안면골 골절에 대한 임상적 고찰

dc.description.abstract[한글]안면골격은 수개의 융기부위 (protuberance )와 궁상구조 (orltal rim nasal pyramid, zygomatic arch and maxilla)로 구성되어 있어 각종사고나 외상에 쉽게 노출된다. 인간의 두뇌가 발달함에 따라 날로 사회생활이 복잡하여지며 교통수단 역시 고속, 대형화 됨으로써 문명의 이기로 인간의 사랑을 받아 왔으나, 이에 비례하여 교통사고 역시 대형화하여 증가일로에 있어 심지어는 달리는 흉기로까지 표현되며 현대의 심각한 사회적 문제로 발전하였다. (Hir,1969 schults, 1975) Braunatein (1957)에 의하면 1000건의 자동차 사고중 발생한 2253명의 수상자의 72.3%인 1213명에서 안면 및 두부손상을 나타냈으며 이중 7.2%에서 안면골 골절상을 보였다고 하였다. 안면골 손상은 정확한 진단과 그에 따른 적절한 치료가 중요함에도 불구하고 우리나라에서는 과거에 이에 대한 관심도가 퍽 미약했던 것이 사실이다. 그러나 다행히 최근 수년동안 안면손상의 중요성과 그 처치방법에 대한 연구가 활발히 진행되어 국내 및 해외 문헌에 많이 보고되어 있다. 저자는 1973년 3월 1일부터 1977년 9월 30일까지 만4년 6개월간 연세의료원에서 안면골 골절로 입원하여 치료받은 환자들의 임상적 소견과 치료방법등에 대해 고찰하여 다음과 같은 결과를 얻었다. 1. 월별 분포를 보면 5월달이 32명으로 제일 많았고 1월달이 15명으로 제일 적었다. 2. 종 245명의 안면골 손상 환자중 남자가 206명 (84.1%) 여자가 39 명 (15.8%)이며, 10 ∼ 40세에서 많이 발생했다. 3. 가장 많은 손상 원인는 교통사고였다. 4. 하악골골절이 101예 (41.2%)로서 가장 많았고, 37예에서 2군데 이상의 골절을 동시에 가졌다. 5. 안면골골절의 동반골절의 빈도는 다음과 같다. 1) 두부 : 22예 2 )상지 : 21예 3) 하지 : 19예 6. 연부조직손상의 빈도는 다음과 같다. 1) 안면부 및 말초부 열상 : 74 2) 신체 중심부의 심한 피부찰과상 및 내장장기손상 : 5예 3) 안부손상중 : 5예 7. 생명에 지장을 주는 동반손상의 빈도는 다음과 같다. 1) 뇌좌상 : 33예 2) 기도폐쇄 : 8예 3) 혈흉 및 기흉 : 6예 8. 치료는 수상후 50%에서 24 ∼ 48시간내에 시행했고 18예 (7.31%)에서 보존요법으로 치료했다. 9. 폐쇄정복술만을 시행했던 경우가 81예 (33.1%) 개방정복술만을 시행했던 경우가 37예 (15.1%)였다. 10. 합병증의 빈도는 다음과 같다 1) 고열 : 7예 2) 국소상처감염 : 6예 3) 흉부 합병증 : 4예 4) 사망 : 4예 4명의 사망예중 3명은 심한 두부손상으로 1명은 복부장기의 파열로 사망했다. [영문]The facial bones are composed of several protuberance and arches (orbital rim, nasal pyramid, zygomatic arch and maxilla) and are apt to be damaged by the accidents. According to the intellectual development of the human, the social life is constantly complicated more than ever. The means of transportation has been increasing in scale and size, consequently the automobile accidents has been also increasing the same as the transportation has been. Moreover the means of transportation has been expressed as the moving weapon which became very serious problem. Braunstein(1957), in a study of 1000 automobile accidents, reported that the head and face area were involved in 72.3% and that, 7.2% of these also sustained fracture of one or more facial bones. The accurate diagnosis and treatment of the facial injuries were always very important, but the interest of this field was very weakened. During recent years, numerous articles have documented the steady increase in the incidence of facial bone fracture which have been observed within the framework of our mobile anf violent society. Author reviewed and evaluated clinical patterns and proper management of fecial bone fracture patients who were admitted and treated in Yonsei Medical Center from March 1, 1973 to September 30, 1977. There were 245 cases of facial bone fractures in that period. The following results were obtained, 1. Monthly incidence of facial bone fracture showed 32 cases in May at the most and at the least in January 15 cases. 2. During this period, a total of 245 patients with fractures of the facial bone were admitted to the hospital. The sex incidence revealed 206 patients(84.1%) in male and 39 patients(15.8%) in female. The great incidence of the fracture occurred in the age group of between 10 to 40 years. 3. Traffic accidents ranked the top as the cause of facial bone fracture. 4. Mandibular fractures were most common facial bone fracture (33.5%) dering that period. 37 patients had multiple facial bone fractures. 5. Associated fracture is as follow in order of frequency 1) Skull: 22 cases 2) Upper extremities; 21 cases 3) Lower extremities: 19 cases 6. Soft tissue injury is as follow in order of frequency 1) Facial and peripheral laceration: 74 cases 2) Severe skin abrasion and contusion, central: 5 cases 3) Eye injury; 5 cases 7. Life threatening associated injury as follow in order of frequency 1) Cerebral contusion: 33 cases 2) Airway obstruction: 8 cases 3) Hemo-pneumothorax: 6 cases 8. Almost 50% of the cases were treated within 48 hours after injury. 18(7.3%) patients were treated, conservatively. 9. Closed reduction alone was done for 81(33.1%) cases of our patients, while 37(15.1%) done open reduction. 10.
