폐경이행기 및 폐경후 난소의 스테로이드 생성에 관한 연구

Abstract

[한글]연세대학교 의과대학 세브란스병원에 입원하여 선택적 개복술이 예정되었던 폐경이행기 및 폐경기 여성환자 9명을 대상으로 양측 난소적출술시 난소정맥과 말초혈액에서 채취한 혈청 testosterone(T), androstenedione(ADD) 및 estradiol 농도를 측정하였다.

여기에 7명의 젊은 가임여셩 말초혈액의 혈청 T, ADD 및 estradiol 농도를 측정하여서 서로를 비교관찰하여 다음과 같은 결과를 얻었다.

1. 측정된 모든 홀몬농도는 난소정맥에서 말초혈액보다 높아 있었다.

2. 난소정맥과 말초혈액에서의 농도차이는 testosterone 경우, 1.5배(0.61 ± 0.09ng/ml vs 0.39±0.05 ng/ml, p<0.01); androstenedione 경우, 1.6배(4.43±0.87ng/ml vs 2.79±0.57ng/ml, p<0.01); 그리고 estradiol 경우, 6.6배(366.32±91.37 pg/ml vs 54.55±11.17pg/ml, p<0.01)이였다.

3. 혈청 testosterone 및 androstenedione 농도에 있어 난소정맥과 말초혈액간에는 유의한 상관관계가 있었다.(상관계수 r=0.85, r=0.87). 반면에 estradiol 농도에서는 난소정맥과 말초혈액간에 유의한 상관관계가 없었다.

본 연구의 결과로 폐경이행기 및 폐경후 난소는 계속하여 testosterone androstenedione 분비를 적극적으로 하고 있음을 확인하였다.

[영문]The concentrations of testosterone, androstenedione, and estradiol were measured in peripheral and ovarian vein sera obtained at the tine of surgery from 9 perimenopausal and postmenopausal women undergoing bilateral oophorectomy.

A higher ovarian than peripheral vein concentration was found for all hormones studied.

The differences between ovarian and peripheral vein concentrations were 1.5 fold for testosterone(0.61±0.09ng/ml vs 0.39±0.05ng/ml, p<0.01) ; 1.6 fold for androstenedione (4.43±0.87ng/ml vs 2.79±0.57ng/ml, p<0.01) and 6.6 fold for

estradiol(366.32±91.37pg/ml vs 54.55±11.17pg/ml, p<0.01).

There were positive correlations(r=0.85, r=0.87) between peripheral and ovarian vein testosterone and androstenedione levels significant(p<0.01).

But there was no correlation between periphral and ovarian vein estradiol levels.

We have concluded from this study that the perimenopausal and postmenopausal ovaries continue to secrete a significant amount of testosterone and androstenedione.
