완전 절제된 I, II기 폐선암에서 종양 괴사가 예후에 미치는 영향 분석

| dc.description.abstract | 폐암은 국내에서 2번째로 흔한 암으로 사망률이 매우 높은 질환이며, 그 예후는 병기와 흡연력 등을 포함한 다양한 인자에 의해 영향을 받는다. 조기 검진을 통한 진단이 점차 증가하면서 폐암의 예후를 예측하는 연구가 활발하게 진행되어 왔다. 본 연구에서는 I, II기 폐선암에서 종양 괴사가 수술 후 예후에 미치는 영향을 세부 병기로 나누어 확인하고자 하였다. 본 연구는 세브란스병원에서 2015년 1월부터 2020년 12월까지 폐선암으로 수술받은 환자들을 기준에 따라 선정하여 후향적으로 진행하였다. 전체 환자는 1,804명, I기는 1,585명, II기는 219명이었다. 환자 연령의 중앙값은 64세로 나타났고, 흡연력은 33.8%에서, 종양 괴사는 9.1%에서 확인되었으며, 병기별로 I기 환자의 6.2% (IA의 2.4%, IB의 18.9%), II기 환자의 30.1% (IIA의 29.4%, IIB의 31.2%)에서 발견되었다. 전체 환자군에서 재발은 12.5%, 사망은 6.4%에서 확인되었다. I기의 8.5%에서 재발이, 5.1%에서 사망이 확인되었으며, II기의 41.4%에서 재발이, 15.5%에서 사망이 확인되었다. 종양 괴사가 재발에 미치는 영향을 확인해보면, 전체 환자군 (HR (Hazard ratio) 1.64 (1.20-2.25), p=0.002), I기 (HR 2.18 (1.43-3.35), p<0.001), IA (HR 2.72 (1.22-6.07), p=0.015), IB (HR 2.13 (1.30-3.49), p=0.003)에서 유의한 값을 보였지만, II기, IIA, IIB에서는 통계학적으로 유의성이 을 확인하였다. 사망과의 연관성을 확인해보면, 전체 환자군 (HR 2.02 (1.28-3.19), p=0.002)과 I기 (HR 2.06 (1.14-3.73), p=0.017)에서 유의한 값을 보였지만, 세부 병기를 나누어 확인해보면 IA (HR 1.74 (0.50-6.00), p=0.381), IB (HR 1.60 (0.80-3.22), p=0.184)에서 통계적 유의성이 을 확인할 수 있었다. II기, IIA, IIB에서도 통계적인 유의성은 없는 것으로 나타났다. 종양 괴사는 I기 폐선암 환자에서 재발과 사망의 위험인자 중 하나로 고려하여야 하고, 종양 괴사가 확인된 경우 주의 깊게 추적 관찰하여야 한다. Lung cancer is second most common cancer with a very high mortality rate in Korea, and its prognosis is influenced by various factors, including stage and smoking history. As the number of diagnoses through early screening continues to increase, research into predicting prognosis is actively underway. In this study, we aimed to determine the effect of tumor necrosis on the postoperative prognosis in stage I and II lung adenocarcinoma by detailed stage. This study was conducted retrospectively by selecting patients who underwent surgery from January 2015 to December 2020 at Severance Hospital. The total number of patients was 1,804, 1,585 in stage I, and 219 in stage II. The median age of the entire patient was 64 years, smoking history was confirmed in 33.8%, and tumor necrosis was confirmed in 9.1%. By stage, tumor necrosis was found in 6.2% of stage I patients (2.4% of IA, 18.9% of IB), and 30.1% of stage II patients (29.4% of IIA, 31.2% of IIB). In the entire patient group, recurrence was observed in 12.5% and death was observed in 6.4%. In stage I, recurrence was confirmed in 8.5% and death in 5.1%, and in stage II, recurrence was confirmed in 14.4% and death in 15.5%. When examining the effect of tumor necrosis on recurrence, statistically significance was observed in entire group (HR (Hazard ratio) 1.64 (1.20-2.25), p=0.002), stage I (HR 2.18 (1.43-3.35), p<0.001), IA (HR 2.72 (1.22-6.07), p=0.015), and IB (HR 2.13 (1.30-3.49), p=0.003). However, it was not significant in stage II, IIA and IIB. When confirming the relationship between tumor necrosis and death, significant values was found in the entire group (HR 2.02 (1.28-3.19), p=0.002), stage I (HR 2.06 (1.14-3.73), p=0.017). However, when looking at the detailed stage, there was no significance in IA (HR 1.74 (0.50-6.00), p=0.381), and IB (HR 1.60 (0.80-3.22), p=0.184). There was no statistical significance in stage II, IIA and IIB. Tumor necrosis should be considered as one of the risk factors for recurrence and overall survival in patients with stage I lung adenocarcinoma, and if tumor necrosis is confirmed, careful follow-up should be performed. | - |
