Abstract
center dot Historically, exercise was restricted in hypertrophic cardiomyopathy (HCM) due to sudden cardiac death concerns, but evolving guidelines are more permissive based on emerging safety evidence. center dot Mild to moderate physical activity (PA) provides cardiovascular and functional benefits and is center dot Recent evidence demonstrates that vigorous PA improves functional capacity and does not increase adverse events in most low-risk HCM patients. center dot Contemporary guidelines advocate for individualized exercise recommendations based on comprehensive risk stratification-including imaging, arrhythmia assessment, and cardiopulmonary exercise testing-and shared decision-making. center dot More research is needed to define safe exercise parameters for high-risk HCM patients and those with prior exertional events.
