국소 코르티코스테로이드 사용에 따른 제 2형 당뇨병 발생위험 : 건강보험공단 표본 코호트 2.0을 이용하여

dc.description.abstract연구 배경 스테로이드 유발성 당뇨병(steroid-induced diabetes mellitus, SIDM)는 60년 전 보고된 널리 알려진 부작용이다. 그러나 관련 근거의 대부분은 경구제 사용에 따른 결과로 피부에 바르는 국소제에 관한 근거는 많이 부족하며 특히 아시아인을 대상으로 진행된 선행 연구는 없었다. 따라서 본 연구에서는 한국인을 대상으로 국소 코르티코스테로이드 사용에 따른 제 2형 당뇨병 발생 위험을 사용 여부, 역가, 사용 빈도로 구분하여 알아보고자 하였다. 대상 및 방법 국민건강보험공단 전 국민 표본코호트 2.0(NHIS-NSC2)을 이용하여 국소 코르티코스테로이드 비사용군 14,944명(남 6,508명, 여 8,436명)과 사용군 171,113명(남 94,898명, 여 76,215명)을 약 9년간 추적 관찰하여 제 2형 당뇨병 발생 위험을 Kaplan-Meier 곡선을 통해 비교하고 Cox proportional hazard model을 통해 보정 위험비와 위험 요인을 확인하였다. 연구 결과 연구 결과 한국인에서의 국소 코르티코스테로이드 사용군은 비사용군에 비해 제 2형 당뇨병 발생 위험이 증가하였고(adjusted hazard ratio[aHR] 1.13, 95% CI 1.06-1.20), 약물의 사용 빈도 증가에 따라 발생 위험도도 함께 증가하였으며(1회, aHR 1.07, 95% CI 1.02-1.13, 2-4회, aHR 1.12, 95% CI 1.05-1.20, 5회 이상, aHR 1.27, 95% CI 1.09-1.48), 5회 이상 사용군의 위험도는 전신 코르티코스테로이드 사용에 따른 발생 위험도(aHR 1.20, 95% CI 1.14-1.26)와 유사하였다. 결론 본 연구를 통해 국소 코르티코스테로이드는 사용 여부, 사용 빈도에 따라 제 2형 당뇨병 발생의 위험도가 증가함을 확인하였다. 따라서 국소 코르티코스테로이드 치료를 시작하기 전 검사를 통해 내당능장애와 고위험군을 미리 진단하여 다양한 치료 방법을 고려하고 대체제가 없는 경우에는 치료기간 중 정기적인 모니터링과 당뇨병 증상 및 스테로이드 유발 당뇨병에 대한 교육을 실시하거나 다른 위험 요인을 제거하는 노력을 하는 것이 요구된다. 제 2형 당뇨병 예방 및 안전한 국소 코르티코스테로이드 사용을 위해 빠른 시일 내 좀 더 체계적인 연구를 바탕으로 한 권고안 제시가 되어야 하겠다. Background The steroid-induced diabetes mellitus (SIDM) is a well-known side effect of corticosteroid agents. Diabetes induced by systemic corticosteroid agents has been reported since 60 years ago, however, there is not many evidence regarding the correlation between topical corticosteroid agents and risk of type 2 diabetes. Subject and Method A prospective, nationwide population-based cohort study was conducted using data from the Korea National Health Insurance Service National Sample Cohort Database (NHIS-NSC2). Total 186,057 adults were identified as case subjects with new-onset type 2 diabetes in this study, and 171,113 adults who have been treated with topical corticosteroid and 14,944 adults who have no experience of topical corticosteroid were included and prospectively followed up for approximately 9 years. The outcome was type 2 diabetes incidences rate following topical corticosteroid use, topical corticosteroid frequency and topical corticosteroid potency respectively. Kaplan-Meier curve and Cox proportional hazard model were used to estimate hazard ratio of type 2 diabetes between each group. Results Use of topical corticosteroid increased the risk of type 2 diabetes (adjusted hazard ratio[aHR] 1.13, 95% CI 1.06-1.20). High-frequency topical corticosteroid group (aHR 1.27, 95% CI 1.09-1.48) and mid-strength potency group (aHR 1.15, 95% CI 1.04-1.27) also increased the risk of type 2 diabetes respectively, we observed the mid-strength potency group induced similar risk on type 2 diabetes with what systemic corticosteroid treatment group did (aHR 1.20, 95% CI 1.14-1.26). Conclusion Use of topical corticosteroid was significantly associated with incident type 2 diabetes in this study. This study suggests that clinician consider the risk factors of diabetes to prescribe topical corticosteroid and monitor diabetes risk of patients, especially who is required long term treatment or mid-strength corticosteroid treatment.-
