사람치수 임파모세관의 미세구조에 관한 연구

Abstract
[한글]치수내혈관분포의 대부분은 모세혈관이 차지하며 이에 대하여는 많은 연구자들에 의해 그 미세구조가 잘 알려져 있다.

그러나 dental region의 임파순환에 대하여는 많이 알려져 있지 않으며, 특히 치수내임파모세관에 대하여는 여러 학자들에 의해 일찍이 연구가 시작되었음에도 불구하고 임파모세관의 존재자체가 아직도 논난중이며, 현재 편찬되어 있는 각종 교과서 등에도 매우 간략하게 기술되어 있을 뿐 아니라 일부 학자들은 아직도 치수내 임파관의 존재에 의문을 표시한다.

저자는 사람의 정상 및 우식치아를 발치하여 전자현미경으로 치수를 관찰하여 임파모세관의 존재와 정상치아와 우식치아에서의 미세구조상의 차이를 밝히고저 시도하였던 바, 다음과 같은 결론을 얻었다.

정상 및 우식치아치수에서 임파모세관이 관찰되었다. 정상치아치수의 임파모세관은 다른 장기에서 나타나는 것과 유사한 일반적 특징을 보였다. 그러나 다른 장기의 것과 다른 특징은 많은 dense body와 외막세포가 출현하는 것이었다. 우식치아치수에서는 내피세포의 벽이 얇아져 있고, 핵과 세포질돌기가 더욱 불규칙한 양상을 띄었다. 아울러 dense body의 수가 감소되었으며 과립내형질세망이 다소 팽대된 듯 한 양상을 띠었고 크기와 수가

많이 증가된 소포가 관찰 되었다.

전자밀도가 다른 두가지 종류의 내피세포가 관찰되었다.

이상의 결과로 미루어 치수내에서 확실히 임파모세관의 존재를 인정할 수 있었으며, 정상치아와 우식치아의 임파모세관의 미세구조는 여러 가지 형태학적인 차이를 볼 수 있었다.



[영문]Blood capillaries constitute a predominant part of the vascular supply in the human dental pulp, and their ultrastructure are very well known by many investigators. The lymphatic circulation in the dental region has not been understood fully yet, and moreover the existence of pulpal lymphatic capilaries are

still the subject of controversy.

One of the factors responsible for the prevailing controvery has been the technical difficulty in demonstrating the pulpal lymphatic capillaries.

Another factor that has further confused investigators is the difficulty in differentiating the lymphatic capillaries from the blood capillaries by their close morphological resemblance.

Presumed lymphatic capillaries have been described in the pulpal tissue by Riedel et al. (1966), Kukietova (1970b) and Dahl & Mjor(1973) on the base of thin walled vessels with large opening in the endothelial lining.

Frank et al. (1977) agreed on these findings by the study of inflammed human dental pulp.

But there is still a lack of agreement whether or not lymphatic capillaries actually are present in the pulpal tissue.

The author attempted to demonstrate the existence of pulpal lymphatic capillaries and their morpholgical alterations in the sate of the normal & decayed condition which may be present. This study was done by electron microscope using the human tooth.

Obtained results are as follows;

1) It can be concluled that typical pulpal lymphatic capillaries are indeed present.

2) The ultrastracture of the lymphatic capillaries in the normal pulp is generally similiar to that of other organs, but the pulpal lymphatic capillaries alone have a pericytes & numerous dense bodies in the cytoplasms of endothelial cells.

3) The lymphatic capillaries in the pulp of decayed teeth showing many more irregularities than the normal, in the form of nucleus, thickness & processes in cytoplasm and in many cellular organells (changes in number, size and morphology).
