Due to muscle balance impairment, brain lesion patients have high prevalence of Class II malocclusion, large overjet, lip incompetency, and anterior open bite. Although caregivers have high demand of orthodontic treatment, it is challenging due to lack of patient cooperation. Therefore, orthodontic treatment of such patients should be realistic and simple. This case report is on treatment of maxillary incisor protrusion using labial bow activation of retainer in a special needs patient. A 9-year-old patient with corpus callosum hypoplasia visited the Department of Pediatric Dentistry, Yonsei University Dental Hospital for orthodontic treatment counselling. Upon oral examination, maxillary incisor protrusion, large overjet, and lip incompetency were found. Hawley type retainer was set, and 4 weeks of adjustment period was given. U-loop was activated every 2-3 months. After 6 months, diastema closure, decreased overjet, and reduced incisor inclination were found. For relapse prevention, retainer was maintained, and periodic evaluation has been conducted.-
