한국인에서의 베체트병

베체트병은 전신적으로 여러 기관을 침범 하는 만성 재발성 질환으로 전세계적으로 발

생하나 지중해 연안, 중동 및 극동지방에 높은 발생 빈도를 보이고 있다. 최근 국내에서

도 많은 예가 보고되고 있으며, 베체트병의 한국인에서의 개인적, 지리적 및 임상적 특성

에 대한 기초자료률 얻고자 본연구를 시행하였다.

1983년 11월부터 1990턴 12월까지 연세대학교 의과대학 세브란스 병원 베체트병 특수진

료소에 내원하여 베체트병으로 진단받은 여덟명을 대상으로 시력검사, 세격등 검사 등의

각종 안과검사를 시행하였으며, 안증상 이외의 임상중상과 환자의 개인적, 지리적, 임상

적 병력 등에 대하여 의무기록을 중심으로 조사하여 다음과 같은 결과를 얻었다.

1. 남녀의 성비는 M:F=1:1.57으로 여자에서의 발생이 많았다. 발병 당시 환자의 연력

분포는 20대가 37.8%으로 가장 많았고, 전체 평균 발병 연령은 29.0세였다.

2. 거주지역의 분포는 서을·경기·인천 지방이 84.9%이었다.

3. 연도별 신환수는 전반적으로 중가하는 추세에 있었다.

4. Shimizu분류법에 따르면 완전형 9.3%, 블완전형 37.8%, 용의형 30.5%, 가능형 22.4%

으로 불완전형이 가장 많았다.

5. 주증상은 구강증상이 97.5%으로 가장 많았고, 안중상은 16.4%에서 나타났으며, 부증

상은 관절증상이 29.5%으로 가장 많았다. 최초로 발생한 주증상은 구강궤양이 78.1%으로

가장 많았다.

6. 초발 중상이 나타난 때부터 안증상이 나타난 때까지의 기간은 평균 44개월이 였다.

7. 안중상을 보인 환자는 모두 239명으로 남녀 비을은 1:0.74으로 남자에서의 발생이

많았다. 발병 당시의 연령분포는 20대가 40.6%으로 가장 많았고, 이들의 평균발병 연령은

28.2세였으며, 양안에 나타난 경우가 편안에 나타난 경우보다 많았다.

8. 안증상은 홍채염이 64.0%으로 가장 많았고, 초자체혼타, 속발성백내장, 맥락망막염,

결막염, 망막변성, 속발성녹내장, 망막출혈, 망라박리순이였다.

9. 초진 교정시력이 0.01 이하인 경우가 31.2%으로 가장 많았고, 24개월 이상 추적한

경우 대부분(50.9%)에서 시력의 악화를 보였다.

10. 치료제로는 현재까지는 부신피질호르몬제, cyclophosphamide, colchicine 등이 사

용되었으나, 부신피질호르몬제, colchicine 등의 면역억제제로 치료받았으나 안증상이 호

전되지 않는 환자 중 신장과 간기능이 정상이고 Cyclosporin A(CYA)의 사용에 동의한 환

자 29명 55안에서 CYA로 치료하였는데, 6개월 이상 CYA 사용 후 50%이상에서 치료 전보다

치료 후에 시력이 향상되었다.

Behcet's disease. is a chronic, Multisystemic disorder characterized by oral

ulceration, multiple skin lesions, genital ulceration, and ocular involvements. It

is far More frequently Seen in the Mediterranean basin, Middle East, and Far East.

1454 patients, who had visited the Behcet's Disease Specialty Clinic of Severance

Hospital of Yonsei University College of Medicine from November 1983 to December,

1990, were grouped into 4 groups by Shimizu's classification. Their personal,

geographic, and clinical characteristics were evaluated

Also, 29 Behcet's disease patients with severe ocular involvement and cytotoxic

and/or systemic corticosteroid agent failure were treated with Cyclosporin A(CYA),

and the effectiveness of Cyclosporin therapy was evaluated.

The results are summarized as follows :

1. The disease occurred more often in women than men with a ratio of 1.57 to 1.

The mean age of onset was 29.0 years old, and the age of onset reached its peak in

the third decade.

2. In geographic distribution, 84.9% of the patients were from Seoul, Kyunggi,

and Inchon.

3. By the Shimizu classification, incomplete type was most common, and suspected,

possible, and complete type were observed in order.

4. The most frequent major symptom was oral ulcer(97.5%), and the most frequent

minor symptom was arthritis(29.5%).

5. The most frequent initial symptom was oral ulcer(78.1%).

6. Ocular lesions were appeared in 239 patients, and their male to female ratio

was 1 to 0.74. The mean age of the patients at the time of onset was 28.2 years

old.

7. The mean interval between the initial symptom of the disease and the onset of

ocular manifestation was 44 months, and 24.3% of the patients had the ocular lesion

as the first manifestation.

8. The ocular lesions affected both eyes in 68.6%. The most common Ocular Symptom

was iritis(64.0%), and vitreous opacity, complicated cataract. chorioretinitis,

conjunctivitis, retinal degeneration, complicated glaucoma, retinal hemorrhage, and

retinal detachment were observed in order.

9. 31.2% of the patients had corrected visual acuity of 0.01 or worse on their

first visit, and loss of useful visual acuity had occurred in 50.9% of the eyes

followed for over 24 months.

10. The number of new patients has increased yearly.

11. There was an improvement in visual acuity in majorlty of the patients treated

over 6 months with Cyclosporin A.
