지역정신보건센터의 서비스 우선순위와 표준 업무수행방식에 대한 연구

우리나라는 1995년 정신보건법의 제정과 지역정신보건센터의 설치를 기점으로 지역사회 정신보건사업을 시작하게 되었다. 그동안 센터의 수는 증가하였고 다양한 프로그램이 제공되어왔지만, 정신보건센터의 역할 설정에 대해서는 점차 그 논란이 가중되어 오는 실정이다. 이에 사업의 가치 및 이념에 대한 공감대 형성과 더불어, 정신보건문제 및 사업목표에 따른 객관적 우선순위 선정이 필요하고 미래 지향적 과업을 고려한 정신보건센터의 정체성 확립이 필요한 시기라고 할 수 있다.본 연구는 델파이 기법을 활용하여 공공정신보건의 가치 실현과 목표 달성을 위해서 중요한 축으로서 기능하고 있는 정신보건센터의 서비스 우선순위와 표준적 업무체계에 대해서 조사해 보고자 하였다. 지역정신보건사업에 상당한 전문적 지식을 가지고 있는 40명의 전문가를 대상으로 두 차례에 걸쳐 델파이 설문지를 작성하게 하였고 이를 분석하였다.본 연구에서 나타난 주요 결과를 요약하면 다음과 같다.첫째, 만성중증정신질환자 대상 사업의 성장이 다른 영역의 성장보다 앞서야 하나, 만성정신질환을 포함하는 중증정신질환자 관리사업의 질적 향상과 상대적 비교우위를 유지하는 전제하에 다른 영역의 사업비중도 점차 증가해야 한다는 결과를 보이고 있음.둘째, 정신보건센터의 업무수행방식 중 우선순위가 높은 항목들은 다음과 같다.- 초기정신질환자. 우울증, 소아청소년, 노인정신질환에 대한 스크리닝 및 일반상담- 만성정신질환자 및 초기정신질환자, 우울증 및 소아청소년에 대한 전문적 사례관리 서비스- 만성정신질환, 초기정신질환, 소아청소년, 우울증 및 자살, 노인정신 건강 영역에 대한 지역사회 교육 및 자문- 자살고위험군 및 중증정신질환에 대한 응급관리- 인식개선과 사회통합을 위한 홍보 및 지역사회 조직 활동본 연구 결과는 향후 국가정신보건서비스 가이드라인으로 활용될 수 있을 것으로 기대한다. 이를 통해 정신보건센터의 표준적서비스 체계가 구축됨으로서 보편타당성을 획득함과 동시에, 지역사회의 다양한 욕구 충족 방안을 수립 실천함으로서 적절한 정신보건 서비스를 제공할 수 있을 것으로 기대한다.

[영문]In Korea, community mental health center(CMHC) is an important infrastructure of national mental health service system and is expected to play a major role for the prevention, promotion, management and rehabilitation of mental illness.Although everyone can agree with the importance of CMHC as an key unit of national mental health system, there are many debates about what and how the CMHC should do. So we need the consensus about the direction and service standard of CMHC in the near future.Therefore, the objective of this study was to define service priority and standard performance of CMHC.This study was carried out by the delphi method. 40 experts of community mental health were participated in this study.The major results are as follows ;1) The future directions and service priority of CMHCBased on a general increase and strengthening the quality of mental health services to serious mental illness, we can expand to other candidates such as child & adolescent, depression, aged people and so on with rational priority setting process.2) Standard performancea. screening and primary mental health services (eg; counseling & providing information) for early psychosis, child & adolescent, depressed people and geriatric mental health problemsb. specialized mental health services(case management) for the serious mentally ill people, depressed people, child and adolescentc. education and consultation for the serious mental illness, child and adolescent, depression and suicide high risk groupd. crisis intervention for the high risk group of suicide and serious mental illnesse. public relations and community organization for the improvement of attitude toward mental illness and for the reintegration of mental illness patients into societyWe expect these results can be applied to national mental health service guideline for the standardization of community mental health centers.
