Triamcinolone(corticosteroid제제)투여가 백서 골격근에 미치는 영향에 관한 실험적 연구

Corticosteroid의 광범위한 이용에 따라 이의 장기사용에 의한 각종 부작용이 중요한

연구과제가 되고있다. Corticosteroid의 부작용으로 골격근에 초래되는 steroid myopathy

는 내인성 및 외인성 hypercorticism에 의한 다수의 임상증례가 보고되고, 이를 토대로

한 실험적 연구결과가 발표되고 있으나 그 발생기전에 대하여는 아직도 논란이 많다. Cor

tixosteroid가 골격근에 미치는 형태학적 변화에 관하여도 보고자에 따라 크게 차이가 있

고 steroid투여량이나 투여기간에 관하여도 기술된 바가 회유하며 steroid 투여를 중단하

면 가역적인가에 대하여도 연구된 바가 없다. 따라서 corticosteroid가 근육에 미치는 영

향과 투여량 및 투여기간과의 관계를 규명하고 steroid myopathy의 기전을 규명코자 본

실험을 시행하였다.

실험동물로는 체중 250gm내외의 웅성백서 72마리를 사용하였으며 이를 정상대조군, ste

roid투여후 즉시도살군, steroid투여후 4주간 회복기간을 둔후 도살군의 3군으로 구분하

여 실험하였다. Steroid제제로는 E.R. Squibb & Sons회사제인 triamcinolone을 매일 1회

씩 상지에 근육주사 하였으며 투여용량에 따라 2mg/kg 투여한 대량투여군, 0.5mg/kg투여

한 중등량투여군, 0.125mg/kg 투여한 소량투여군으로 소분하였으며 각 실험소군을 다시 2

분하여 1주간 투여군과 3주간 투여군으로 재분하였다. 도살직전 채혈하여 혈청 triglycer

ide와 total cholesterol치 및 CPK와 LDH치를 측정하였고 도살시에 양측 soleug근을 제거

하여 liquid nitrogen에 15초간 동결한 다음 근육의 정중앙부위를 10μ의 두께로 연속절

편을 만들었다. 연속절편으로는 근육의 일반적 변화를 보기위한 hermatoxylin-eosin염색

과 지질침착을 보기위한 sudan black B염색을 시행하였으며 근세포의 종류를 식별하기 위

한 sudan black B-PAS의 복합염색과 ATPase(pH 4.3 및 10.3)염색을 시행하였고, 전자현미

경적 관찰을 위하여는 통상적인 방법을 사용하였다.

실험결과를 요약하면 다음과 같다.

1. Triamcinolone을 대량투여하면 실험동물의 사망율이 높았고 생존한 동물에서는 근세

포와 팽대와 급성과사 및 지질축적등이 일어났으며 이러한 결과 혈청내 CPK 및 LDH의 증

가를 초래하였다.

2. Triamcinolone을 중등량이나 소량으로 투여한 동물에서는 근섬유의 괴사는 초래하지

않았으나 근세포의 위축 및 지질축적이 일어났으며 이러한 변화는 약물투여기간과 비례

하였다. 그러나 혈청 CPK 및 LDH의 변동은 일어나지 않았다.

3. Triamcinolone투여에 의한 근섬유의 손상은 근섬유 전반에 걸친 일률적인 것이 아니

고 근섬유 일부에서 관찰되었으며 PAS염색상 중등도 반응세포에서, ATPase염색상 type Ⅱ

섬유의 일부에서 일어났다.

4. 미세구조상 triamcinolone의 대량투여는 주로 근섬유의 파괴를 보였고 중등량투여는

mitochondria의 증식과 근막주위로의 밀집현상 및 지질침착을 일으켰다.

5. Triamcinolone의 지속적인 대량투여는 동물을 사망케 하였고 중등량이하에서는 약물

투여를 중지할 경우 형태학적 변화들이 대부분 원상태로 회복되었다.

