손상건 수복과정에 관한 실험적 연구

손상된 건의 기능회복에 관한 정형외과적 술식의 발전과 다수의 연구 업적이 이루어졌음에도 불구하고 건과 주위조직사이에서의 견고한 유착형성에 대한 명확한 기전과 성질의 파악은 아직 해결하기 어려운 과제로 남아 있다. 이 유착은 건을 견고하게 고정시키고 효과적으로 정상기능에 필요한 건의 활주운동을 봉쇄한다. 건과 주위조직과의 유착을 최소한도로 방지하는 가장 성공적인 건 수복의 문제 해결을 위해서는 비손상 외과적 술식(atraumatic surgical technique)이 가장 중요하며 또한 이를 위해서는 여러가지 조건이 필요하다. 즉 미세하고 정교한 수술기구, 수술시야의 확대와 지혈, 이물반응을 일으키지 않는 봉합재료, 건을 접합시키는 특수 봉합술등이다. 이들 여러 필수조건중에서 건 유합술에 가장 이상적인 봉합물질은 조직반응을 일으키지 않고 건 수복과정이 진행되는 최소 3내지 4주간 손상건을 접합상태로 유지할 수 있어야 하며 강도(tensile strength)가 강해야 하고 수술시 조작하기가 용이해야 한다. 이러한 조건을 충족시키는 가장 좋은 건 봉합물질이 어느 것이냐 하는 문제에 대해서는 각 봉합물질이 각각의 장단점을 가지고 있기 때문에 결정하기가 어렵고 이로써 사용자에 따라 각자 다른 많은 봉합재료가 선택 사용되어 왔다.

이상과 같은 봉합재료의 선택문제와는 달리 현재 Achilles건 파열의 치료방법에 있어서 서로 상반되는 수술적 건 봉합법과 비수술적 치료법이 대립되고 있고 건 수복의 기전에 대해서도 건 재생이 건초에서부터 유래한다는 설과 파열 건조직 자체에서부터 유래한다는

설이 대립되고 있다.

본 연구에서는 건 수복과정에서 최소한의 이물반응과 영향을 유발시키는 봉합재료를 선택하고 Achilles건 수복에서 봉합군과 비봉합군 사이에 교원질형성의 조직학적 차이점을 관찰하여 우수한 재생과정을 저하는 치료방법을 선정하고 아울러 건초의 건 수복과정에

대한 영향을 조직학적으로 관찰하여 그 수복기전의 일단을 추구하고자 본 실험을 기도하였다.

실험동물로 체중 2kg내외의 건강한 웅성 가토를 사용하여 건 봉합을 하지 않은 군, 4종의 봉합재료를 각자 사용하여 건 봉합을 실시한 군 및 건초절제와 건 봉합을 병합한 군의 6군으로 나누었고 각 군은 희생시킨 시기에 따라 수술 제 10일 후와 28일 후로 다시 나누었으며 각 군당 6마리씩 모두 72개의 Achilles건에 실험을 실시하였다. 봉합물질은 주위에서 쉽게 이용될 수 있고 널리 사용되고 있는 동일한 굵기 4-0의 silk, nylon, dexon(polyglycolic acid)과 stainless steel wire를 사용하였다. 각 실험동물에서 Achilles건의 중간 부위를 예리하게 횡단면으로 절단시킨 후 비봉합군은 파열 건을 봉합하지 않고 봉합군은 각 봉합재료로 파열건을 Bunnell씨 봉합법과 단부연 단순봉합법으로 봉합한 후 슬관절은 굴곡, 족관절은 척굴곡 위치에서 석고붕대고정을 하였다. 실험동물은 수술 10일 및 28일 후에 희생시켜 파열부분을 중심으로 Achilles건을 절제한 후 고정시키고 약 6μ두께의 조직절편을 만들었다. 절편의 일부는 Goldner-Foot modification의 Masson-trichrome 염색, 나머지는 hematoxylin-eosin 염색으로 처리한 후 검경하에서 비교 관찰하였던 바 다음과 같은 결과를 얻었다.

1. 건 수복과정에서의 교원질형성은 비봉합군에서 가장 풍부하였고 stainless steel wire 봉합군에서 불량하였으며 silk, nylon 및 dexon 봉합군에서는 중등도였다.

2. 이물반응은 silk, dexon 봉합군에서 가장 심하였고 stainless steel wire 봉합군에서 극히 경미하였다.

3. 원건조직(original tendon)의 변성(degeneration)은 stainless steel wire 봉합군에서 가장 심하였다.

