dc.description.abstract | 전립선암은 이질적인, 다양한 위험성과 여러 종류의 치료 방법이 존재한다. 초고위험 전립선암의 치료 방법을 정할 때 의료진은 환자의 동반 질환을 반드시 고려해야 한다. 본 저자는 Age adjusted Charlson Comorbidity Index (ACCI)를 초고위험 전립선암에서 근치적 전립선제거술 시행 후 예후인자로써의 효용성을 조사하였다. 저자들은 본 기관에서 1992년부터 2010년까지 근치적 전립선 제거술을 시행받은 환자의 의무기록을 후향적으로 조사하였다. 초고위험 전립선암은 National Comprehensive Cancer Network 가이드라인에 따라 정의하였다. 대상환자 중 술전 보조요법이나 의무기록이 불충분한 자는 제외하였다. 동반 질환은 ACCI에 따라 분류되었다. ACCI의 예후인자로써의 효용성은 단변수 및 다변수 Cox 회귀 분석, Competing risk 회귀분석 및 Kaplan-Meier 곡선에 의해 분석되었다. 최종 분석은 228명의 환자를 포함하였으며 연령의 중앙값은 66세였다. 전립선 특이 항원의 중앙값은 10.7 ng/ml이었으며 41명 (18%) 환자의 CCI가 4이상이었다. 8년 전립선암 특이 생존율은 91.6%였으며 전체 생존율은 83.4%로 나타났다. Competing risk 회귀 분석에서 CCI는 비 전립선암 특이 생존율 및 전체 생존율과 유의미한 상관관계를 보였다 (p<0.05). ACCI는 초고위험 전립선암에서 비 전립선암 특이 생존율과 전체생존율 예측에 있어 효과적인 예후인자였다. 근치적 전립선절제술은 CCI가 4이상인 환자에서 신중하게 고려되어야한다. | - |
