이 연구는 간호의 질 보장을 위한 과정적 접근 방법의 평가도구를 사용하여 군병원 입

원환자가 지각하는 간호행위의 질을 측정함으로써 어느 정도 질적인 간호를 받았는지 확

인하고 현재 시행되고 있는 간호활동 상태를 파악하여 군병원 간호의 질 개선에의 방향을

제시해 주는 기초 자료를 제공하고자 시도하였다.

1991년 4월 21일부터 4월 24일까지 서울 및 경기도 내에 소재한 3개의 군병원에 입원한

환자 중 493명을 대상으로 Wandelt와 Ager의 질적간호평가도구(Quality Patient Care Sc

ale)를 기초로 연구자가 고안한 질문지로 조사를 실시하였다. 통계적 처리방법으로는 백

분율, 산술평균 및 표준편차를 구하고 t-test, ANOVA로 분석하였다.

연구결과는 다음과 같다.

1. 대상자의 일반적인 특성에서 평균연령은 23.1세로 21세에서 25세에 88.6%, 계급은

병사가 92.9%, 입원기간은 2개월 미만이 59.4%로 가장 많았다. 교육정도는 대상자 모두가

고졸 이상의 학력을 소유하고 있었고 평균교육연수가 13.3년이며 53.1%가 대학 이상의

학력을 소유하고 있었다. 종교는 기독교가 36.9%로 가장 많았으며, 결혼 상태는 미혼이 8

9.7%이며, 후송경험이 없는 사람이 73.8%, 출신지는 대도시가 41.4%로 많았다. 대상자의

입원과목의 분포는 외과계가 61.6%로 가장 많았고, 입원경험이 없는 사람이 전체 대상자

의 74.2%이며, 군병원에 입원경험이 있는 자는 전체 대상자의 10.1%, 입원횟수는 1회 입

원이 8.5%로 가장 많았다. 사회병원 입원경험이 있는 사람이 전체의 16.6%이며, 1회 입원

이 11.4%로 가장 많았다. 생활수준은 중정도가 76.7%였고, 수술경험이 있는 자가 20.5%,

사회병원 수술경험자는 15.6%, 군병원 수술경험자는 4.9%였다. 입대전 직업은 49.1%가 재

학생이거나 재수생이었다.

2. 대상자가 지각하는 간호행위의 질 정도는 5개 간호행위 영역내 47개 문항의 질 평점

척도분포가 3.00 - 3,86으로 「보통」인 것으로 나타났고, 47개 문항에 대한 질의 평균

평점 3.46이었다.

3. 47개 간호행위 문항 중 간호의 질 평점이 가장 높은 것은 "정맥주사를 맞았을 때 알

맞은 속도가 유지되며 붓지 않는지 확인한다"(3.86/5.00)이었고, 가장 낮은 것은 "입원시

병실의 제반시설(콜벨, 음료수시설, 냉난방장치, 목욕탕, 화장실, 식당 등)에 대하여 설

명해 준다"'(3.00/5.00)였다.

4. 5개 영역별 간호의 질 정도가 평균평점에 의해 높은 순위는 전문적 간호(3.61), 의

사소통적 간호(3.56), 신체적 간호(3.43), 정신사회적 간호(3.42), 일반적 간호(3.35)순

으로 나타났다.

5. 간호행위 질 정도와 일반적 특성과의 관계를 보면 간호행위 전체 영역의 평점에 영

향을 미치는 변수는 입원기간(F=7.35, p<0.005), 결혼상태(F=3.53, p<0.05), 입원과목(F=

4.71, p<0.05), 생활수준(F=2.02, p<0.05)이었다. 정신사회적 간호의 질 평점에 영향을

미치는 변수는 입원기간(F=7.46, p<0.005), 결혼상태(F=3.81, p<0.05), 입원과목(F=4.71,

p<0.005), 생활수준(F=3.34, p<0.05)이고 신체적 간호의 질 평점에 영향을 미치는 변수

는 연령(F=4.26, p<0.01), 입원기간(F=6.77, p<0.0005), 결혼상태(F=6.18, p<0.01), 입원

과목(F=4.17, p<0.01)이었다. 일반적 간호의 질평점에 영향을 주는 변수는 입원기간(F=6.

37, p<0.05)이었다. 그리고 전문적 간호의 질에 영향을 주는 변수는 입원기간(F=6.13, p<

0.005), 결혼상태(F=3.37, p<0.05), 입원과목(F=2.84, p<0.05)이었다.