이상의 결과로 보아 corticosteroid제제에 의한 근손상은 일부특정 근섬유에 선택적으

로 일어나며 형태학적으로 주로 mitochondria의 변성, 증식과 밀집 및 지질침착등의 과정

을 거쳐 일어나고 대량을 투여하면 myofilament의 파괴까지 초래하여 근섬유괴사를 유발

하게되나, 이러한 변화는 약물투여중지로서 회복되는 가역성변화로 생각된다.





An Experimental Study of the Effect of Triamcinelone (corticosteroid) on the

Skeletal Muscle of the Rats

The widespread and prolonged use of corticosteroid compounds in the treatment of

various conditions causes deleterious effecta in the number of vital organs. Among

various side effects of a long-term use of corticosteroid, muscle atrophy and

weakness have been implicated as one of the major complications affecting the

skeletal muscle. However, the exact pathogenetic mechanism of the steroid myopathy

remains controversial, in spite of increasing clinical and experimental reports on

steroid myopathy due to either exogenous or endogenous hypercorticism. The results

on the histologic studies of the steroid myopathy are different according to

investigators, ranging from no pathologic changes to fiber necrosis, regeneration

and muscle atrophy. The studies of reversibility of the morphological changes and

the relationship between the dosage or duration of the corticosteroid treatment and

steroid myopathy have not been studied well experimentally. Therefore, the present

study is aimed to elucidate the relationship between the dosage and duration of the

corticosteroid administration and the nature of steroid myopathy by morphological

studies.

A total of 72 white male rats were used for the experiment, and divided into

three groups; control(Ⅰ), triamcinolone treated and sacrificed immediately(Ⅱ),

and triamcinolone treated and sacrificed 4 weeks later(Ⅲ). Triamcinolone treated

groups were divided into three subgroups; large dose, 2 mg/kg; medium dose, 0.5

mg/kg; and small dose, 0.125 mg/kg. Animals of triamcinolone treated group were

injected triamcinolone, an intermediate acting glucocorticoid, once every day for 1

and 3 weeks. Animals were sacrificed immediately after 1 and 3 weeks treatment of

triamcinolone in group Ⅱ, and 4 weeks later in group Ⅲ. At the time of sacrifice,

blood samples were obtained to measure serum triglyceride, total cholesterol, CPK

and LDH. For the morphologic examinations, bilateral soleus muscle were removed.

The one side of midportion of soleus muscle was freezed with liquid nitrogen for 15

seconds and serial frozen sections were prepared. Frozen section were stained

routinely with hematoxylin-eosin to examine histological changes of muscle cells

and sudan black B to evaluate fat deposit in the muscle. The sudan black B-PAS and

ATPase(pH 4.3 & 10.3) staining were made to differentiate the types of muscle

fibers. From another side of the soleus muscle, a portion was prepared for the

electron microscopic observation by routine procedure:

The results are as follows.

1. Administration of a large dose of triamcinolone resulted in a high mortality

rate of the experimental animals and the survived animals showed acute necrosis,

ballooning and lipid accumulation in muscle cells accompanied by elevation of serum

CPK and LDH.

2. Administration of a medium dose of triamcinolone resulted in an atrophy of

muscle cells and lipid accumulation in parallel with the duration of corticosteroid

administration, but no alteration of serum CPK or LDH was noted.

3. Histological alterations of the muscle fibers were observed only in certain

type of musclefibers, which was proved to be intermediate intensity fibers when

stained with PAS and a part of type Ⅱ fibers by ATPase stein.

4. Ultrastructually, a large dose of triamcinolone induced lysis of myofibrils

and degradation of organelles, but medium dose of triamcinolone induced

degeneration, proliferation and aggregation of mitochondria associated with lipid

accumulation.

5. Morphologic alterations of the muscle fibers induced by triamcinolone

treatment were reversible as evidenced by almost complete recovery in 4 weeks.

Based on these results, it is concluded that muscle injury induced by

corticosteroid compounds is selective to certain type of muscle fibers, probably

initiated by mitochondrial changes such as degeneration, proliferation and lipid

accumulation, leading to acute necrosis, but reversible following the cessation of

drugs.
