치과건강보험 청구 실태조사

Abstract

[한글]본 연구는 치과의원에서 건강보험청구업무를 담당하는 청구자의 태도 및 특성을 파악하고 그에 따른 적절한 건강보험교육을 실현함으로서 우리나라 치과건강보험 청구 업무에 기초 자료로 제공하는 것을 목적으로 한다.연구대상은 크게 두 가지로 나뉘는데,첫째, 설문조사 자료는 2004년 대한치과의사협회 회원명부에 등재된 치과진료기관을 대상으로 하여 각 지역(광역시, 도, 군)별로 서울 및 경기지역을 40%, 그 밖의 지역을 60%로 임의 표본추출을 통하여 선정하였다. 580개 치과의원에 대해 우편을 발송하였고 120개 기관을 방문, 총 700개의 치과병의원을 조사한 결과 195개 치과의원에서 응답하였으며 그 중 전국치과의원 자료와 연계가 가능한 치과의원은 109개소였다.둘째, 건강보험심사평가원에 요청하여 입수된 전국 치과의원 자료는 전국치과의원 중 EDI청구가 이루어지고 있는 치과의원으로서 11,643개 치과의원의 자료이다.설문조사는 2005년 4월 3일부터 5월 10일까지 진행되었으며, 조사방법으로는 우편과 통신 및 직접방문에 의한 자기기입식 설문조사로 이루어 졌다.데이터는 spss var 10.0 program을 사용하여 분석하였으며 연구의 결과를 요약하면 다음과 같다.첫째, 청구자별(치과위생사와 그 외 인력) 삭감률의 비교에서 여러 인격(치과의사, 간호조무사, 행정직원 등)에 의한 청구 보다 치과위생사에 의한 청구 시 삭감률이 작은 것으로 나타났다.둘째, 이전 청구의 삭감내역을 반영하는 사람의 삭감률이 반영하지 않은 사람보다 더 낮은 것으로 나타났다. 삭감내용을 보면 진료 분야 중 치주진료부문, 진료항목에선 행위료의 적용에 있어 많은 삭감이 있다. 삭감사유는 진료에 따른 챠트 내용이 심사원칙에 맞지 않아서 삭감되는 것이 가장 높게 나타났다.셋째, 건강보험청구교육과 관련하여 교육을 받은 자와 받지 않은 자 사이의 삭감률에 차이가 있었다. 교육을 받게 된 동기는 본인의 필요 또는 원장의 요구에 의한 경우가 많았다. 교육은 주로 치과관련협회의 교육을 이수하였으며, 교육에 대한 만족도는 대부분이 만족하는(약 60%)것으로 나타났다.넷째, 개원 기간에 따른 삭감결과에선 개원기간이 길수록 삭감률이 감소된 것으로 나타났다.이러한 연구 결과를 토대로 볼 때 치과에서 건강보험청구업무의 개선을 위해서는 청구자의 끊임없는 노력과 정진하는 자세가 필요하고 치과진료의 철저하고 정확한 기록, 그리고 청구 작업을 보다 쉽게 접근할 수 있기 위한 용이한 프로그램의 개발과 실무에 적합한 교육이 이루어져야 할 것으로 사료된다.

[영문]The purpose of this study is to provide a basic understanding of the Korean Dental Health Insurance Program define the persons responsible for actions within the program; provide an in-depth characterization of the specific differences across the Dental Profession; and recommend ways to improve the education of Dental Health Professionals thereby improving the Dental Health System.There were two groups that participated in the surveys for this study. The samples for the first group were selected from dental agents who were listed as members of the Korean Dental Association (KDA) in 2004. Of those chosen, 40% were from the Seoul and Kyong-gi area and 60% from remaining cities, provinces and districts throughout Korea. I surveyed 580 dental clinics and hospitals by mail and visited 120 dental agents for a total of 700. Of those surveyed, there were 195 responses and only 109 clinics were related to the whole of Korea. The second sampling was taken from a request to HIRA for 11,643 dental clinics that use the EDI demand system.The survey was conducted from April 3 to May 10, 2005. Those surveyed were required to fill out the survey on their own and responded by mail and visit.The data for the surveys was analyzed using a spss var 10.0 program. Results from the summary are as follows.First, it showed a lesser differential in the demand versus payment for dental hygienists than for other dental professionals (dentists, assistants, officers, etc).Second, the groups that provided details from previous months showed less of a differential from those that did not provide the details. There were many cases where the service tax remained unpaid for periodontal treatment. This is a result of a lack of details in dental records that does not meet the rules of inspection.Third, it showed a lesser difference in demand versus payment between the groups that received an education in health insurance demand versus those that did not. There were many cases of motivated individuals pursuing this education on their own or had it provided by their dentist. Most completed their education from the Dental Association and they were satisfied with 60% of the training.Fourth, the dental clinics that have been open longer showed less of a differential in demand versus payment than other newer clinics.Based on all of the results I concluded that we need to devote greater effort to improving the dental health insurance program in the field. This is accomplished by having more detailed dental records and developing better educational programs for our dental professionals regarding the dental health insurance program.
