dc.description.abstractBackground and purpose : Nicotine is an addictive substance. Electronic cigarette(e-cigarette) users can control the amount of nicotine, but the nicotine intake may vary significantly depending on the user's smoking behaviors and nicotine levels in electronic cigarette liquids(e-liquids). In order to determine the total amount of nicotine consumed, the method of measuring the e-cigarette usage was to establish first. In this study, we calculated the total amount of nicotine consumed in a month by multiplying the number of e-liquids bottles used per month and the dose of pure nicotine per e-liquids bottle. Adjusting the sociodemographic and environmental factors, I would like to describe the factors related to the total amount of nicotine consumed(mg/month) and the duration from filling up with e-liquids until emptying completely. Methods : This research was a secondary analysis study based on the data of MFDS "College Student Tobacco Product Functional Evaluation (2015)". Among 2,387 male and female students who participated in the questionnaires at 14 colleges in Seoul, Gyeonggi, Gyeongbuk, Busan, Chungbuk, Daejeon, Jeonju, Gwangju and Jeju, 759 participants who reported the experiences of cigarette smoking, combustible cigarette users(daily smoking, occasionally smoking, past smoked) as well as whether they have ever used an e-cigarette (yes or no) were analyzed. Participants reporting both combustible cigarette users(daily smoking, occasionally smoking) and e-cigarette ever users were classified as dual users. Nicotine dependence was evaluated with Fagerstrom test. Statistical analysis was performed with SAS v9.4. Result : Among 759 final participants who reported the experience of smoking, 322 participants were dual users, 162 participants were cigarette only users, and 92 were e-cigarette only users. The total number of cigarettes per day which was the sum of ‘number of cigarettes per day’ and ‘total amount of puff number per day converted into the number of cigarettes’ were calculated as 28.1 cigarettes per day for male dual users, and 16.7 cigarettes per day for female dual users which were higher than combustible cigarette or e-cigarette only users(for male, p=0.0037, for female, p=0.0047). The median of total amount of nicotine consumed during the past month(mg/month) was calculated as 30.0mg/month in dual users that showed higher tendency when compared with 17.5mg/month in e-cigarette only users(p=0.3747). ‘Increased amount of nicotine consumed’ and ‘quick consumption completely after filling up e-liquids’ were associated with the amount of cigarette per past 24 hours in case of daily smokers(OR 1.1, 95%CI 1.0-1.3, OR 1.1, 95%CI 1.0-1.2) and the degree of inhaling smoke or puff(OR 3.2, 95%CI 1.0-10.3, OR 3.8, 95%CI 1.3-11.3). Among total participants who had experience of smoking, high proportion(42.4%) of users were dual users, and they showed high total score of Fagerstrom Test for Nicotine Dependence(FTND)(p<0.0001) and higher total daily smoking amount. Conclusion : This study was