Purpose: The number of children with serum transaminase elevation by health screen examination is being increased. But there are a few guidelines or reports for them. The purpose of this study is to know its present clinical manifestation and to help their management.

Methods: I collected the clinical informations of children who have visited from January 2004 to December 2007 at Department of Pediatrics, YongIn Severance Hospital, YongIn, Korea for the elevation of hepatic enzyme elevation after it. They have received multiple tests such as anthropometric check, abdominal ultrasonography, serologic biochemical tests. Their features such as annual occurrence, age, sex, symptoms, physical signs, laboratory findings, disease course were reviewed retrospectively.

Results: Their were 37 cases. The annual cases were increased from 2004 to 2007 and rapidly in 2007. It was more common in the group of 10 and more years old (94.6% of all cases), male (78.4%). All of them except 2 cases of hepatitis B (94.6%) showed non-alcoholic fatty liver disease (NAFLD) by ultrasonography. Most of them did not showed any specific symptom and sign. The mean value of initial serum alanine aminotransferase (ALT), asparate aminotransferase, were 108IU/L, 96IU/L respectively. Almost two third of them (65.7%) revealed dyslipidemia. Clinical follow up and management could be done less than half of them and there was no normalization of ALT level.

Conclusion: This study showed that NAFLD is prevalent in boy and 10 and over years old. Management for them was not effective. Further effective management guideline for it should be made in a near future.
