외래 물리치료실 이용환자의 만족도에 미치는 요인조사

이 연구는 외래 물리치료실을 이용하고 있는 환자들이 물리치료 서비스에 대하여 어느

정도 만족하는가와, 어떤 요인들이 만족도에 영향을 미치는가를 알아보기 위하여 서울지

역에 위치한 의료기관 중 임의로 선택한 20개 의료기관의 외래 물리치료실을 이용하고 있

는 환자 400명을 대상으로 조사하였다.

연구에 사용한 구조화된 설문지는 사전조사를 통해 수정, 보완하였으며 본 조사는 1990

년 2월 23일 부터 같은해 4월 23일까지 시행하였다. 총400건이 분석에 사용되었다. 만족

도 측정도구의 신뢰도를 평가하기 위한 Cronbach's alpha의 내적일치도값은 0.85였다.

이 연구의 결과는 다음과 같다.

1. 조사대상자의 성별분포는 남자가 257명(64.4%)이었고, 여자는 143명(35.6%)이었다.

연령은 평균 35.2세이었고, 교육수준은 조사대상자의 81.8%가 고졸이상이었다. 직업은 직

종별로 다양한 분포를 보였고, 조사대상자의 67.1%가 종교를 갖고 있었다. 가족수는 평균

4.5명이었고, 가구 총 수입은 100만원이상이 30.5%로 가장 많았다.

2. 의료기관 접근도의 특성으로 고통수단은 버스를 가장 많이 이용하고 있었으며, 집에

서 의료기관까지의 소요기간은 평균 42.4분이었다. 일일 물리치료 비용은 평균 약 4,600

원이었다. 물리치료실 이용횟수는 평균 65.2회이었고, 치료전 대기시간은 평균 20.5분이

었다.

3. 외래 물리치료실 이용환자의 만족도 점수는 평균 113.9점이었고, 최소점수는 73점,

최대점수는 146점이었다. 만족도 평균 점수를 100점 만점으로 환산하면 69.0점이었다.

4. 외래 물리치료실 이용만족도는 연령, 결혼여부, 종교유무, 직업, 가구내총수입, 치

료전 대기시간에 따라 통계학적으로 유의한 차이가 있었으나, 교통수단, 내원소요시간,

물리치료비용, 진료과, 병원규모등에 따른 만족도의 차이는 없었다.

5. 만족도에 대한 요인분석에서 8개의 요인이 선택되었다. 첫째요인은 물리치료사의 개

인적인 특성에 대한 문항으로 구성되었고, 둘째요인는 물리치료자체에 대한 만족을 측정

하는 요인들이었다.

6. 물리치료 이용만족도를 예측할수있는 변수로 치료전 대기시간이 가장 예측력이 높은

변수로 선택되었고, 그 다음이 연령, 직업유무, 종교유무의 순이었다. 그러나 이들변수

가 만족도를 예측하는 크기는 적었다.

7. 요인분석 후 선택된 첫째요인을 예측할수 있는 변수는 종교유무와 치료전 대기시간

이었고, 둘째요인는 가구내 총수입이 예측변수였다.

위의 결과들로 보아 외래 물리치료실에서 제공하는 서비스에 대한 환자들의 만족정도는

나쁘지 않았으며, 더욱 양질의 서비스를 제공하기 위해서는 치료전에 기다리는 시간을

최소한으로 줄이는 방향으로 노력해야 할것이다. 추후의 연구에는 신뢰도와 함께 타당도

가 높은 만족도 측정도구의 개발과 함께 수용도, 구비도등이 고려되고, 환자의 질병형태

별 그리고 객관적인 측정이 어려운 노인이나 소아환자들의 만족도를 측정 가능한 측정도

구의 개발이 필요하다고 사료된다.

The purposes of this study were to measure the degree of out-patient satisfaction

with physical therapy service and to identify factors affecting the satisfaction.

Subjects were 400 out-patients (257 males and 143 females) at 20 treatment

facilities in Seoul. Questionnaire survey was conducted from February 23, 1990 to

April 23, 1990. Four hundred valid questionnaires were obtained and analyzed.

Analysis of the data was done by use of t-test, ANOVA, factor analysis and stepwise

multiple regression.

The results of this study were as follows:

1. The mean score for the satisfaction was 113.9 ranging from 73 to 146(Minimum

score=30, Maximum score=150).

2. Sex, age, marital status, occupation, average monthly income and waiting time

for treatment were significantly related to out-patient satisfaction.

3. In order to separate basic factors of the satisfaction, factor analysis with

varimax rotation was performed. Eight factors were derived from the analysis. The

proportion of variance explained by these factors was 56.8%.

4. The? first factor labeled "personal characteristics of physical therapist"

accounted 26.3% of the variance. The second factor labeled "treatment effect of

self-reported physical therapy" accounted 8.6% of the variance.

5. Using multiple regression, the most predictive variable for the total

out-patient satisfaction was waiting time for treatment and the next ones were age,

occupation, religion in that order (R**2=0.061).

6. For the first factor extracted from the factor analysis, the predictive

variables were religion, waiting time for treatment. For the second factor, the

predictive variable was family income.

These results suggest that out-patient satisfaction were largely related to

"personal characteristics of a physical therapist" and "treatment effect of

self-reported physical therapy". Therefore physical therapist should consider

personal characteristics and treatment effect to increase the patient satisfaction.
