S.pneumoniae는 1960년대까지 penicillin에 대해 높은 감수성을 유지해왔다 그러나 198

0년대에 들어 세계 전 지역에서 penicillin에 대해 내성인 균주의 증가가 보고되기 시작

했다. 우리나라에서도 최근 내성 S.pneumoniae가 증가되고 있음이 보고되고 있으나, 자료

가 많지는 않다. 본 연구에서는 1989년부터 1992년까지 임상검체에서 분리한 124주의 S.p

neumoniae를 한천희석법으로 항균제 감수성을 시험하였고, 아울러 59주의 S.pyogenes와 8

7주의 S.agalactiae의 감수성을 시험하여 비교하였다. 그 결과는 다음과 같다.

1. S.pneumoniae는 penicillin G에 71.0%, cefaclor에 65.4%, cefadroxile에 62.1%, di

rithromycin에 20.2%, clindamycin에 17.0%, erythromycin에 15.5%, chloramphenicol에 2

.4% 그리고 telracycline에 54.8%의 내성율을 보였고, cefepime, cefotaxime, imipenem,

ofloxacin, teicoplanin 그리고 vancomycin에는 모든 균주가 감수성이었다.

2. S.pyogenes는 dirithromycin에 1.7%, erythromycin에 1.7%, tetracycline에 39.0%의

내성율을 보였고, penicillin G, cefaclor, cefepime, cefotaxime, imipenem, clindamyc

in, chloramphenicol, ofloxacin, teicoplanin 그리고 vancomycin에는 모든 균주가 감수

성이었다.

3. S.agalactiae는 penicillin G에 1.1%, dirithromycin에 4.6%, clindamycin에 2.3%,

erythromycin에 6.0%, tetracycline에 89.7% 그리고 ofloxacin에 1.1%의 내성율을 보였고

, cefaclor, cefepime, cefotaxime, imipenem, chloramphenicol, teicoplanin 그리고 van

comycin에는 모든 균주가 감수성을 보였다.

이상의 성적으로 S.pneumoniae의 penicillin G, cefaclor, cefadroxile tetracycline에

대한 내성균이 많고, S.pyogenes중에는 penicillin G에 내성인 균주가 없으며, S.agalac

tiae중에는 1균주만이 penicillin G에 내성이고 tetracycline을 제외한 다른 항균제에 대

한 S.pyogenes와 S.agalactiae의 성율은 낮다는 결론을 얻었다.

Streptococcus pneumoniae has retained a high degree of susceptibility

penicillin(MIC, 0.005-0.01 μg/ml) until 1960s. However, during the 1980s reports

on the increased isolation of penicillin G-resistant strains started to appear in

the literature from many parts of the world. In Korea, as the reports on the

susceptibility of S.pneumoniae started to appear onl recently, our knowledge on

this matter is very limited at the moment.

In this study, the activities of various antimicrobial agents 124 strains of

S.pneumoniae, isolated during 1989-1992, were tested by agar dilution method. For

comparison, the activities against 59 isolates of S.pyogenes and 87 isolates of

S.agalactiae were also tested.

The results were as follows :

1. The resistance rates of S.pneumoniae were penicillin G, 71.0% cefaclor, 65.4%;

cefadroxile, 62.1%; dirithromycin, 20.2%; clindamycin, 17.0%; erythromycin, 15.5%;

chloramphenicol, 2.4%; tetracycline, 54.8%; All isolated strains were susceptible

to cefepime, cefotaxime, imipenem, ofloxacin, teicoplanin and vancomycin.

2. The resistance rates of S.pyogenes were dirithromycin, 1.7% erythromycin,

1.7%; tetracycline, 39.0%. All isolated strains were susceptible to penicillin G,

cefaclor, cefepime, cefotaxime, imipenem, clindamycin, chloramphenicol, ofloxacin,

teicoplanin and vancomycin.

3. The resistance rates of S.agalactiae were penicillin G, 1.1%; dirithromycin,

4.6%; clindamycin, 2.3%; erythromycin, 6.0%; tetracycline, 89.7%; ofloxacin, 1.1%.

All isolated strains were susceptible to cefaclor, cefepime, cefotaxime, imipenem,

chloramphenicol, teicoplanin and vancomycln.

It is concluded that among the S.pneumoniae strains isolated recentl from

clinical materials in Korea are often resistant to penicillin G, tetracycline and

the first generations cephalosporins, while S.pyogenes an S.agalactiae remain

highly susceptible to penicillin G and other antimicrobial agents excluding

tetracycline.
