일부 나환자의 신체상과 우울과의 관계 연구

Abstract

[한글]본 연구는 나환자의 신체상과 우울정도를 파악하고 신체상과 우울과의 관계를 규명하기 위해 시도된 기술적 상관관계 연구이다.

연구대상은 1개 나환자정착촌에서 50명과 1개 환자 수용병원에서 30명을 편의추출하여 80명을 대상으로 하였다.

연구도구로는 신체상을 측정하기 위해 본 연구자가 개발한 신체의미측정도구를 사용하였으며 이것은 의미어를 적용하여 총체적인 신체태도를 측정하기 위한 것으로 Osgood과 그의 동료들이 개발한 의미 척도법을 기초로 제작되었다.

우울을 측정하기 위해서는 Beck의 Oepression Inventory를 사용하였다.

수집된 자료를 분석하기 위하여 일반적인 특성은 실수와 백분율, 신체상 정도와 우울정도는 평균점수와 편차를 구하였고 우울과 신체상의 상관관계에 의한 가설검증은 Pearson's product correlation coeffcient를, 신체상 및 우울과 일반적 특성과의 관계는 Pearson

's product correlation coefficient, ANOVA, t-test, Scheffe procedure를 사용하였다.

본 연구 결과는 다음과 같다.

1. 가설, "나환자의 신체상 정도가 낮을수록 우울정도는 높을 것이다."는 채택되었다(r=-.5034, p<.001).

2. 대상자의 신체상 정도파악은 나균이 침범하여 신체적 변형이 나타난 부분에서 낮은 점수를 보였으며 신체적 변형이 없는 부분은 높은 점수를 보였다. 평균점수 43.97로 그 이하를 나타낸 신체부분으로는 발이 33.03, 얼굴표정 36.86, 다리 37.41, 손 38.69, 걸음걸이 39.16, 눈 42.27, 피부 43.80으로 신체적인 손상이 나타나는 부분에 대한 신체태도가 부정적임을 알 수 있었다.

3. 대상자의 우울정도의 평균은 25.70으로 Beck의 분류에 의하면 나환자들은 우울군에 속하는 것으로 나타났다.

4. 신체상정도와 일반적 특성과의 관계에서는 연령(r=-.4059, P<.001), 결혼상태(F=9.7578, p<.001), 학력(F=2.9953, p<.05) 신체장애정도 (F=14.3540, p<.0001), 거주형태(t=3.62, p<.001)에서 통계적으로 유의한 차이를 보였다.

5. 우울정도와 일반적 특성과의 관계에서는 연령 (r=.2502, p<.05), 결혼상태(F=4.3682, p<.05), 신체장애정도(F=6.5148, p<.001), 거주형태(t=-2.24, p<.05)에서 통계적으로 유의한 차이를 보였다.

결론적으로 나환자의 신체상과 우울은 역상관관계를 가지는 것으로 나타나 나환자의 신체상정도가 낮을수록 우울정도가 높은 것을 알 수 있었다.

[영문]

This descriptive-correlational study was undertaken in order to examine if there was a relationships between body image and depression in patients with Hansen's Disease.

80 Hansen's Disease patients were selected from one resettlement village and one leprosarium for this study.

The sampling used was a non-probability, convenient sampling technique.

Data was collected by means of an interview and the study was carried out between September 21st and September 30th in 1987.

The instruments used for this study were the Body Meaning Scale developed by the researcher and the Beck Depression Inventory by Beck.

The Body Meaning Scale was composed of 13 body concepts to be rated on a semantic dtfferential scale of 10 adjectives.

Analysis of data was done using percentages, frequencies means, standard deviations, Pearson's product correlation coefficient, t-test, ANOVA and Scheff'e procedure.

The findings of this study were as follows:

1. The Hypothesis “The lower body image scored, the higher the depression score” was accepted.(r=-.5034, p<0.01).

2. In the body image score for Hansen's Disease patients, the body image score for the physically handicapped part of the instrument was low and the body image score for the physically non-handicapped part was high. The total mean score was 43.97. Body part which were scored below the total mean score were feet, face, legs, hands, gait, eyes, skin.

3. The depression score for the Hansen's Disease patient had a mean score was 25.70.

4. The demographic factors related to body image in Hansen's Disease patients were age, marital status, education level, the degree of handicap and type of residence (resettlement village vs leprosarium).

5. The demographic factors related to depression in Hansen's Disease patients were age, marital status, the degree of handicap and the type of residence. (resettlement village vs leprosarium).

In conclusion, there was negative correlation between body image and depression; poor body image correlated with high depression.
