dc.description.abstractRecently, the prolene hernia system (PHS) has become a commonly accepted technique. We introduce a balloon method to effectively dissect the preperitoneal space for insertion of the underlay during the PHS hernioplasty. The procedure for dissection of the hernial sac and the spermatic cord is proceeded as the conventional method. The dissected hernia sac is reducted through the inguinal ring. Then, the balloon is introduced into the preperitoneal space and 500 ml of normal saline is insufflated. The preperitoneal space is automatically exfoliated as a spherical space of approximately 12 cm in diameter. After draining the saline from the balloon, the balloon is removed. Both the underlay and onlay of the PHS is placed. This method allows for an easy and consistent gain of space regarding the effective placement of the underlay in the preperitoneal space and is also supposed to reduce the risk of bleeding through rough manual dissection.-
