Abstract
Families of ICU patients face substantial psychological stress due to the unexpected and uncertain nature of critical illness. Although they are essential to patient recovery, the mental health needs of ICU family caregivers are often overlooked. Responsibility for addressing these needs remains unclear, and guidance on effective interventions is limited. Despite growing interest in psychological support, existing evidence is affected by inconsistencies in measurement tools, timing, study design, and sample diversity. Given these challenges, there is a clear need for more robust clinical guidance, standardized protocols, and scalable strategies that emphasize long-term mental health outcomes. Advancing research and fostering interdisciplinary collaboration are critical to ensuring caregivers are both acknowledged and adequately supported throughout the critical care process. Box 1 outlines current practical recommendations for improving psychological outcomes in ICU family caregivers. In conclusion, while caregivers play a vital role in recovery, their psychological needs are insufficiently addressed in current practice. The variability in intervention effectiveness and limited long-term data highlight the need for rigorous, standardized research and evidence-based policies that integrate caregiver support into critical care systems.