결론적으로 입원환자가 지각하는 간호행위의 질은 모든 영역에서 「보통」정도이며, 이

중에서 일반적 간호의 질적 수준이 가장 낮은 것으로 나타났고 전체 간호행위의 질 정도

는 입원기간, 결혼상태, 입원과목, 생활수준에 따라 유의한 차이가 있는 것으로 나타났다

.

군병원 임상간호의 질적인 향상을 위해서는 이러한 저해 요인들을 재검토하여 그에 따

른 변화를 시도해야 될 것이며 이 연구에서 고려하지 못했던 점들과 제한점을 보완시키는

연구가 필요하다고 생각한다.









[영문]

The purpose of this study was to measure the level of quality care in nursing

that the patients perceive in milltary hospitals, thereby to provide the basis for

improvement of quality of nursing care in military hospitals.

The subjects for the study were 493 patients who had been hospitalized for three

days or more. They were selected from three military hospitals in Seoul and Kyungki

area during the period from April 21 to April 24, 1991. The instruments used for

this study are one developed by the researcher through literature review and a

quality patient care scale developed by Wandelt and Ager. Data were collected from

the subject using the questionnaire method and were analyzed in terms of

percentage, mean, standard deviation, t-test, ANOVA.

The results of this study can be summarized as follows :

1. The general characteristics of the subjects were : The age range 21-25 (88.6%)

: The rank of the majority of the soldiers was enlisted (92.9%) : The duration of

hospitalization was less than 2 months (59.4%) : All of the subjects had a least

high school graduation (100.0%) with more than half being college students (53.1%)

: The Christian religious was most frequent(36.9%) : Most were not married (89.7%)

and not transfered (73.8%) : Most were resident in city : Most were admitted for

surgical procedures (61.6%) and had not had a precious hospitalization experience

(74.2%). For those who had not had precious hospitalization experience, military

hospitalization experience was not frequent(10.1%) and the number of

hospitalizations was once(8.5%) followed by community hospitalization(16.6%) with

the number of hospitalization also being one(11.4%). Those with average economic

status were most frequent as were (76.7%) : Surgical operation experience (20.5%)

in community hospitals (15.6%) and in military hospitals (4.9%) : Most had been

student before enlisting military service(49.1%).

2. The level of quality care in the nursing process. The mean distribution of

quality care for 47 items in the five areas of the nursing process was 3.00-3.86.

3. The highest quality level of the 47 items was for "observe and check the

adquate maintainance of TV infusion and TV site swelling". The lowest quality level

of the 47 items was for "explain about hospital facilities (call bell, drinking

water, air conditioner, bathroom, toilet, restaurant) on admission.

4. The highest level of quality care for the area was the area of professional

implication followed by the communication care area, the physical care area, the

psychosocial care area, the general care area in descending order.

5. The relationship between the level of quality care in the nursing process the

general characteristics of subjects showed that in all areas in the nursing

process, the variables with influenced the level of quality care were during of

hospitalization (F=7.35, p<0.005), marital status(F=3.53, p<0.05), department in

which the subject were hospitalized (F=4.71, p<0.05) and economic status (F=2.02,

p<0.05). In the psychosocial care area, they were duration of hospitalization

(F=7.46, p<0.005), marital status(F=3.81, p<0.05), department in which the subject

were hospitalized (F=4.71, p<0.005) and economic status (F=3.34, p<0.05).

In the physical care area, they were age(F=4.26, p<0.01), duration of

hospitalization (F=6.77, p<0.005), marital status (F=6.18, p<0.01), department in

which the subjects were hospitalized (F=4.17, p<0.01), community hospitalization

experience (t=-1.98, p<0.01) and economic status (F=3.63, p<0.01). In the general

care area, they were duration of hospitalization(F=6.37, p<0.005), department in

which the subjects were hospitalized (F=4.48, p<0.005). In the communication care

area, they were duration of hospitalization (F=5.06, p<0.005), department in which

the subjects were hospitalized (F=4.48, p<0.005). In the professional care, they

were duration of hospitalization (F=6.13, p<0.005), marital status (F=3.37,

p<0.05), department in which the subjects were hospitalized (F=2.84, p<0.05).

Based on the results, it is possible to conclude that the level of quality care

in the nursing process that patients perceive in military hospitals was -average-

in all areas. Among the various area of care, the level of quality care in the

general care area was the lowest. In all areas in the nursing process, the

variables which influenced the level of quality care were duration of

hospitalization, marital status, department in which the subjects were hospitalized

and economic status. Therefore it is important motivate nurse to improve the

quality of nursing care an attempt to initiate modification in association with

evaluation of major hindrance to quality care performance of the nursing process.