conducted in Korea, where there was little research on the amount of nicotine consumed of e-cigarettes. It is meaningful as pilot study in Korea to understand the cigarette use behavior according to the smoking pattern(dual users, combustible cigarette only users, e-cigarette only users, past combustible cigarette smokers). Our results indicated that dual users consumed more total amount of consumed nicotine. If a smoker intended to reduce smoking or quit smoking with the help of substitute e-cigarette, our result showed on the contrary substitution induced increased amount of smoking. Our result recommended health educators to discourage the use of e-cigarettes for reducing or quiting smoking. They should promote and educate for dual users to raise awareness of e-liquids and nicotine and reduce exposure to high amount of nicotine. Future research should examine the total amount of nicotine consumed of dual users and e-cigarette only users longitudinally to determine patterns across time. Additional studies are also needed for follow-up studies considering the smoking behavior differences in electronic cigarette users(dual or only). 배경 및 목적 : 니코틴은 중독성을 유발하는 물질이다. 액상전자담배는 사용자가 니코틴 양을 조절할 수 있으나, 사용자의 흡연행태와 니코틴 용액 농도에 따라 니코틴 흡입 편차가 클 수 있다. 전자담배를 사용할 때 함유된 니코틴 용량을 파악하기 앞서, 전자담배 사용량 측정 방법은 확립되지 않았다. 이 연구에서는 액상전자담배 용액에 함유된 순수 니코틴 용량과 병당 사용기간을 고려한 총 니코틴 사용량 계산하여 인구사회학적 요인과 환경요인을 보정하여 한 달간 총 니코틴 사용량 및 전자담배 용액을 가득 주입한 후 다 떨어질 때까지 소요시간 관련 요인을 알아보고자 한다. 연구방법 : 식약처「대학생 담배제품 기능성 평가 연구(2015년)」 자료를 이용한 2차 분석연구이다. 서울, 경기, 경북, 경남, 부산, 충북, 대전, 전주, 광주, 제주에 위치한 14개 대학교에서 진행하고 남녀 대학생 설문에 참여한 2,387명 중 지금까지 담배를 한 두 모금이라도 피워본 여부에 ‘예’로 응답하고, 현재 궐련 흡연 여부(매일 피움, 가끔 피움, 과거 피움), 전자담배를 한번이라도 사용한 경험 여부(예, 아니오)에 모두 응답한 최종 759명을 연구대상으로 최종 분석하였다. 현재 궐련 흡연여부에 매일 또는 가끔 피운 자 중 전자담배를 한번이라도 사용한 경험이 있는 자를 궐련담배 및 전자담배 중복사용자(이하 “중복사용자”라 한다)로 정의하였다. 니코틴 의존도는 Fagerstrom test를 사용하였다. 통계 분석은 SAS version 9.4를 사용하였다. 연구결과 : 흡연 경험이 있는 최종 연구대상자 759명 중 중복사용자가 322명, 궐련담배 단독사용자가 162명, 전자담배 단독사용자가 92명으로 나타났다. 총 하루 평균 흡연량(‘궐련담배 흡연량(개비)’과 ‘전자담배 총 흡입횟수를 개비수로 환산한 사용량(개비)’의 합)은 궐련담배 단독사용자 및 전자담배 단독사용자에 비해 중복사용자가(남자 28.1개비, p=0.0037, 여자 16.7개비, p=0.0047) 가장 많이 피웠다. 지난 한 달간 액상전자담배의 총 니코틴 사용량의 경우 통계적으로 유의하지 않았지만, 중복사용자의 중위수는 30.0mg/month으로 전자담배 단독사용자의 중위수가 17.5mg/month에 비해 많이 사용하는 것으로 나타났다(p=0.3747). ‘총 니코틴 사용량 다량 사용’과 ‘전자담배에 용액을 가득주입한 후 다 떨어질 때까지 소요시간(일) 조기사용’은 ‘매일 흡연하는 경우 지난 24시간 동안 흡연량(개비)(OR 1.1, 95%CI 1.0-1.3, p=0.0406, OR 1.1, 95%CI 1.0-1.2, p= 0.0481)’과 ‘궐련담배 연기를 들이마시는 정도(OR 3.2, 95%CI 1.0-10.3, p=0.0496 OR 3.8, 95%CI 1.3-11.3, p= 0.0176)’에서 유의한 관련이 있었다. 흡연경험이 있는 대상자 중 중복사용자 비율(42.4%)이 컸고, 이들이 니코틴 의존도 검사 총점이 높고(p<0.0001) 총 하루 평균 흡연량도 많은 것으로 나타났다. 결론 : 본 연구는 한국인을 대상으로 전자담배 니코틴 사용량에 관한 연구가 거의 없는 실정에서 최초로 계산을 시도하였고, 흡연양상(중복사용자, 전자담배 단독사용자, 궐련담배 단독사용자, 궐련담배 과거흡연자)에 따른 담배사용행태를 파악한 선행연구로서 의의가 있다. 흡연자들은 금연 또는 흡연량을 줄이는 목적으로 전자담배를 사용하나, 오히려 총 니코틴 사용량과 흡연량이 많은 것으로 나타났다. 보건교육자는 전자담배를 금연 또는 흡연량을 줄이는 목적으로 사용하는 것을 줄일 수 있도록 안내가 필요하며, 중복사용자에게 고용량의 니코틴 노출을 감소하고 전자담배 액상과 니코틴에 대한 경각심을 가질 수 있도록 홍보 및 교육이 이루어져야 할 것이다. 추후 총 니코틴 사용량을 반복 측정하고 패턴을 분석할 필요가 있으며 전자담배 사용자의 담배 사용 시 행동적 차이를 고려한 후속 연구의 필요성이 크다.-
