건강신념이 치료지시이행에 미치는 영향에 관한 연구 : 서울시 일부여자대학생을 대상으로

Abstract

[한글]건강신념모형 (Health Belief Model)을 이론적 근거로 한 치료지시이행에 관한 본 연구는 이화여자대학교 대학보건소에서 건강검사시 일반질환과 치과질환 모두 치료지시를 받은 학생 149명을 대상으로 1985년 10월28일∼11월2일 기간에 설문조사를 실시한 결과 91명이 조사에 응답하였다.

1984년 이화여자대학교 대학보건소 건강검사시 치료지시 받은 학생의 이행률을 보면 일반질환인 경우 51.3%인데 반해 치과질환은 5.4%로 큰 차이를 보이고 있다. 이에 본 연구는 각 질환의 치료지시이행에 관련되는 제요인을 알아보고, 영향력을 미치는 요인이 무엇인지 파악하는데 그 목적이 있다.

이러한 목적을 위하여 "건강신념과 치료행위모형"을 설정하여 분석한 결론은 다음과 같다.

1. 질환의 감수성에 영향을 미치는 요인으로는 질환의 기왕력, 질환명 인지, 진단에 대한 믿음 등으로 나타났다. 단 치과질환에서는 질환명 인지와 진단에 대한 믿음만이 영향을 미치는 것으로 나타났다.

2. 질환의 심각성에 영향을 미치는 요인으로, 일반질환의 경우 질환의 기왕력만이 영향을 주는 것으로 나타났으며, 치과질환에서는 질환의 기왕력, 치료권유와 관계가 있는 것으로 나타났다.

3. 치료지시이행과 질환의 감수성, 심각성, 이익성과의 관계 분석에서는 일반질환에서 심각성만이 영향을 미치는 것으로 나타났고, 치과질환에는 3가지 변수 모두가 영향을 미치지 않았다.

4. 치료지시이행과 제 변수간의 분석에서는 일반질환, 치과질환 모두 질환명 인지가 치료지시이행에 영향력을 미치는 요인으로 나타났다.

[영문]A theoretical framework of "Health Belief Model" were used examine the factors which are assumed to impact upon health belief and treatment behavior. The survey was carried out from Oct. 28 to Nov. 2, 1985 with a representative sample of EWHA womans University students (N=91) by structured questionaire. The results of Analysis of Variance and multiple regression analysis are as follows:

1. According to the results of Analysis of Variance, knows name of disease effects significantly on the treatment behavior (disease: p<0.01, dental disease; p<0.01).

Student's prior experience that illness, knows name of disease, belief in diagnosis effects significantly on the perceived susceptibility.

Student's prior experience that illness, belief in diagnosis effect significantly on the perceived severity.

2. According to results obtained by multiple regression analysis, the susceptibility about disease is affected by student's prior experience that illness (β=0.3146305), belief in diagnosis (β=0.2701096), knows name of disease (β=0.2572760) at significant level (p<0.05).

The severity about disease is only affected by student's prior experience that illness (β=-0.2290330).

The susceptibility about dental disease is affected by knows name of disease (β=0.2804311), and belief in diagnosis (β=0.5014658) at very significant level (p<0.01).

The severity about dental disease is affected by student's prior experience that illness (β=0.2475175), advice from other(β=-0.1995435) (p<.05).

The factory affecting the treatment behavior in disease is knows name of disease (β=0.4992968) (p<0.01).

The factor affecting the treatment behavior in dental disease is knows name of disease (β=0.2455705) (p<0.05).

In this point, it can be assumed that treatment behavior is affected only by the knows name of disease.
