소아 철결핍성빈혈에서의 혈소판수 변동에 관한 고찰

철결핍성빈혈은 소아에게 가장 흔한 영양결핍성빈혈이다. 철결핍성빈혈에서의 혈소판변

동에 대한보고는 많이 있으나 그 결과에 대하여서는 저자에 따라 견해차가 있다.

이에 저자는 철결핍성빈혈환아에서 빈혈의 정도와 철분치료에 따른 혈소판수의 변화를

관찰, 검토할 목적으로 본 연구를 시행하였다.

1975년 1월부터 198l년 6월까지 만 5년 6개월동안 연세대학교 부속 세브란스병원 소아

과에서 철결핍성빈혈로 진단받은 83예와 대조군 23예를 대상으로 다음과 같은 결과를 관

찰하였다.

소아 철결핍성빈혈은 연령별로 2세 이하가 61 percent로 가장 많았으며 그 원인은 필요

한 철분의 섭취부족이 대부분이었고, 그 다음으로는 10세 이상이었는데 주로 출혈로 인한

철분소실에 의한 것이었다.

혈소판수는 철결핍성빈혈환아군에서 정상보다 의의있게 증가되어 있었다(P<0.05).

빈혈의 정도에 따른 관찰에서는 혈색소가 7 gm/dl이하이었던 군에 비하여, 7 gm/dl 이

상이었던 군에서 혈소판수가 의의있게 증가되어 있었다(P<0.05).

망상적혈구수, 비종대 및 출혈유무, 혈청철치, transferrin포화도 등과 혈소판수 간에

는 의의있는 차이를 볼 수 없었다 (P>0.05).

철분치료를 함에 따라 혈소판수는 증가 혹은 감소하여 정상범위내로 환원되는 것을 볼

수 있었다.

Iron deficiency anemia(IDA) is the most common nutritional anemia among children.

A study on the changes of platelet count in IDA was conducted with 83 children who

were diagnosed at the Severance Hospital in a 5½-year period from Jan. 1975 to

June 1981. The results are summarized as follows.

2. The platelet count was more significantly increased in patients with IDA than

children without IDA.

3. In IDA the platelet count of patients with 7 gm/dl or higher hemoglobin levels

was significantly higher than the groups with hemoglobin levels lower than 7 gm/dl

(P<0.05).

4. In IDA the platelet count was correlated with neither reticulocyte count,

splenomegaly, bleeding and serum iron, nor transferrin saturation.

5. Oral iron therapy modified the platelet count (increased or decreased) toward

the normal range.
