인형 거대세포바이러스(Human cytomegalovirus, HCMV)는 Herpesviridae에 속하는 바이

러스로서 흔히 잠복 감염을 일으키며 면역기능 저하 환자에서 치명적인 감염증을 내기도

하고 선천성 감염으로 인한 신체 기형 또는 기능 장애를 유발한다. 최근 감염 환자의 경

우 소변, 질 분비물, 타액등의 신체 분비물로 장기간 바이러스를 유출하는데 감염증의 진

단에는 다른 바이러스 질환과 마찬가지로 세포 배양을 통한 세포병변 효과(cytopathic ef

fect, CPE) 확인 및 분리법, 혈청학적 진단법이 사용된다. 그러나 증식 속도가 느리고 혈

청학적 진단법은 비특이적인 결과가 얻어질 수 있어서 근래에 외국에서는 신속 정확한 진

단법으로서 검체를 접종한 세포 배양상에서 단세포군항체 또는 DNA탐식자 등을 이용한 감

염세포의 탐지법을 시도하여 오고있다.

본 연구는 국내 학자에 의하여 독자적으로 개발된 과산화 효소 결합 HCMV 특이 단세포

군항체 (MCMVA-57, MCMVA-93)를 사용하여 그 대응 항원들의 특성을 규명함으로써 점차 의

학적 관심이 높아가고 있는 HCMV의 실험실내 생물학적 연구 재료로서의 효용성을 제사하

고자 하였다. 또한 한편으로는 실험실 적응주인 AD-169로 만들어진 이들 항체를 세포 배

양상에 검체를 접종한 후 감염 세포의 탐지 또는 소변에서의 직접적인 항원 탐지에 탐식

자로 이용하여 국내 HCMV 감염 환자의 진단에 실제로 이용할 수 있는가를 검토하고자 시

행하였다.

면역 침전법과 autoradiography를 통하여 MCMVA-57은 50Kd, 44Kd, MCMVA-93은 150Kd, 3

7Kd, 27Kd 크기의 항원들에 반응하며 또한 두 항체는 HCMV 중화력이 없음을 알았다. 바이

러스가 감염된 섬유아세포를 Cycloheximide와 Actinomycin D, Phosphonoacetic acid, Cyt

osine arabinoside, Tunicamycin 등의 대사 억제 물질을 처리하여, 3일간 배양한 후 과산

화효소 결합 MCMVA-57, MCMVA-93으로 염색함으로서 각 대응 항원의 생성 시기와 glycosyl

ation 여부를 확인하였다. 그 결과 모두 후기 항원(late antigen)이며 MCMVA-57의 항원은

glycoprotein이지만 MCMVA-93대응 항원은 glycoprotein이 아닌 것을 알았다. 바이러스

입자를 potassium tartrate gradient를 이용한 초고속 원심분리에 의하여 분류하고 dot b

lot로 반응성을 본 결과 MCMVA-57만이 3종류의 바이러스 입자에 반응하는 것을 확인하였

다.

위같은 결과와 감염후 시간 경과에 따른 염색상 변화, 그리고 이미 보고된 단백 항원들

과의 비교등으로 MCMVA-57은 HCMV envelope에 존재하는 glycoprotein과 반응하는 항체이

며 MCMVA-93은 바이러스 구조항원의 전구물질 또는 핵산 결합 단백으로 추정할 수 있었다

이들 과산화효소 결합 단세포군 항체는 acetone, xylol, CCl^^4등으로 고정한 AIDS 환

자의 부검폐 조직과 HCMV가 분리된 환아들의 소변이 접종된 섬유아세포 배양상에서 특징

적인 염색상에 의하여 바이러스에 감염된 세포를 명확하게 찾아낼 수 있게함으로써 두 HC

MV 특이 항체는 국내 HCMV 감염 환자의 진단에 탐식자로 사용될 수 있음을 보였다.

Human cytomegalovirus(HCMV) is one of the members of herpesviridae and known to

cause latent infection. Reactivation of these viruses leading usually to fatal

infections is common in patients who are being treated to suppress the immune

function after organ transplantation. It is also enlisted as one of the causative

agents of fetal death, congenital malformation, defect after birth. Acutely

infected patients secrete HCMV in urine and other secretions. Serology to detect

IgM, IgG and isolation of virus on cell cultures are the traditional methods for

diagnosis. Isolation of the HCMV from the suspected specimens, recognized by

characteristic cytopathic effect is the most reliable method but hampered by the

slow replication. Recently some monoclonal antibodies and DNA probes are used for

rapid and sensitive detection of HCMV infected cells on cell cultures.

However a panel of HCMV specific monoclonal antibodies were reported in 1988, and

peroxidase conjugated MCMVA-57 and MCMVA-93 were kindly provided by Dr. Cha for the

further characterization of the corresponding antigens and its applicability to

wild strain.

Molecular weights of the antigens were determined to be 50Kd, 44Kd in MCMVA-57,

and 150Kd, 37Kd, 27Kd in MCMVA-57 and MCMVA-93. Two monoclonal antibodies could not

neutralize the HCMV Metabolic inhibitors(cycloheximide, actinomycin D,

phosphonoacetic acid, cytosine arabinoside) that could differentiate the stages of

the expression of the HCMV antigens, completely inhibited their production, which

means they are late proteins. Only the antigens reacting to MCMVA-57 were proved to

be glycoproteins and reacted to three kinds of viral particles separated by

potassium tartrate, gradient ultracentrifugation. Dot blot reactivity of the

MCMVA-93 was not shown with the viral particles but with the viral stock solution

prepared by repeated freezing and thawing.

Those results and characteristic stained morphology suggest that MCMVA-57 is

reactive to one of the glycoproteins on the viral envelope and MCMVA-93 is reactive

to one of the precursor of the capsid protein.

Three kinds of fixatives, acetone, xylol, CCl^^4, could be used efficiently for

the fixation of the suspected specimen.

On the cell cultures which were inoculated with urine of two patients and in the

autopsy specimen of the AIDS patient, peroxidase conjugated MCMVA-57 and MCMVA-93

could be used efficiently to detect HCMV infected cells. Thus it must be evident

that they are proper probes for the detection of the HCMV antigens in clinical

specimens.
