소아 로타바이러스 위장관염의 최근 임상적 특성

dc.description.abstractPurpose: Rotavirus is an important cause of childhood gastroenteritis as before, but there are only a few domestic reports for it since the introduction of its vaccination. I did this study to know update clinical features of rotavirus gastroenteritis (RVGE). Methods: I collected the clinical informations of RVGE patients who had been admitted at the Department of Pediatrics, Yogin Severance Hospital from January 2013 to December 2015. Rotavirus antigen from stool was detected with an immunochromatographic assay. Their data were investigated and compared by year. Results: There were 42 RVGE cases for 3 years. Thirteen cases (31.0%) were found in April. Their mean age was 56.1±45.5 months old. Sixteen cases (33.3%) were 5 years old and over. Their common symptoms were vomiting (83.3%), diarrhea (83.3%), poor feeding (71.4%), fever (66.7%). Their mean admission duration was 3.3±1.7 days. Low serum bicarbonate, hypertransaminasemia, ketonuria, hyperuricemiawere found in 41 cases (97.6%), 32 cases (76.2%), 28 cases (71.8%) and 20 cases (47.6%), respectively. Patients in 2013 (group A, 48.8±46.3 months old) looked younger than patients in 2014(group B, 62.0±49.3 months old) and 2015 (group C, 67.3±37.4 months old). The mean admission duration of group A (3.8±1.8 days) was longer than that of group B (2.3±0.9 days; P =0.031). The proportion of patients with prolonged diarrhea after admission (≧2 days) was higher in group A (55.6%) than group B (0%; P =0.010). The mean serum aspartate transaminase was higher in group A (29.6±14.1 IU/L) than group C (20.1±6.2 IU/L; P =0.027). There was no significant clinical difference between group B and group C. Conclusion: April is the most prevalent month of RVGE. The patient age becomes older. Rotavirus infection still can result in severe gastroenteritis although recent prognosis and clinical features of RVGE look to be better by year. That should be noticed for physicians and confirmed with further and multi-hospital based studies.-
