측두 하악관절의 기능적 변화가 있는 환자의 측모두부방사선 계측학적 연구

측두하악관절 및 이와 관련된 동통 및 이상에 관한 병리적 병변의 연구 및 고찰에 있어 병리기전의 혼동과 논란, 과학적인 방법의 결여등으로 이에 대한 연구가 어려웠다. 최근에 여러 진단방법을 통해 "악관절 내장증 "이라는 개념이 대두되면서 악관절 질환의 병리적 기전에 대한 연구가 활발해지고 있으며, 특히 교정적 영역에서의 부정교합과 악관절이상과의 연관성여부는 관심이 되고 있다. 본 연구는 악판절 내장증이라고 진단된 실험군 34명과 악관절 질환 및 장애가 없는 정상군 35명을 대상으로 측모 두부 방사선 사진을 촬

영하여, 골격성 및 치성 계측항목을 측정하여, 계측항목간의 유의성을 검증하여, 아래와 같은 결과을 얻었다.

1 . 제 Ⅰ급 교합관계를 갖는 군에서의 비교에서, 대조군에 비해 실험군에서 하악골이 후하방으로 회전된 양상을 보였고, 상하악치아가 설측경사된 양상을 보였다.

2 . 수직적 관계를 배제하였을 때 , 실험군이 대조군에 비하여, 상하악전치간 각도(interincisor angle) 는 크고 상악전치 설측경사도는 낮은 값을 보였다.

Researches in pathologic entities and syndromes that describe temporomandibular joint(TMJ) or TMJ-related pain and dysfunction, have been hampered by confusion and conflict over the pathogenesis of myofascial and joint pain, the character and

causes of joint noises, and the lack of scientific methods. Investigators have more recently correlated clinical diagnosis and arthrographic and surgical findings to demonstrate "internal derangements" of the TMJ, It is thought that the structural

and functional changes characteristic of internal derangements constitute the principal pathologic entity of the TMJ.

Cephalometric data from a group of 34 subjects with documented functional changes in the TMJ were compared with those of a group of 35 subjects from a control sample.

The results were as follows;

1. Comparison of a group which shows Class Ⅰ relationship, there was a tendency to show clock-wise rotation of mandible and linguoversion of upper and lower anterior teeth in the experimental sample.

2. Excluding the vertical relationship, there was a tendency to show low values of the lingual surface slope of the upper central incisor and high values of the interincisor angle in the experimental sample.