4. 섬유아세포의 출현도는 stainless steel wire 봉합군에서 희소하였고 기타 봉합군에서는 정상이었으며 따라서 치유의 성숙도도 stainless steel wire 봉합군에서 불량하였다.

5. 교원질을 포함한 육아조직이 초기에 건초로부터 유래하여 건 재생을 이루었다.

이상의 결과로 미루어 보아 nylon이 건 봉합에 가장 좋은 봉합재료이고 우수한 Achilles 건 수복방법은 보존적인 비봉합법으로 생각되며 건 재생과정 초기에 건초의 역할이 중요한 것으로 사료된다.

Despite advances in knowledge and refinement of technique made in tendon surgery, the apparent mechanism and propensity for formation of dense, unyielding adhesions between the site of repair and surrounding tissue remain a formidable clinical problem in tendon surgery. These adhesions firmly tether the tendon and effectively block the gliding movement which is so necessary for normal function. The most important factor in the successful solution of the problem of tendon repair is the employment of an atraumatic surgical technique; (1) gentleness, fine instruments and the judicious use of magnification; (2) bloodless field; (3) adequate physiologic incisions; (4) nonreactive suture material; (5) special sutures and techniques to hold the tendon.

The ideal suture material should have little or no tissue reaction, high tensile strength and a character of easy handling and also remain at least three to four weeks while the slow precesses of tendon repair takes place. It has been very difficult to select the best suture material for tendon repair because each material has its advantages and disadvantages and thus many suture materials for tendon repair have been used by many authors.

For Achilles tendon rupture, operative repair has been the most generally accepted treatment until recently. Some now believe that operative repair of Achilles tendon rupture is unnecessary and excellent results are obtained by simple cast immobilization for an adequate period of time and the high incidence of complications following operative repair is advoided(Lea and Smith 1972; Nistor 1976).

The healing process of tendon wounds has presented varied controverses. The process of healing of tendon wounds has evolved into two main schools of thought. One view is that new tendon arises mainly from the sheaths that surround the tendon stumps and that the tendon tissue itself plays no significant role (Skoog and Persson, 1954; Potenza, 1962; Entin, 1973). Another school holds that regeneration take place by proliferation from the cut ends of the tendon (Matthews and Richards, 1974; Furlow, 1976). Flynn and Graham(1962) concluded that union is first effected by proliferation of the sheath tissues, but that after the fourth or fifth day the tendon itself proliferates and sends cells into the callus, as reported by Mason and Shearon(1932).

The purposes of this study were to select the suture material with the least effect on the healing process and the least foreign body reaction in the tendon, to determine a more satisfactory method of repair of Achilles tendon rupture by observing the histological differences between sutured and unsutured groups and also to study histologically the role of tendon sheaths in the healing process.

For this study 72 Achilles tendons of male adult rabbits with an initial weight of about 2kg were used. The animals were divided into 6 groups which included an unsutured group, sutured group with four different suture materials and a sutured group combined with tendon sheath excision. Each groups was subdivided into 2 subgroups by the sacrificing period (10 and 28 days). Suture materials used were silk, nylon and stainless steel wire, which have been popular and easily available, and absorbable dexon(polyglycolic acid), which was synthesized recently with various advantages. All sutures were the same size. In the animals, after sharp transection of the mid-portion of the Achilles tendon, the tendon stumps were approximated and sutured with each suture material by Bunnell's suture method and also tendon end marginal suture. Then a long leg cast in the position of flexion of the knee joint and of plantar flexion of the ankle joint was applied. On th 10 th and 28 th days after operation, the animals were sacrificed and the specimens 6 μ in thickness were cut after excision and fixation of the ruptured Achilles tendon. Part of the specimens were stained with hematoxylin-eosin (H & E) and in remainder Masson-trichrome stains (modification of Goldner-Foot) were performed.

The histological findings are as follows.

1. Collagen formation was most rich in the unsutured group, moderate in the group sutured with silk, nlyon and dexon and least in the group sutured with stainless steel wire.

2. The greatest foreign body reaction was noticed in the group sutured with silk or dexon, a little with nylon and least with stainless steel wire.

3. The most severe degeneration of the original tendons was seen in the group sutured with stainless steel wire

4. Maturity of the tendon wound and cellularity of fibroblasts were the most satisfactory in the unsutured group and very unsatisfactory in the group sutured with stainless steel wire.

5. Union of ruptured tendon was first effected by proliferation of the sheath tissues.

From these results, it is suggested that the best relative suture material in tendon repair is nylon and that a better repair method of Achilles tendon rupture is conservative treatment. Also the tendon sheath has an important role in early

healing process.
