지방자치단체 노인복지 예산 규모와 사전연명의료의향서 작성의 관련성

Abstract

급격한 고령화와 도시화라는 전 세계적인 흐름 속에서 우리 한국 사회는 2025년 초고령사회 진입 등으로 지역사회에서의 노년기의 삶, 웰다잉(well-dying)에 관한 관심이 높아졌고 노인복지에 대한 지방자치단체의 역할이 커졌다. 이에 개인의 자기결정권을 보장하는 제도적 장치로서 사전연명의료의향서 작성 등 2018년 연명의료결정제도가 시행되었고 여전히 사전연명의료의향서 작성의 지역별 격차가 있다. 이에 이 연구는 지방자치단체의 노인복지 예산 규모와 사전연명의료의향서 작성 간 연관성을 분석하고자 하였다. 이를 위해 2022년부터 2024년까지 3개년에 걸친 전국 229개 지방자치단체의 자료로 혼합효과모형을 적용하여 분석하였다. 그 결과 60세 이상 1인당 노인복지 예산액이 1백만원 증가할수록 60세 이상 1,000명당 사전연명의료의향서 작성 수가 4.03건 증가하는 경향이 나타났다(β=4.03, p<0.001). 이에 이 연구는 지자체의 노인복지 예산 규모 확대가 노인 삶의 질 향상과 자기결정권 보장에 기여할 수 있음을 시사하며, 지역 중심의 예산 확대 및 적극적인 제도 활성화 마련에 대한 기초자료로 활용될 수 있을 것이다.

Amid global trends of rapid population aging and urbanization, South Korea officially entered a super-aged society in 2025. This demographic shift has intensified societal interest in the quality of life in later years and the concept of well-dying, thereby expanding the role of local governments in elderly welfare. In response, the Act on Decisions on Life-Sustaining Treatment was enacted in 2018 to institutionalize advance directives as a means of safeguarding individual autonomy. However, notable regional disparities in the completion rates of advance directives persist. This study examines the relationship between the scale of local government expenditure on elderly welfare and the number of completed advance directives. A mixed-effects model was applied using data from 229 local governments across South Korea over a three-year period (2022–2024). The analysis revealed that higher elderly welfare expenditure per capita (aged 60 and above) was significantly associated with an increased number of completed advance directives per 1,000 older adults (β = 4.03, p < 0.001). These findings suggest that expanding local government budgets for elderly welfare may contribute to enhancing the quality of life in old age and strengthening individuals’ right to self-determination. This study provides empirical evidence to support region-specific budget planning and to promote more active implementation of the advance directives system.
