**++ 결합을 경쟁적으로 억제하며, 고농도 K^^+ -유발 활동전압의 크기 및 탈분극 속도를 저하시킨다는 보고 등은 kanamycin이 calcium 전류에 대해 길항제적 효과를 갖고 있음을 추측케 한다. 따라서 본실험에서는 단일 심근 세포를 이용한 막전압 고정법을 통해 kanamycin이 Ca**++ current에 미치는 영향을 직접 측정함으로써 kanamycin의 Ca**++ 길항제적 성상의 특징과 kanamycin에 의한 심근 수축력 저하의 기전을 규명해보고자 실험하여 다음과 같은 결과를 얻었다.
1. Kanamycin 투여량에 비례하여 Ca**++ current의 크기가 감소하였고, 이같은 감소 효과는 모든 막전압 값에서 관찰되었다.
2. 관류액의 Ca**++ 농도를 2mM에서 20mM로 증가시킨 경우 kanamycin에 의한 Ca**++ current 감소 정도는 아주 미약하였으며, 이같은 현상은 관류액의 Ca**++ 을 20mM Sr**++ 으로 대치한 경우에도 동일하게 관찰되었다. 반면에 inorganic Ca**++ antagonist인 Cd**
++ 은 관류액의 charge carrier로 20mM Sr**++ 을 사용한 경우, charge carrier로 20mM Ca**++ 을 사용한 경우에 비해 Ca**++ channel을 통한 전류의 크기를 현저히 감소시켜주었다.
3. 관류액의 Ca**++ 농도를 변화시켜가면서 kanamycin에 의한 Ca**++ 전류 감소 효과의 변화를 Lineweaver-Burk plot을 이용하여 분석한 결과 kanamycin이 Ca**++ 전류에 대해 경쟁적 억제 현상을 보이는 것을 관찰할 수 있었다.
4. 반복되는 탈분극 자극시 kanamycin에 의한 Ca**++ current 의 감소 정도(use dependency)는 verapamil(organic Ca**++ antagonist)의 경우와 같이 특징적이지 못하였다.
5. Neomycin 역시 kanamycin과 유사하게 Ca**++ current를 감소시켜주었다.
이상의 실험 결과로 보아 kanamycin은 Ca**++ 전류에 대해 길항제적 효과를 갖고 있으며, 이같은 효과는 kanamycin이 Ca**++ 전류에 대해 경쟁적 억제제로 작용하여 나타나는 것으로 생각된다.

It has been reported that kanamycin, a useful agent in the treatment of gram-negative and other infectious disease, has a negative inotropic action in isolated cardiac muscle preparation and also it decreases the amplitude of high K**+ -induced action potential of guinea pig atrium. These findings imply that
kanamycin has a property of Ca**+ antagonist.
In this study, the effects of kanamycin on cardiac Ca**++ current were investigated in isolated rabbit ventricular cells by using a whole cell clamp method
The results are summarized as follows;
1. Kanamycin caused a depression of cardiac Ca**++ current in a dose dependent manner and its effect was observed in a whole membrane potential range.
2. The decreasing effect of kanamycin on Ca**++ current was inhibited by high Ca**++ -Tyrode solution and such inhibition was also observed in high Sr**++ -Tyrode solution, but in the case of Cd**++ , well known inorganic Ca**++ antagonist, the current through Ca**++ channel was greatly decreased when the perfusate was changed from high Ca**++ -Tyrode to high Sr**++ -Tyrode solution.
3. The effect of kanamycin on Ca**++ current was analyzed by using a Lineweaver-Burk plot, and it showed the characteristics of competitive inhibition.
4. In the presense of kanamycin, the decreasing pattern of Ca**++ current by repetitive depolarization was not specific as in the case of verapamil.
5. Neomycin decreased Ca**++ current similar to kanamycin.
From the above results, it may be concluded that kanamycin has competitive antagonistic effect on cardiac Ca**++ current.
