소아물리치료 현황과 개선방안: 해외제도비교 및 장애아동 부모 요구도 분석을 중심으로

Abstract

According to the World Health Organization (WHO), pediatric rehabilitation is an activity that helps children with congenital or neurological damage and is an important medical and welfare that contributes to enhancing the child's capacity after growth in terms of education and social activities. Pediatric rehabilitation is defined as a service. Despite the importance of pediatric rehabilitation, there are many difficulties in pediatric rehabilitation in Korea due to undersupply in the current medical system. In order to address this issue, various improvement measures have been attempted from various angles, including the “Disabled Child Rehabilitation Treatment Project” in 2010 and the “Public Children's Rehabilitation Hospital Construction Project” in 2020. However, a number of pediatric rehabilitation needs are not met yet. Among them, the demand for pediatric physical therapy, which is a field of pediatric rehabilitation, is showing a high demand in terms of both quantity and quality. Pediatric physical therapy is an important rehabilitation treatment for disabled children with brain lesion disorders and developmental delay symptoms due to central nervous system damage, and is an essential treatment for children with disabilities to acquire normal motor development. This paper focuses on this field of pediatric physical therapy and aims to examine the domestic and international status of pediatric physical therapy, suggesting meaningful ways to improve it. To this end, the data from domestic and international academic journals and public information sites were analyzed to compare domestic and foreign systems of pediatric physical therapy. In addition, we analyzed the needs of parents of children with disabilities in Korea through an in-depth interview method, and a method of providing effective pediatric physical therapy that best fits the domestic situation was proposed. The results of the present study are as follows. First, while there have been many recent attempts to improve domestic pediatric physical therapy in terms of welfare and education in addition to medical institutions, the outcomes remain remarkably insufficient compared to the current needs. It is difficult to estimate the exact supply level, even considering the reality of pediatric physical therapy in non-institutional areas. Therefore, we argue that it is urgent to prepare effective countermeasures in order to solve the problem of pediatric physical therapy, which is absolutely insufficient, and propose appropriate information provision, service expansion, professional manpower enhancement, and support policy expansion.

WHO(World Health Organization)에 의하면 소아재활은 선천적 혹은 신경학적으로 손상이 있는 아동의 발달을 돕는 행위이며, 향후 교육 및 사회 활동의 거시적 측면에서 성장 후 아동의 역량을 높이는 데 기여하는 중요한 의료·복지 서비스로 정의된다. 이 같은 소아재활의 중요성에도 불구하고 우리나라의 소아재활은 현행 의료시스템상 과소공급으로 인한 여러 어려움이 지속적으로 나타났다. 이 어려움을 해결하기 위해 2010년 ‘장애아동재활치료사업’을 비롯하여 2020년 현재 ‘공공어린이재활병원 건립사업’까지 다각도로 다양한 개선방안이 시도되고 있다. 그러나 여전히 소아재활의 수요를 온전히 충족시키기에는 절대적으로 그 수가 부족한 실정이다. 그중에서도 특히 소아재활의 한 분야인 소아물리치료에 대한 수요는 양적·질적 측면에서 높은 요구도를 보이고 있다. 소아물리치료는 중추신경계 손상에 따른 뇌병변장애와 발달지연 증상을 보이는 장애아동에게 중요한 재활치료로서, 장애아동이 정상운동발달을 습득하는 데 매우 필수적인 치료이다. 본고에서는 특별히 이 소아물리치료 분야를 집중적으로 조명하여 소아물리치료의 국내외 현황을 살펴보고 그 개선방안을 제시하였다. 이를 위해 국내외 학술지 및 공개정보 사이트의 자료를 분석하여 소아물리치료의 국내외 제도를 상호 비교해 보았다. 그리고 국내 장애아동 부모의 요구도를 심층면접 방식으로 분석하여 국내 실정에 가장 맞는 효과적인 소아물리치료 제공 방안을 제안하였다. 분석 결과는 다음과 같다. 먼저 국내의 소아물리치료는 현재 의료기관 외에 복지·교육적 측면에서 여러 가지 시도를 진행하고 있으나 그 요구도에 비해 여전히 현저하게 부족한 상태였다. 비제도권에서 이루어지는 소아물리치료 실상까지 고려해 본다면 그 정확한 공급 정도를 추정하기는 어렵다. 이에 본고는 절대적으로 부족한 소아 물리치료의 수요 문제를 해결하기 위해서는 실효성 있는 대책 마련이 시급하다고 보고, 이에 대한 개선방안으로 적절한 정보제공, 서비스 확대, 전문인력 역량강화, 지원정책 확대를 제안하였다.
