Kawasaki 병에서 가용성 Interleukin-2 수용체에 관한 연구

dc.description.abstract[한글] Kawasaki병은 1967넌 급성 열성 피부 임파절 증후군이라는 병명으로 보고된 이래 많은 연구가 진행되어 왔으나 아직 정확한 병인론은 밝혀지지 않고 있다. 근자에 와서 이 질환 의 검사소견상 여러가지 면역 이상 소견이 보고되면서 이 질환이 면역성 질환으로 생각되 고 있다. 저자는 Kawasaki병에서 면역계의 활성화를 측정하는 면역학적 지표중에서 급성기의 가 용성 interleukin-2 수용체(sIL-2R)치의 번화 양상을 관찰하고 다른 질환 및 정상 대조군 과 비교하며 회복기로 진행함에 따른 sIL-2R치의 변화를 알아보고 혈중 interleukin-5(IL -6) 및 C-reactive protein(CRP)치와 비교 하여 이 질환에서의 벙리생태를 규명하여 보고 자 본 연구를 시도하였다. 1990넌 9월부터 1991닌 7월까지 세브란스 병원에 내원하였던 Kawasaki병 환아 20명, 급 성 임파구성 백혈병 환아 9멍, Enterovirus에 의한 뇌막염 환아 8명, 전신성 홍반성 낭창 환아 12멍 및 정상 대조군 환아 7멍을 대상으로 하였고, 각 환아의 헐청은 -70。C에서 냉동 보관후에 검사를 시행하였다. sIL-2R치는 T Cell Sciences사의 kit틀 이용하여 ELIS A 방법으로 측정하였고, IL-6치는 Fujirebio Inc.사의 kit를 이용하여 ELISA 방법으로 측 정하였으며 CRP는 TDx기계를 이용하여 fluorescenoe polarization immunoassay 방법을 사 용하여 다음과 같은 결과를 얻었다. Kawasaki 병에서의 sIL-2R치는 급성기에 3,275±1,780 (펑균±표준펀차)U/ml 로 정상 대조군의 315±145 U/ml에 비해 현저한 증가를 보였다. 이들 20명의 Kawasaki 병 환아중 13명에서 아급성기 및 회복기에 추적 검사를 하였는데 1,841±930 U/ml로서 급성기에 비 하여 통계학적으로 의의 있게 감소하였으나(p<0.01), 정상 보다는 계속 높은 치를 유지하 였다. 급성임파구성 백혈병, 뇌막염, 전신성 홍반성 낭창 등애서의 sIL-2R 치는 각각 1,3 79± 1,290 U/ml, 1,290±257 U/ml, 820± 321 U/ml로서 정상 데조군 보다는 높지만 Kawa saki병의 급성기와 비교할때는 훨씬 낮은 수준이었다. Kawasaki 병 환아의 급성기 혈청에 서 Interleukin-6(IL-6)치는 급성기에 136.9±24,5 pg/ml이었고 회복기에는 20.9±9.8 pg /ml 였는데 sIL-2R와의 관계를 보면 직선적인 상관성을 보여서 이는 IL-6 치의 증가가 T 세포의 활성화와 관련이 있는 것으로 사료되었다. CRP의 경우에는 Kawasaki 병 환아에서 급성기에 13.9±6.5 mg/dl 로서 증가된 양상을 보였으나 sIL-2R 치와 직선적인 상관 관계 는 보이지 않았다. 혈소판수는 급성기에 389,000±84,000/mm**3이었고 회복기에 763,000 ±35,OO0/mm**3 이었는데 회복기에 증가된 혈소판이 증가된 soL-2R와 상관관계가 있는 지 를 조사해 보았으나 관계가 없었다. 급성기의 Kawasaki 병 환아에서 T 임파구의 활성화를 의미하는 혈청sIL-2R치가 현저하 게 증가되어 있으며, 회복기에 들어서면서 sIL-2R 치가 뚜렷하게 감소하지만 정상에 비하 여 어느정도 지속적으로 증가되어 있는 것은 임파구의 활성화가 지속되어 있음을 의미한 다. 이러한 혈청 sIL-2R치의 증가는 혈청 IL-6치의 증가와 상관관계를 보이고 있어서, Ka wasaki 병에서는 혈청 IL-6치의 증가가 T 세포의 활성화와 관련이 있는 것으로 사료된다. [영문] Kawasaki disease has been known as an acute febrile mucocutaneous lymph node syndrome since 1967. But its definite pathophysiology still retains unclear. In order to study the immunologic basis for imnunologic abnormalities in this disease, we studied the level of soluble interleukin-2 receptor(sIL-2R), interleukin-6(IL-6) and C-reactive protein(CRP) at the acute and convalescent phases of the disease, comparing therm with the same phases in other diseases and a control group. The subjects were 20 patients with Kawasaki disease, 9 patients of acute lymphocytic leukemia, 8 patients of enteroviral meningitis, 12 patients of systemic lupus erythematosus and 7 of a control group whose members had visited Severance Hospital from Sept. 1990 to July 1991. Al1 the sera were kept in deep freez at -7O。C until use. The serum sIL-2R levels were measured by the Sandwich ELISA method of T cell sciences, the serum IL-6 levels by the Sandwich ELISA method (Fujirebio Inc, Tokyo, Japan), and serum CRP levels by fluorescence polarization immunoassay(TDx: Abbott Inc) . The results were as follows: In the acute phase of kawasaki disease, the mean serum sIL-2R level was 3,275±1,780 U/ml which were significantly higher than 315±145 U/ml in the control group and suggested some relation to T-cell activation. In 13 out of 20 Kawasaki patients the lean serum sIL-2R levels at the subacute and recovery phases were 1,841±930u/m1, significantly lower than that of acute phase but still higher than control group. The sIL-2R levels in acute lymphocytic leukemia, menigitis and systemic lupus erythematosus patients were 1,290±257U/ml, 1,290±257 U/m1 and 820±321 U/ml each. All levels were higher than those founded in the control group but significantly lower than those of acute phase Kawasaki disease patients. The mean observed level of IL-6 in the acute phase of Kawasaki disease was 136.9±24.5 pg/ml. There was linear correlation between serum sIL-2R and IL-6 and increased IL-6 levels suggesting some relation to T-cell activation. The mean level of CRP in the acute phase was 13.9±6.5 mg/dl, but there was no linear correlation with sIL-2R levels. Increased platelet counts in the recovery phase did not relate with sIL-2R levels. In conclusion, serum sIL-2R levels increased significantly in the acute phase of Kawasaki disease and decreased significantly in the recovery phase but it was consistently higher than control group. This suggested persistent activation of lymphocytes during the disease. Serum IL-2R levels showed linear correlation with serum IL-6 levels. Thus, increased serum IL-6 levels seemed to have some relation to T-cell activation.-
