고혈압 환자의 운동 습관과 허혈성 심질환 발생 위험도와의 관련성

dc.description.abstract[한글] 본 연구의 목적은 단면적인 연구로 허혈성 심질환 발생예측모형을 통하여 고혈압 환자에게 있어 운동습관과 허혈성 심질환 발생 위험도와의 관련성을 확인하기 위함이다. 연구대상자는 2000년 1월부터 2004년 10월 15일까지 Y 의료원 건강검진센터를 내원하여 문진, 의학적 진단, 임상적 검사, 운동능력 검사를 하고 JNC & WHO-ISH의 Criteria에 준하여 Stage 1-3 고혈압 진단을 받은 30세 이상의 남여, 총 2,662명이다. 본 연구에서는 고혈압 환자를 대상으로 운동 습관에 따라 운동군, 저운동군, 비운동군으로 분류하여 세 군 간에 허혈성 심질환 위험요인들을 비교하고, 또한, 허혈성 심질환 발생 위험도와의 관련성을 확인하고자 하였다. 통계 분석은 SAS Ver. 8.2를 사용하였으며. 연구 결과는 다음과 같다. 첫째, 고혈압 대상자들의 운동시행정도에 따라 임상적 신체검사와 혈액검사를 비교한 결과, 운동군은 비운동군에 비하여 적혈구, 헤모글로빈, 헤마토크리트는 유의하게 높았으며, 혈소판과 Alanine transaminase(ALT)는 유의하게 낮았다. 신체조성에 있어서는 BMR은 유의하게 낮았고, Lean mass는 유의하게 높았다. 둘째, 허혈성 심질환의 주요 위험요인인 혈압, BMI, 혈당에 대하여 운동군은 비운동군에 비하여 수축기압, 이완압, BMI, glucose는 유의하게 낮았으며, Lipid profile에 있어서는 중성지방과 Atherogenic index(AI)가 유의하게 낮았다. 셋째, 미국의 Framingham 위험 점수로 허혈성 심질환 발생 위험도를 구한 결과, Framingham point score와 10-year risk(%)가 여성 집단에 있어 운동군이 비운동군에 비하여 유의하게 낮았다. 넷째, 허혈성 심질환 발생예측모형(Health risk appraisal)을 통하여 10년 이내에 허혈성 심질환이 발생할 위험도를 구한 결과, 10-year risk(%)가 여성집단에 있어 운동군이 비운동군에 비하여 유의하게 낮았다. 이상의 연구결과를 종합하여 보면, 운동군은 저운동군과 비운동군에 비하여 허혈성 심질환 위험인자가 유의하게 낮다. 여성에 있어 운동시행정도에 따라 Framingham 위험점수와 한국인을 대상으로 개발된 허혈성 심질환 발생 예측모형을 이용하여 추정한 허혈성 심질환 위험도는 운동군이 비운동군에 비하여 위험도가 유의하게 낮았다. 따라서, 허혈성 심질환의 예방을 위하여 고혈압 여성 집단에 있어 어떤 종류의 육체운동을 어느 수준의 강도와 시간동안 하게 하는 것이 안전하고 효과가 있는지를 확인하는 심층연구가 앞으로 수행되어야 할 것으로 생각한다. 핵심어 : 허혈성 심질환, 위험인자, 운동, Framingham 위험점수, 허혈성 심질환 발생 위험도 [영문]The management of risk factors of coronary heart disease is highly needed because the prevalence of ischemic heart disease (IHD) is increasing in Korea. The objectives of this study are to investigate the associations of exercise habit on risk factors of coronary heart disease (CHD) using Framingham Point Score and individualized Health Risk Appraisal (HRA) in patients with hypertension. This study included 2,662 hypertension patients aged 30-80 who visited "Y" University Hospital for health screening from January 1991 to October 2004 without CHD. Subjects were divided into three groups, sedentary, inadequate and adequate by physical exercise habit. We surveyed medical history of the subjects, and measured their blood pressure, serum glucose and lipid profile. Risk points from the Framingham chart were given to age, systolic BP, total-C, HDL-C, smoking, diabetes, and the global risk score was calculated from their sum. We analyzed the Framingham Risk Scores and each HRA risk point among the exercise groups. Exercise group showed significantly different diastolic BP, RBC, hemoglobin, hematocrit, platelet, Uric acid, BMI, BMR, Lean mass, glucose and triglyceride compared with sedentary group. The mean values of the Framingham Risk Point Scores were significantly higher in females, but not in males. The mean values of HRA model of IHD were significantly higher in the sedentary group than in other physically more active groups among females, but not among males. The physically inactive group showed significantly higher levels of cardiovascular risk factors compared with physically more active female hypertension patients. In conclusion, it is well recommended that we need further indepth researches to determine whether physical activity is beneficial to prevent CHD particularly in Korean females.
