Hoffmann 외고정 장치를 이용한 경골 골절 치료에 관한 임상적 고찰

Abstract

[한글]외고정 장치에 의한 골절치료의 방법은 새로운 방법의 하나로 개량, 발전되어 왔다. 특히 Vidal(1983)에 의해 개량된 Hoffmann 외고정 장치는 보다 견고한 고정이 가능하여 심한 연부조직 손상이 동반된 골절에 유용하게 사용될 수 있었다.

저자는 1981년 10월부터 1983년 12월까지 연세대학교 의과대학부속 세브란스병원 정형외과에서 Hoffmann 외고정 장치를 이용하여 치료를 하였던 경골 골절 환자 28예를 대상으로 이 장치의 장점, 단점 및 합병증 등에 관한 결과를 분석 검토하여 다음과 같은 결론을 얻었다.

1. 성별로는 남자가 25예(89.3%), 연령별로는 20세에서 39세까지가 15예(53.7%)로 가장 많았다.

2. 발생원인으로는 교통사고 22예(78.6%), 산업재해 5예(17.9%)로 high energy injury로 인정되는 원인이 대부분이었다.

3. Hoffmann 외고정 장치 평균 착용기간은 16.6주였으며 평균 골유합기간은 30.1 주였다.

4. 지연유합 및 불유합 15예 (53.6%)등 많은 합병증이 있었으나 Hoffmann 외고정 장치 자체의 문제보다는 대부분 골절의 성격으로 인한 합병증으로 인정되었다.

5. Hoffmann 외고정 장치로 비교적 견고한 고정과 정확한 골절 정복이 가능하였고 다발성 외상 환자에서 타 부위 손상에 대한 처치를 쉽게 할 수 있었다.

6. Hoffmann 외고정 장치 착용중 많은 수술을 병행할 수 있어 환자당 평균 2회의 수술이 시행되었다.

7. Hoffmann 외고정 장치는 심한 연부조직 손상이 동반된 골절에 유용하게 사용되어한 처치를 쉽게 하였다.

[영문]External fixation was improved in its devices and techniques as a specific method of fracture treatment. It was possible to immobilize the fractures firmly with the use of Vidal modified Hoffmann external fixation and could be used effectively in difficult fractures with severe soft tissue injury From October 1981 to December 1983, 28 patients of tibial fracture who had been treated with Hoffmann external fixation at Severence Hospital, Yonsei university College of Medicine were studied and analyzed retrospectively and the results are summerized as follows :

1. The age of patients varied from 14 to 66 years old, the majority(53.6%) being between 20 to 39 years old and males comprized 89.3%.

2. Traffic accidents and machinary injuries comprized 96.4%.

3. Average duration with Hoffmann frame was 16.6 weeks and average time of bone union was 30.1 weeks.

4. Many complications were noticed but majority were related to personalities of the fractures , not due to Hoffmann external fixation itself.

5. Hoffmann device was relatively easy and quick to apply: sufficient stability was obtained: reasonable anatomic reduction of major fragments was possible : facilitated treatment and care of polytraumatized patients.

6. Secondary operations, e.g.,bone grafting and even plastic surgery, had been possible without necessitating removal of the device.

7. It was useful in open fractures with severe soft tissue injury and also in taking care of further necessary managements.
